Microsurgical Approaches to the Ambient Cistern Region: An Anatomic and Qualitative Study.

OBJECTIVE: We used microscopy to conduct qualitative and quantitative analysis of 4 surgical approaches commonly used in the surgery of the ambient cistern: infratentorial supracerebellar (SC), occipital interhemispheric, subtemporal (ST), and transchoroidal (TC). In addition, we performed a parahippocampal gyrus resection in the ST context. METHODS: Each approach was performed in 3 cadaveric heads (6 sides). After the microscopic anatomic dissection, the parahippocampal gyrus was resected through an ST approach. The qualitative analysis was based on anatomic observation and the quantitative

[Spontaneous cerebrospinal fluid leak may cause intracranial hypotension].

Spontaneous intracranial hypotension (SIH) is often misinterpreted as migraine or tension headache. This type of headache is, however, orthostatic and resolves in supine position. CT scan/MRI of the brain has characteristic findings, enhancement of the pachymeninges and bilateral hygroma. An extreme situation of a 70-year-old woman with sagging midbrain is described in this case report. Although this type of headache may be caused by a dural fistula with spinal fluid leak it is not necessary to locate the lesion with myelografi/MR. Timely treatment with an epidural blood patch at any lumbal le

Isolated Unilateral Ptosis due to Paramedian Midbrain Infarction.

A 59-year-old man who had hypertension, dyslipidemia, diabetes mellitus, and left eye glaucoma developed sudden vertigo and left ptosis; he did not notice diplopia. He visited our hospital on day 3 after onset and neurologic examination showed left ptosis. His left visual acuity was counting fingers, and the light reflex was sluggish owing to glaucoma. Pupil sizes were equal, and eye movements and the lower lid were unremarkable. Magnetic resonance images revealed an acute infarction of the left paramedian midbrain. We considered that selective damage to the oculomotor fascicles innervating th

Disorders of vergence eye movements.

PURPOSE OF REVIEW: The aim is to re-interpret disorders of vergence in the light of recent studies that view disjunctive eye movements as but one component of three-dimensional gaze control. RECENT FINDINGS: Most natural eye movements combine vergence with saccades, pursuit and vestibular eye movements. Electrophysiological studies in epileptic patients, as well as evidence from monkeys, indicate that frontal and parietal cortex govern vergence as a component of three-dimensional gaze. Clinicians apply Hering's law of equal innervation to interpret disjunctive movements as the superposition o

Neuro-Behçet's disease with dizziness.

A 30-year-old man had complete-type Behçet's disease since he was 23 years old. Disease signs and symptoms were well controlled. After experiencing no symptoms for some years, however, he experienced dizziness, headache, fever, dysarthria, right facial nerve palsy, and right tinnitus. He showed spontaneous horizontal-rotatory nystagmus directed toward the right side, and upbeat nystagmus. T2-weighted and fluid-attenuated inversion recovery MRI showed slight hyperintense signals in the medulla oblongata, pons, and left midbrain. Neurological involvement in Behçet's disease was diagnosed.

Morning glory sign: a particular MR finding in progressive supranuclear palsy.

BACKGROUND AND PURPOSE: We have encountered a peculiar atrophic change in the midbrain in some patients with parkinsonian syndromes. We discovered these patients had vertical supranuclear gaze-palsy, an eye movement disorder. The purpose of this study was to elucidate whether this atrophic pattern of the midbrain (which we have termed morning glory sign) is related to the vertical eye movement disorder, in particular to progressive supranuclear palsy (PSP). METHODS: We reviewed T2-weighted axial images obtained from 42 patients with parkinsonian syndromes, including five patients with PSP, 23

Expanding Virchow Robin spaces in the midbrain causing hydrocephalus.

We describe a patient who showed cystic lesions in the midbrain and obstructive hydrocephalus. MR imaging features corresponded to abnormal dilatation of perivascular spaces. Similar clinical and MR imaging findings were described in previous reports of patients presenting with mesencephalo-thalamic expanding lacunae; the diagnosis of some of those cases was confirmed at postmortem examination This condition is extremely rare but should be part of a differential diagnosis when evaluating cystic abnormalities in this area of the brain.

Glial cyst of the pineal gland: case report and considerations about surgical management.

Symptomatic glial cyst of the pineal gland are rare lesions. Origin, natural history and factors leading to cyst enlargement are not completely clear; thus management remain uncertain in some cases. We report a case of symptomatic glial cyst and analyze the implication for surgery. Surgical management is indicated in patients presenting hydrocephalus, mass effect or symptoms related to mesencephalic dysfunction. The infratentorial supracerebellar approach represent the first choice for this condition allowing easy orientation with wide exposure of the tumor and good visibility of deep venous s

[Upward and downward gaze palsy with a unilateral mesencephalic hemorrhage].

