Symptoms suggestive of a transient disturbance in cardiac rhythm often prompt a patient to seek medical help. Transtelephonic monitoring (TTM) can document the rhythm at the exact time that the patient is experiencing clinical symptoms. Our experience with TTM from 1979 to 1983 involved 526 patients who had one or more of the following symptoms: near-syncope (in 121), light-headedness (in 248), intermittent palpitations (in 281), and tachycardia (in 321). To characterize and classify neonatal seizures, we studied 349 neonates, using a portable, cribside EEG/polygraphic/video monitoring system. We recorded 415 clinical seizures in 71 infants; 11 other infants had electrical seizure activity without clinical accompaniments. Each seizure was analyzed in terms of its clinical character and its relationship to the presence of EEG seizure activity. A procedure is described which aims to classify an EEG recorded during neurosurgery, for example intracerebral aneurysm clipping. A parametric approach is used; it employs auto-regressive (AR) modelling and Kalman filtering to quantify directly the dynamics of the EEG generating mechanism, supposing it to be a linear, time-invariant system driven by white noise. We analyzed 22 sickle cell trait hyphemas. Fourteen of the 22 eyes were adequately controlled with medical therapy alone, ie, the intraocular pressure averaged less than 25 mm Hg during consecutive 24-hour periods and there were no repeated transient rises of IOP above 30 mm Hg. Surgery was performed in eight eyes, because of inadequate medical control. Thirteen of the 14 medically treated eyes had acceptable levels of IOP within the first 24 hours; only one of the eight surgically treated eyes had adequate control during the first 24 hours. One hundred eighty-nine Swan-Ganz catheters were inserted in 165 critically ill patients with different techniques. Percutaneous catheterization through the subclavian, internal jugular, and antecubital veins and antecubital venous cutdown with or without fluoroscopy were compared. The antecubital approach, percutaneously or through a venous cutdown, was associated with most complications. We conclude that the method of choice is the percutaneous insertion of the balloon-tipped catheter through the subclavian or internal jugular veins and that fluoroscopy is unnecessary.

A nonpyrogenic polyethylene sheath system with a hemostatic valve assembly and side port extension developed by the Cordis Corporation is now routinely used for central venous access in critically ill patients. It is inserted percutaneously via the subclavian or internal jugular vein and allows rapid and efficient catheterization for hemodynamic monitoring, infusion of multiple solutions simultaneously, and uniflow hemodialysis. This system represents a significant advance in catheter technology.

Long-term epidural intracranial pressure (ICP) recordings were performed in 10 hydrocephalic children, not affected by cerebral tumor. The work concerns the identification of A and B Lundberg's waves during normal sleep in the ICP pattern. Plateau-waves of high amplitude and long duration were measured in one patient. A base-line pressure higher than normal was found in the other cases in which waves during sleep were seldom revealed. Five children were monitored by studying the pressure pattern after shunt surgery. The Authors discuss the implication of the rarity of plateau-waves in infancy
