[Diagnosis and management of nail psoriasis].

Nail psoriasis has traditionally been considered as one of the most difficult forms of psoriasis to treat. The most recent studies have found a higher prevalence of fungal infections in psoriatic nails and therefore cultures before and during treatment are necessary. Over the last few years, novel psoriatic therapies have been developed, some of which are effective for both skin and nail lesions (cyclosporine and biologic agents). Of the topical agents, vitamin D and A derivatives as well as nail lacquer containing 8 % clobetasol propionate can help improve lesions of both the nail bed and mat…

METHODS: Seventy-nine patients with PV, including 59 new patients and 20 patients in relapse, were entered into the study. Microscopic examination in potassium hydroxide and culture for fungus were performed on all clinically abnormal nails. RESULTS: Twenty-five (31.6%) of 79 patients showed nail changes, with paronychia (n = 8) and onychomadesis (n = 6) being the most common.…

Biological therapy and nail psoriasis.

Nail psoriasis affects 50% of psoriasis patients and in many cases causes impairment of manual dexterity, pain, and psychologic stress. Despite the fact that about 80% of psoriatic arthritis patients have nail involvement, patients rarely receive treatment for nail disease. Because of great difficulty in drug delivery to site of action and significant toxicities of most conventional systemic therapies, treatment of nail psoriasis can be very challenging. Biological therapy for psoriasis and psoriatic arthritis is now widely used, and early evidence indicates it may have significant benefit for

Onycholysis induced by nail hardener.

Nail hardeners appeared in the market during the 1960s. They were basically solutions of formaldehyde. The first adverse effects were published in 1966 (1). Reactions were onycholysis, chromonychia, subungual haemorrhage, and hyperkeratosis. Onycholysis may be non-inflammatory or inflammatory, and is accompanied by throbbing pain. Inflammatory reactions are followed by paronychia and occasional dermatitis on the digital pulpa.

Side effects of nail cosmetics.

Cosmetics applied on the nail encompass three types: (1) coatings that harden upon evaporation; (2) coatings that polymerize; and (3) stick-on nail dressings (synthetic covers). The adverse reactions induced by the two first types present with both local reactions and distant contact dermatitis. Whereas nail enamel applications result especially in ectopic contact dermatitis, polymerizing coatings and synthetic covers represent the main culprit for sometimes severe, local reactions. Whatever the nature of the nail cosmetics, they may produce some adverse reactions. They include: (1) local reac
