Palliative and End-of-Life Care in a Small Caribbean Country: A Mortality Follow-back Study of Home Deaths.

CONTEXT: Empirical information on circumstances of dying from advanced illness in developing countries remains sparse. Evidence indicates that out-of-hospital end-of-life care can have significant benefits such as increased satisfaction for the patient and caregivers and cost-effective for a health-care system. Services that are aimed to deliver care at private homes may be a good model for low- and middle-income countries or other low-resourced settings. OBJECTIVES: To examine specialized, generalist, and informal palliative care provision and to describe the end-of-life care goals and treat

Financial incentives for quitting smoking in pregnancy: Are they cost-effective?

AIMS: To evaluate whether adding financial incentives to usual care is cost-effective in encouraging pregnant women to quit tobacco smoking, compared with usual care alone. DESIGN: Cost-effectiveness analysis (CEA) and cost-utility analysis (CUA) from a health-care provider's perspective, embedded in the Smoking Cessation in Pregnancy Incentives Trial (CPIT III). Long-term analyses were conducted from the same perspective, using an existing Markov model over a life-time horizon. SETTING: Seven maternity smoking cessation sites in Scotland, England and Northern Ireland in the United Kingdom.

Building multi-professional UK partnerships and networks to improve access to palliative care for people experiencing homelessness.

BACKGROUND: People experiencing homelessness have high rates of multi-morbidity and age-related conditions at a young age. Despite having high support needs, they have disparately low access to palliative care services and often die at a young age. To facilitate access to support for this group towards the end of life, a multi-professional approach should be taken. Over recent years, clinical and research activities have begun to address this issue. However, until now, there has been no centralised United Kingdom-based group to facilitate collaboration and shared learning. AIM: To build multi

Supportive care needs of adults living with a peripherally inserted central catheter (PICC) at home: a qualitative content analysis.

BackgroundPeripherally inserted central catheters (PICCs) are common vascular access devices inserted for adults undergoing intravenous treatment in the community setting. Individuals with a PICC report challenges understanding information and adapting to the device both practically and psychologically at home. There is a lack of research investigating the supportive care needs of individuals with a PICC to inform nursing assessment and the provision of additional supports they may require to successfully adapt to life with a PICC. The aim of this study was to identify the supportive care need

Palliative and end-of-life nursing care in Saudi Arabia: A systematic review of nursing practices, challenges, and patient-family outcomes.

BACKGROUND: Palliative and end-of-life (EOL) care is gaining increasing importance in Saudi Arabia due to the rising burden of chronic and life-limiting illnesses. Nurses play a central role in delivering comprehensive, culturally appropriate palliative care; however, their practices are influenced by educational preparation, institutional support, and sociocultural and religious contexts. To date, evidence on palliative nursing care in Saudi Arabia remains fragmented and insufficiently synthesized. AIM: This systematic review aimed to synthesize existing evidence on palliative and EOL nursin

Quality of life dynamics in the gastric cancer dyad: an actor-partner interdependence model analysis guiding the development of a family-centered nursing framework.

PURPOSE: To explore the mechanisms linking quality of life (QoL) between postoperative GC patients and their family caregivers, and to construct a family-centered nursing framework. METHODS: 353 patient-caregiver dyads were recruited. Based on a validated theoretical framework, we analyzed factors influencing QoL and used the Actor-Partner Interdependence Model (APIM) to examine bidirectional pathways. RESULTS: Patient and caregiver QoL were positively correlated (r = 0.287, p < 0.001). Actor effects: Patient QoL was positively predicted by patient health literacy, benefit finding, and posit

Caregiving for Adults With Down Syndrome: Caregiver Experiences and Support Needs.

BACKGROUND: Family caregivers of adults with Down syndrome often provide life-long caregiving support for their loved one. Long-term caregiving can impact caregivers' health and well-being, yet their experiences and support needs are underexplored. METHOD: Semi-structured interviews were conducted with caregivers of adults with Down syndrome to understand their caregiving experiences and perceived caregiver support needs. Transcripts of the recorded interviews were coded and analysed thematically. RESULTS: Seventeen family caregivers (94.1% female, Mage = 58.8 years) of adults with Down synd

Systematic review of caregiver burden, unmet needs and quality-of-life among informal caregivers of patients with pancreatic cancer

Abstract Purpose Informal caregivers play an important supportive care role for patients with cancer. This may be especially true for pancreatic cancer which is often diagnosed late, has a poor prognosis and is associated with a significant symptom burden. We systematically reviewed the evidence on caregiver burden, unmet needs and quality-of-life of informal caregivers to patients with pancreatic cancer. Method PubMed, Medline, CINAHL and Embase databases were systematically searched on 31 August 2021. Qualitative and quantitative data on informal caregivers’ experiences were extracted and co

Palliative care and support for persons with HIV/AIDS in 7 African countries: implementation experience and future priorities.

