Aging of Skeletal Muscle: From Molecular Mechanisms to Therapeutic Interventions.

Skeletal muscle aging is a major cause of frailty, metabolic dysfunction, and loss of independence in later life, yet it cannot be explained by muscle mass loss alone. Recent single-cell, multi-omics, and translational studies show that aged muscle is shaped by coordinated changes in myofibers, stem and stromal cells, immune and vascular compartments, neuromuscular control, and systemic metabolism. This review describes skeletal muscle aging as a gradual loss of tissue resilience rather than a simple decline in muscle mass. Impaired proteostasis, mitochondrial dysfunction, chronic inflammation

Human Adipose-Derived Stem Cells Delay Muscular Atrophy after Peripheral Nerve Injury in Rats.

INTRODUCTION: Given that denervation atrophy often occurs in muscle after peripheral nerve injury, the effects of injections of human adipose-derived stem cells (hADSCs) and platelet-rich plasma (PRP) into muscle after peripheral nerve injury were examined. METHODS: hADSCs were isolated from human subcutaneous fat tissue, and PRP was prepared from rat whole blood before injection into a rat sciatic nerve injury model. Muscle atrophy was evaluated by quantitating the gross musculature and muscle fiber area and walking footprint analysis. RESULTS: At 4 weeks post-surgery, there were significan

Incidence of adverse events associated with percutaneous muscular biopsy among healthy and diseased subjects.

The development of the percutaneous muscle biopsy technique is recognized as one of the most important scientific contributions in advancing our understanding of skeletal muscle physiology. However, a concern that this procedure may be associated with adverse events still exists. We reported the incidence of adverse outcomes associated with percutaneous muscle biopsy in healthy and diseased subjects. Medical records of 274 volunteers (496 muscle biopsies) were reviewed. This included 168 healthy subjects (330 muscle biopsies) as well as 106 chronically ill patients (166 muscle biopsies). This

Pain and calf hypertrophy associated with spontaneous repetitive discharges treated with botulinum toxin.

OBJECTIVE: To report a case of neurogenic calf hypertrophy associated with root lesion responsive to botulinum toxin (BTX). In addition, we systematically review the literature for cases of muscle hypertrophy associated with radiculopathy. METHODS: A 56-year-old man with a previous history of S1 root lesion presented with marked calf hypertrophy, muscle pain, cramps, fasciculations, and increased CK level. Magnetic resonance imaging and muscle biopsy confirmed true muscle hypertrophy. Electromyography (EMG) showed signs of chronic denervation-reinnervation in the right S1 myotome, with greatl

Klotho Deficiency Promotes Skeletal Muscle Weakness and Is Associated with Impaired Motor Unit Connectivity.

Muscle wasting and weakness are critical clinical problems that limit mobility and independence, reduce health span, and increase the risk of physical disability. The molecular basis for this has not been fully determined. Klotho expression is downregulated in conditions associated with muscle wasting, including aging, chronic kidney disease, and myopathy. The objective of this study was to investigate a mechanistic role for Klotho in regulating muscle wasting and weakness. Body weight, lean mass, muscle mass, and myofiber caliber were reduced in Klotho-deficient mice. In the tibialis anterior

[Results of a denervating operation in radial and ulnar humeral epicondylitis].

The treatment of lateral epicondylitis is first of all conservative, and only resistant cases need operative care. The authors performed denervation after the technique of Wilhelm with a slight modification on 63 elbows in 55 patients. The results at follow-up were excellent in 47 elbows and good in 11 (=92%), fair in three and failed in one. According to the anatomical model it is suggested that this procedure permits not only a selective neurotomy of the nerve supply to the lateral epicondyle, but also release of the extensor muscles at their attachment on the epicondyle and on the orbicular

Postoperative Flank Defects, Hernias, and Bulges: A Reliable Method for Repair.

BACKGROUND: Although there is a high incidence of flank defects after lateral abdominal access, there is a paucity of large studies discussing this problem. Most studies express nihilism regarding their surgical management. The goal of this study was to describe the authors' conceptualization of flank defects, with a determination of the number of true hernias versus bulges, and outcomes of surgical repair in these patients. METHODS: The authors carried out a 13-year retrospective review of 31 consecutive flank defects repaired by the senior author (G.A.D.). Patients were treated with a 7.5-c

Augmenting nerve regeneration with electrical stimulation.

OBJECTIVE: Poor functional recovery after peripheral nerve injury is generally attributed to irreversible target atrophy. In rats, we addressed the functional outcomes of prolonged neuronal separation from targets (chronic axotomy for up to 1 year) and atrophy of Schwann cells (SCs) in distal nerve stumps, and whether electrical stimulation (ES) accelerates axon regeneration. In carpal tunnel syndrome (CTS) patients with severe axon degeneration and release surgery, we asked whether ES accelerates muscle reinnervation. METHODS: Reinnervated motor unit (MUs) and regenerating neuron numbers wer

Nervous control of distension-induced relaxation of the porcine lower oesophageal sphincter.

