BACKGROUND AND OBJECTIVES: Atheroembolism, caused by peripheral embolization of small cholesterol crystals that fracture off of ruptured ath … mbolism among inpatients in an acute hospital setting.
PATIENTS AND METHODS: We surveyed inpatient nephrology consultations during a 7-month period from January through July 1994. From a pool of 402 consultation charts, 99 were identified with two or more substantive risk factors for atheroembolism. The records of 85 of these patients were available for car …

This story was written by a patient with immunoglobulin light chain amyloidosis who had a stem cell transplant and eventually a heart and kidney transplant. She was treated by Horacio E. Adrogue, MD, FASN, and a team of health care professionals in the Division of Nephrology, Transplantation, and Hypertension at Houston Methodist Hospital in Houston, Texas. We appreciate her willingness to share her story and hope it will inspire others.

Renal dysfunction is a common, life-threatening complication occurring in patients with liver disease. Hepatorenal syndrome (HRS) has been defined as a purely "functional" type of renal failure that often occurs in patients with cirrhosis in the setting of marked abnormalities in arterial circulation, as well as overactivity of the endogenous vasoactive systems.4,5 In 2007, the International Club of Ascites (ICA) classified HRS into types 1 and 2 (HRS-1 and HRS-2).5 HRS-1 is characterised by a rapid deterioration of renal function that often occurs because of a precipitating event, while HRS-2

Renal transplantation in India costs about US$5000 ($1=Rupees 48.25), azathioprine costs $200 a year and cyclosporine costs $2000. Against this the average per capita income is $279 (Rs. 12989) per year; 36% of the population earn less than $105, and only 2.2% earn more than $1000. The country cannot afford to treat end-stage renal disease. Thirty percent of chronic renal failure is due to diabetic nephropathy, and 10% each to hypertensive nephropathy and chronic pyelonephritis. Social and preventive health workers of the Kidney Help Trust administered a questionnaire at the homes of a study p

Vitamin D insufficiency is endemic amongst renal transplant recipients,as it is in other individuals with chronic diseases, both within and beyond nephrology. Few data exist to guide vitamin D replacement strategies, but indirect evidence points to likely skeletal, and possibly extraskeletal, benefits from supplementation.

Mitochondrial diseases, rated as rarities in paediatrics and neurology, become to be more and more frequently diagnosed in various body systems, including the kidneys. In nephrology, mitochondrial defects play an important role in the pathogenesis of tubular syndromes, interstitial nepohritis, focal and segmental glomerulosclerosis.

Renal biopsy remains the gold standard for diagnosing kidney disease; it is performed in accordance with generally accepted clinical indications and evaluated using well-established histopathological techniques. This is usually done by specialized renal pathology departments, which have the necessary special techniques at their disposal, such as special staining, immunofluorescence or immunohistology, and, above all, electron microscopy, which are not available at all pathology institutes. In addition to classic histology-based analysis methods, new modern complementary methods are increasingl

The pilot transition from outsourced to in-house hemodialy sis (HD) services at a 274-bed acute care hospital resulted in significant cost savings, operational improvements, and enhanced patient care. By adopting an advanced in-house HD model, the pilot hospital reduced per-treatment costs by 89.2%, eliminated outsourced vendor-imposed penalties, and achieved a 0% turnover rate among nephrology nurses. A structured, multidisciplinary strategy streamlined workflows and improved staff engagement. The pilot's success catalyzed system-wide expansion over four years across eight hospitals, demonstr…

As patients progress through chronic kidney disease, they are eventually faced with the reality of having to choose which type of treatment they want when their kidneys are no longer functioning enough to sustain them - when they have reached end stage kidney disease. These discussions with patients about options should start early and include the whole multidisciplinary team. Treatment options in clude transplant, peritoneal dialysis, hemodialysis, and supportive care.

Anaemia is common in chronic kidney disease (CKD) encompassing non-dialysis dependent CKD (NDD-CKD) and dialysis dependent CKD (DD-CKD); people on peritoneal dialysis (PD) and haemodialysis (HD); and kidney transplant recipients (KTR). Iron deficiency and erythropoietin deficiency are the most common causes of anaemia in people with CKD, especially those requiring kidney replacement therapy (KRT). The Renal National Service Framework and National Institute for Health and Clinical Excellence in the UK, and Kidney Disease Improving Global Outcomes (KDIGO), all advocate treatment of anaemia in pe

Compared to women with normal kidney function, women with end-stage kidney disease (ESKD) are much less likely to become pregnant, but more likely to have a complicated pregnancy. While renal transplantation remains the best option for women who desire to have a child, pregnancy on dialysis should be considered an option for those who may not receive a transplant during their reproductive years. Many physicians do not encounter a pregnant patient with ESKD during their nephrology training, and may not feel prepared to provide care for such patients in a busy practice. In this paper, we briefly…

BACKGROUND: Renal involvement in ankylosing spondylitis (AS) is recognised but understudied, with heterogeneous reports ranging from urinalysis abnormalities to end-stage disease and malignancy. Robust, contemporary and population-level estimates remain lacking.
OBJECTIVE: To quantify the burden and risk of major nephrological outcomes in AS versus matched controls, estimate long-term occurrence, and explore sex-specific differences.
METHODS: We performed a retrospective, population-based cohort study using the TriNetX Global Collaborative Network of anonymised electronic health records. Adu

Background: BRASH syndrome is a life-threatening condition that involves bradycardia, renal failure, atrioventricular blockade, shock, and hyperkalemia. It is often resistant to conventional treatments and requires prompt diagnosis and management. We report a case of BRASH syndrome successfully treated in the Emergency Department and Nephrology Department. Case Presentation: A 57-year-old man with hypertension, diabetes, ischemic heart disease, and chronic kidney disease presented with severe diarrhea, lethargy, and shock. He had hyperkalemia, metabolic acidosis, and acute kidney injury. His e…

Incorporating genetic testing in routine outpatient nephrology clinic can improve on chronic kidney disease (CKD) diagnosis and utilization of precision medicine. We sent a genetic test on patients with atypical presentation of common kidney diseases, electrolytes derangements, and cystic kidney diseases. We were able to identify a gene variant contributing to patients' kidney disease in more than half of our cohort. We then showed that patients with ApoL1 risk allele have likely worse kidney disease, and we were able to confirm genetic focal segmental glomerulosclerosis (FSGS) in 2 patients a…

The 60th anniversary of Israel coincides with the advent of the first hemodialysis in the Middle East. It was performed by Dr. Kurt Steinitz … ialysis to Prof. David Ullman from the Hadassah Hospital in Jerusalem. Ullman established the first nephrology unit with a regular dialysis service, becoming the pioneer of modern nephrology in Israel.

The TATH study informs the feasibility of twice-weekly hemodialysis in select patients. However, associated biological signals suggest underdialysis despite preserved residual kidney function. We highlight the importance of volume status and adequacy in comparator groups to truly assess the benefits and limitations of incremental dialysis strategies in modern nephrology.