To combat morbidity and mortality from the worldwide epidemic of the human immunodeficiency virus (HIV), the United States Congress implemented a President's Emergency Plan for AIDS Relief (PEPFAR) in 30 resource-limited countries to integrate combination antiretroviral therapy (ART) for both prevention and cure. Over 35% of eligible persons have been successfully treated. Initial legislation cited palliative care as an essential aspect of this plan but overall health strengthening became critical to sustainability of programming and funding priorities shifted to assure staffing for care deliv

Lived experiences of people with or at risk for aortic dissection: A qualitative assessment.

Aortic dissection (AD) is a life-changing event that is often accompanied by a loss of normal quality of life. Survivors of AD go on to have a chronic disease that requires lifestyle modification, medical management, and surgical repair of the aorta. Clinical care includes multiple disciplines, health care settings, and often different geographic locations. This qualitative assessment examined the experiences of people with and at risk for AD. The following four themes emerged: "unnecessary drama" at diagnosis, unmet needs for information and support, the burden of self-advocacy and care coord

Residential immersive life skills programs for youth with disabilities: a case study of youth developmental trajectories of personal growth and caregiver perspectives.

BACKGROUND: Professional support in pediatric and rehabilitation care environments has been recommended as a means to build youth competence in life skills during their transition to adulthood. Life skills are the essential psychosocial competencies and interpersonal skills needed to manage one's life. Residential immersive life skills (RILS) programs offer youth with physical disabilities enriched learning environments to acquire these skills. This study explored trajectories of personal growth in life skills and positive psychological outcomes among youth participating in a RILS program and

An exploration of the extent to which attending Headway enhances quality of life after traumatic brain injury.

PURPOSE: This paper explores the perception of quality of life (QOL) of four men following traumatic brain injury (TBI) and the effects of their injury on family, social and working life. METHOD: It also highlights their on-going difficulties 6, 8, 10 and 11 years post injury and that long term support is not available from rehabilitation services often due to the NHS financial restraints. RESULTS: Participants in this study have highlighted: (a) the need for long term support over an unspecified continuum; (b) the value of peer support; (c) the value of an understanding environment prepared

Time of Care and Time of Dying: A Multidisciplinary Case Report on End-of-Life Experience Within the Italian Legal Framework.

In this segment of the Palliative Care Unit case series, we introduce a patient with a long history of oncological treatments for recurrent breast cancer. After active treatments and a global control of the neoplasm, disease progression made the patient access different lines of chemotherapies, even asking for them in anticipation of a few advantages in the balance between benefits and risks. When the patient decided to permanently discontinue chemotherapy, she felt she had disrupted her values. Also, as a reaction to breaking bad news without estimating alternative paths, she considered her d

Some dramatic clinical situations, rare yet recurrent such as perinatal asphyxia, open the door to the taboo issue of active ending of life of newborn babies. Law and morals explicitely forbid it, while they both open their arms to the booming neonatal palliative care. Yet this taboo leads to two paradoxes. Some of these babies will die, in due palliative care, despite their parents’ fierce will to support their handicap. Whereas other babies will survive in awful conditions, to the great dismay of their parents and the medical team which are both deprived of the “liberty” of

Social determinants of health and quality of life among Korean Americans with chronic hepatitis B.

BACKGROUND: Hepatitis B virus (HBV) infection disproportionately affects Korean Americans, who often experience persistent social and structural challenges that may influence quality of life. Recent community studies showed most Korean Americans with chronic hepatitis B (CHB) were disengaged from clinical care, emphasizing the need to better understand factors affecting their well-being. This study examined associations between social determinants of health (SDOH) and physical and mental health related quality of life (HRQoL) among Korean Americans with CHB. METHODS: A total of 365 CHB patient