The mechanisms involved in the relaxation of the lower oesophageal sphincter induced by distension of the oesophagus and different parts of the stomach, were studied in an anaesthetized porcine model. A computer technique was developed allowing on-line digitizing of lower oesophageal sphincter (sleeve device) and intragastric pressures. Basal sphincter tone was slightly reduced by truncal vagotomy, an effect which seemed to be reversed by sectioning of the vagosympathetic trunks in the neck. Balloon distension of the body of the oesophagus, relaxed the sphincter irrespective of the denervation

Selective peripheral denervation for torticollis: preliminary results.

Herein we report the preliminary results in nine patients who have undergone selective peripheral denervation for spasmodic torticollis and have been followed up for at least 13 months. All patients had improvement immediately after surgical intervention, and the results have been maintained in five patients. In one patient who had recurrent torticollis, a second procedure in conjunction with injection of botulinum toxin has produced substantial improvement; however, follow-up was brief (6 months). No surgical complications occurred. We believe that selective peripheral denervation is safe and

Assessment of facial reinnervation by use of chronic electromyographic monitoring.

The study of muscle reinnervation has been difficult because of lack of an accurate, reproducible method to monitor return of function. Visual assessment relies on subjective interpretation. Histology provides anatomic, not functional, information. Electromyography and anatomic tracing have been most effective in evaluating physiologic return of muscle function. It has been difficult to assess the timing of functional return electromyographically because measurements are intermittent and electrode placement varies. A method was designed to allow long-term monitoring of electromyographic (EMG)

Section of fibular nerve affects activity pattern and contractile properties of soleus motor units in adult rats.

Transection of the common fibular (FIB) nerve caused an immediate reduction in the total amount of soleus (SOL) motor unit activity, which declined further during the following 10 days and then remained stable at less than half of normal values. In addition, there was an immediate reduction in median impulse rate from about 20.0 to 14.9 Hz followed by a return to normal values during the first 10 days. Short interval (3-4 ms) double discharges, occurring either in isolation or at the same regular intervals as single impulses, were observed 2-5 days after FIB nerve section in a few motor units.

Dorsal approach in transfer of the distal spinal accessory nerve into the suprascapular nerve: histomorphometric analysis and clinical results in 14 cases of upper brachial plexus injuries.

PURPOSE: The spinal accessory nerve (SAN) is conventionally transferred to the suprascapular nerve (SSN) through an incision in the supraclavicular region (the anterior approach) to improve shoulder function in brachial plexus injuries. This approach carries a risk of partial denervation of upper trapezius muscle. Here we describe how dorsal nerve transfer through an incision placed directly over the scapular spine preserves the proximal branches to the upper trapezius muscle and allows nerve transfer close to target muscles. METHODS: We report our experience with the dorsal approach in 14 ca

Increased expression of muscular neural cell adhesion molecule in congenital aganglionosis.

BACKGROUND: Neural cell adhesion molecule (NCAM) is down regulated during morphogenesis and innervation of cardiac and skeletal muscle. In mature muscle, its reexpression over the entire sarcolemma occurs in response to denervation or paralysis of muscle and in some myopathies. No information is available regarding NCAM expression in human enteric muscle either in health or in disease. Our aim was to test whether NCAM is present in nerves and muscle of normal infant bowel and to determine how its expression is altered in congenital aganglionosis. METHODS: Using immunocytochemistry for light m

Heat Shock Proteins: Important Helpers for the Development, Maintenance and Regeneration of Skeletal Muscles.

The skeletal muscle is a highly plastic tissue that shows a remarkable adaptive capacity in response to acute and resistance exercise, and modifies its composition to adapt to use and disuse, a process referred to as muscle plasticity. Heat shock proteins (HSPs), a class of evolutionarily conserved molecular chaperones, have been implicated in the regulation of skeletal muscle plasticity. Here, we summarize key findings supporting the notion that HSPs are important components required to maintain skeletal muscle integrity and functionality. HSPs participate in the transcriptional program requi

Skeletal muscle-gut axis: emerging mechanisms of sarcopenia for intestinal and extra intestinal diseases.

In recent years, there has been an increasing interest on muscle wasting, considering the reduction of quality of life and the increase of morbidity and mortality associated. Sarcopenia and cachexia represent two conditions of reduction of muscle mass, sharing several elements involved in their pathogenesis, such as systemic inflammation, impaired muscle protein synthesis, increased muscle apoptosis, mitochondrial dysfunction in skeletal muscle tissue and insulin resistance. These features often characterize cancer, inactivity or denervation, but also inflammatory diseases, such as chronic obs

Anterior horn cell involvement in myelitis with atopic diathesis (atopic myelitis).

To clarify the involvement of anterior horn cells in myelitis with atopic diathesis (atopic myelitis), 20 patients with atopic myelitis were subjected to neurological evaluation, concentric needle electromyography (EMG), spinal cord magnetic resonance imaging (MRI) and motor and somatosensory evoked potentials. Apparent muscle atrophy was present only 1 of 20 patients (5%) and the rests clinically showed no lower motor neuron sign. On needle EMG, 12 patients (60%) showed varying degrees of lower motor neuron involvement. On-going denervation potentials, such as fasciculation potentials, fibril
