Adoption Decisions for Medical Devices in the Field of Cardiology: Results from a European Survey.

Decisions to adopt medical devices at the hospital level have consequences for health technology assessment (HTA) on system level and are therefore important to decision makers. Our aim was to investigate the characteristics of organizations and individuals that are more inclined to adopt and utilize cardiovascular devices based on a comprehensive analysis of environmental, organizational, individual, and technological factors and to identify corresponding implications for HTA. Seven random intercept hurdle models were estimated using the data obtained from 1249 surveys completed by members of

Knowledge, experience and practice of SLTs regarding (re)habilitation in deaf children with cochlear implants.

OBJECTIVES: To assess the knowledge, experience and practices of speech and language therapists (SLTs) in Greece regarding children with cochlear implants. METHODS: A special designed questionnaire was originally completed electronically by 313 SLTs via surveymonkey platform. RESULTS: From the 313 respondents 35% had worked with implanted children, 37% received course training and 44% had participated in post-graduate seminars. Although 96% believe that there are differences in the management of these children, almost 47% of the participants did not have adequate knowledge on the candidacy c

Evaluation and Restoration of Competence to Stand Trial: Intercepting the Forensic System Using the Sequential Intercept Model.

The sequential intercept model (SIM) is used to reduce the penetration of persons with mental illness and substance use disorders in the criminal legal system. Its framework recommends identifying individuals with mental illness at various decision points of criminal case processing, from arrest to return from incarceration, so that they can be diverted toward treatment rather than permeate deeper into the criminal justice system. Communities frequently use the model to augment and inform jail diversion services. Despite the model's widespread adoption, individuals with serious mental illness,

Is a differentiated care model needed for patients with TB? A cohort analysis of risk factors contributing to unfavourable outcomes among TB patients in two states in South India.

BACKGROUND: TB is a preventable and treatable disease. Yet, successful treatment outcomes at desired levels are elusive in many national TB programs, including India. We aim to identify risk factors for unfavourable outcomes to TB treatment, in order to subsequently design a care model that would improve treatment outcomes among these at-risk patients. METHODS: We conducted a cohort analysis among TB patients who had been recently initiated on treatment. The study was part of the internal program evaluation of a USAID-THALI project, implemented in select towns/cities of Karnataka and Telangan

Asistencial and therapeutic continuity after release from prison: an urgent problem that requires solutions. The model applied in Catalonian prisons.

Deficits in health after being released are common, even with a higher risk of death. In these cases, the main cause of death is the use of heroin and other opioids, but there are more causes of death and most are potentially preventable. The most vulnerable groups to the increase of post-release morbidity and mortality are the drug users, the mentally ill and the foreign inmates from countries with few economic resources. It is urgent to implement interventions that optimize access to health devices after the prison release, avoid interruptions in the continuity of care and treatment prescrib

Strengthening community mental health competence-A realist informed case study from Dehradun, North India.

Few accounts exist of programmes in low- and middle-income countries seeking to strengthen community knowledge and skills in mental health. This case study uses a realist lens to explore how a mental health project in a context with few mental health services, strengthened community mental health competence by increasing community knowledge, creating safer social spaces and engaging partnerships for action. We used predominantly qualitative methods to explore relationships between context, interventions, mechanisms and outcomes in the "natural setting" of a community-based mental health projec

Socio-ecological dynamics and challenges to the governance of Neglected Tropical Disease control.

The current global attempts to control the so-called "Neglected Tropical Diseases (NTDs)" have the potential to significantly reduce the morbidity suffered by some of the world's poorest communities. However, the governance of these control programmes is driven by a managerial rationality that assumes predictability of proposed interventions, and which thus primarily seeks to improve the cost-effectiveness of implementation by measuring performance in terms of pre-determined outputs. Here, we argue that this approach has reinforced the narrow normal-science model for controlling parasitic dise

Waiting management at the emergency department - a grounded theory study.