INTRODUCTION: Acute vascular lesions limited to the midbrain can yield varied disturbances of eye movements. They are more often infarcts than hemorrhages. CLINICAL CASE: We present the case of a 49 year old, non-hypertensive man who presented with sudden onset of bilateral up and down gaze palsy. CT defined a unilateral hemorrhage in the right mesencephalic tegmentum. CONCLUSION: Cases of upward and downward gaze palsy in association with unilateral upper midbrain hemorrhage are very uncommon. We present a new case.

[Midbrain infarction presenting isolated inferior rectus nuclear palsy].

We presented a patient of isolated inferior rectus muscle palsy from midbrain lacunar infarction involving the oculomotor nucleus. The patient noticed sudden onset diplopia gazing to the right side, especially to the right-lower direction. He did not have any other symptom, and neurological examination revealed no other findings. Brain MRI documented the focal hyperintense lesion on T2-weighted images in the right-median midbrain ventral to the aqueduct at the level of the superior colliculus. This lesion involved the right oculomotor nucleus, especially the dorso-lateral subnucleus extend to

Individual hemispheric asymmetry in vocal fold control of the squirrel monkey.

In ten squirrel monkeys (Saimiri sciureus), electrodes were implanted into the left and right facial motor cortex at sites producing bilateral vocal fold adduction when electrically stimulated. All animals, in addition, had electrodes in the periaqueductal grey of the midbrain (PAG) at sites producing vocalization when electrically stimulated. In eight out of ten animals, motorcortical stimulation during periaqueductally elicited vocalization caused a change in vocal output. This change, in four cases, was more severe with left-sided ipsilateral motor cortex/PAG stimulation than with right-sid

[Posttraumatic intention tremor. Report of a case with a lesion in the mesencephalic area].

We present a child who suffered severe coma because of an automovile accident remaining two months in coma with brain stem dysfunction and one and a half additional months until he was able to recognize people. After recovering, he showed severe kinetic tremor in the upper right extremity and discrete right hemiparesis as well as a moderate clinical cerebellar disfunction. Magnetic resonance (MR) study, performed 1 and 3 years after the coma, showed normal images of the cerebellum and thalami, while it disclosed signal changes in the mesencephalic region next to both red nuclei, being more sev

Hyperacute perimesencephalic subarachnoid hemorrhage: demonstration of blood extravasation with MRI.

Perimesencephalic subarachnoid hemorrhage (SAH) is a benign subtype of SAH without known vascular pathology. We report a case of current subarachnoid bleeding in the vicinity of the basilar artery. The patient was on anticoagulation medication and had sustained a head injury 2 days earlier. The bleeding was documented by contrast agent extravasation in MRI. Because of the isointensity of hyperacute blood to CSF in conventional T2 sequences, FLAIR images are mandatory for diagnosis in the hyperacute state.

A clinical conundrum: unilateral decerebrate rigidity.

A case is described in which, following a motorcycle accident resulting in a closed head injury, a young woman was rendered unconscious. On examination she was found to have unilateral decerebrate rigidity. Further clinical and radiological examination revealed that there was no damage to the limbs, vertebral column, or spinal cord. Readers are expected to be able to make an anatomical diagnosis on the basis of the facts presented.

Asymmetric predominantly ipsilateral blepharospasm and contralateral parkinsonism in an elderly patient with a right mesencephalic cyst.

A 66-year-old woman presented with a 3-year history of predominantly right-sided blepharospasm and a 1-year history of progressive predominantly left-sided hemiparkinsonism manifested by a left upper extremity resting tremor and left-sided bradykinesia. Magnetic resonance imaging of the brain revealed a large right mesencephalic cyst with mass effect. Positron emission tomography revealed bilateral striatal hypometabolism consistent with nigrostriatal dopaminergic dysfunction. The association of predominantly ipsilateral blepharospasm and predominantly contralateral hemiparkinsonism is very ra

Role of the midbrain periaqueductal gray in maternal nurturance and aggression: c-fos and electrolytic lesion studies in lactating rats.

the upright, crouched, or kyphotic, nursing posture of lactating rats is dependent on suckling stimulation from pups. Because of the neuroanatomical connections of the periaqueductal gray (PAG) and its sensorimotor integration of the analogous lordosis posture displayed by sexually receptive female rats, the possible role of the PAG in kyphosis was investigated using c-fos immunocytochemistry and electrolytic lesions. Lactating rats interacting with and nursing a litter of suckling pups showed greater Fos-immunoreactive nuclei in the lateral and ventrolateral caudal PAG (cPAGl,vl) compared wit