BACKGROUND: An emergency department (ED) should offer timely care for acutely ill or injured persons that require the attention of specialized nurses and physicians. This study was aimed at exploring what is actually going on at an ED. METHODS: Qualitative data was collected 2009 to 2011 at one Swedish ED (ED1) with 53.000 yearly visits serving a population of 251.000. Constant comparative analysis according to classic grounded theory was applied to both focus group interviews with ED1 staff, participant observation data, and literature data. Quantitative data from ED1 and two other Swedish E

Delivering interventions to reduce the global burden of stillbirths: improving service supply and community demand.

BACKGROUND: Although a number of antenatal and intrapartum interventions have shown some evidence of impact on stillbirth incidence, much confusion surrounds ideal strategies for delivering these interventions within health systems, particularly in low-/middle-income countries where 98% of the world's stillbirths occur. Improving the uptake of quality antenatal and intrapartum care is critical for evidence-based interventions to generate an impact at the population level. This concluding paper of a series of papers reviewing the evidence for stillbirth interventions examines the evidence for c

Increasing the demand for and use of effective smoking-cessation treatments reaping the full health benefits of tobacco-control science and policy gains--in our lifetime.

More adults in the United States have quit smoking than remain current smokers. But 45 million adults (20.9%) continue to smoke, with highest rates among low socioeconomic status (SES), blue-collar, and Native American populations. More than two thirds (70%) of adult smokers want to quit, and approximately 40% make a serious quit attempt each year, but only 20%-30% of quitters use an effective behavioral counseling or pharmacologic treatment. The lowest rates of treatment use are seen in the populations with the highest rates of tobacco use. Fully harvesting the last 4 decades of progress in t

Risk-adjustment of cesarean delivery rates: a practical method for use in quality improvement.

Risk-adjustment of cesarean birthrates has been hampered by inadequacies in the existing secondary data sources or by the need for extensive chart review. This study presents an efficient risk-adjustment model for cesarean birth, based on easily retrievable ICD-9 codes and clinical risk factors least influenced by physician practice style. Data are presented for mothers undergoing 7322 deliveries from 1997-1998 at a large academic medical center with a cesarean birth rate of 15.9%. Multiple logistic regression was used to predict the likelihood of cesarean delivery controlled for maternal age,

Elucidating insights on how care was prioritized, adapted, and missed during and post pandemic.

Globally, healthcare systems continue to recover and manage system demands, including our sustained HHR pressures exacerbated by the COVID-19 pandemic. Health system leaders need to understand how healthcare was adapted during the pandemic, what contributed to these changes, and the impact of these changes to inform future efforts. The overarching research questions included: What changes to models of care were made during COVID-19 and post-recovery? What factors contributed to changes in models of care? What was the impact of these changes? An exploratory interpretative descriptive qualitativ

Machine learning models for minimizing aggravation in work-related musculoskeletal disorders among slaughterhouse workers.

BackgroundWork-related musculoskeletal disorders (WMSDs) are common in Brazilian slaughterhouses. The repetitive and strenuous nature of meat processing, especially in slaughterhouses, makes employees highly susceptible to developing WMSDs. Prolonged standing, repetitive motions, and forceful actions such as lifting and cutting are common contributing factors.ObjectiveThis study aimed to develop models to predict the risk of aggravating WMSDs in slaughterhouse workers using the data mining concept.MethodsData were retrieved from an open-source governmental database, and descriptive statistics

Health systems and climate change mitigation through an applied health informatics and sustainability perspective.

Climate change poses a growing threat to U.S. healthcare systems, which are uniquely positioned to mitigate its impacts while addressing their own greenhouse gas emissions from energy use, supply chains, clinical operations, and waste. Clinical informaticists can advance mitigation by integrating environmental data into health system operations and decision-making. Tracking emissions across scopes enables data-informed strategies, while frameworks such as I-CLIMATE and One Digital Health can help coordinate disparate data streams and reduce healthcare's environmental impact.
