The clinical diagnostic utility of transcranial magnetic stimulation: report of an IFCN committee.

The review focuses on the clinical diagnostic utility of transcranial magnetic stimulation (TMS). The central motor conduction time (CMCT) is a sensitive method to detect myelopathy and abnormalities may be detected in the absence of radiological changes. CMCT may also detect upper motor neuron involvement in amyotrophic lateral sclerosis. The diagnostic sensitivity may be increased by using the triple stimulation technique (TST), by combining several parameters such as CMCT, motor threshold and silent period, or by studying multiple muscles. In peripheral facial nerve palsies, TMS may be used

Carpal tunnel syndrome in occupational medicine practice.

Work-related overload syndromes are chiefly associated with the upper limbs, where carpal tunnel syndrome (CTS) plays a leading role. This article analyses methods of diagnosing CTS, with special emphasis on those that can be used by physicians in early diagnosis of CTS in workers doing monotonous work. It also discusses occupational (e.g., assembly work, typing, playing instruments, packaging and work associated with the use of a hammer or pruning scissors) and extra-occupational factors (e.g., post-traumatic deformation of bone elements of the carpal tunnel, degenerative and inflammatory cha

Myositis with sensory neuronopathy.

Inflammatory myopathies (IM) are a heterogeneous group of diseases characterized by immune-mediated damage to skeletal muscle. Sensory abnormalities are rare in patients with IM. We report two patients, one with dermatomyositis and the other with inclusion-body myositis, who presented with unexpected sensory abnormalities due to probable immune-mediated damage to dorsal root ganglia. We emphasize the importance of combined neuroimaging and neurophysiological assessment for proper diagnosis.

Effects of botulinum toxin on strength-duration properties.

Axonal excitability studies have been used in several diseases to investigate the underlying pathophysiology. The threshold tracking technique was developed to measure noninvasively several indices of axonal excitability, such as strength-duration properties. This study investigated the possible effects of botulinum toxin on strength-duration time constant (SDTC) in patients with the symptoms and signs of botulism. The clinical and electrophysiological findings of 13 patients who were admitted to the authors' clinic with botulism signs and symptoms were evaluated in a 5-day period after exposu

Evaluation of work-related carpal tunnel syndrome.

OBJECTIVES: Carpal tunnel syndrome (CTS) is common in the industrial setting but there are still some advocates who argue that CTS is not a work related problem. There are also controversies about the proper way to establish the diagnosis and whether screening for CTS in the industrial setting is warranted. METHODS: A comprehensive literature review. RESULTS: The literature does demonstrate that the prevalence of CTS in the industrial setting is significantly higher than in the general population. Numerous epidemiologic studies have identified independent risk factors, which include repetiti

[Electroneuromyographic study of the radial nerve at the brachium level: standard values and conduction block].

A study aims to design an electroneuromyographic technique of radial nerve investigation and conduction block (CB) definition at the spiral groove level using surface electrodes. Eight healthy volunteers, 17 patients with focal radial nerve neuropathy in the spiral groove, 4 patients with a complete injury of the radial nerve aged from 22 to 54 years have been studied. The M-response was recorded from the extensor digitorum communis. Stimulation was performed at (1) the lateral brachium; (2) the Erb's point; (3) the medical brachium (stimulation of the median and ulnar nerves). Normal values h

Critical reappraisal of referrals to electromyography and nerve conduction studies.

A large number of examinees referred to electromyographic (EMG) laboratories do not have symptoms or signs suggestive of a peripheral nervous system disorder, and the aim of the present study was to check this. All examinees evaluated by the author in a "general" EMG laboratory in the first 4 months of 2002 were included. Data on examinees, referral physicians and diagnoses, clinical symptoms and signs, and electrodiagnostic findings were statistically evaluated. Three hundred examinees, 42% men, were included. A neurological diagnosis was provided in 55% of referrals. Electrodiagnostic abnorm

Time-frequency analysis and estimation of muscle fiber conduction velocity from surface EMG signals during explosive dynamic contractions.

Time-frequency analysis of the surface electromyographic (EMG) signal is used to assess muscle fiber membrane properties during dynamic contractions. The aim of this study was to compare the direct estimation of average muscle fiber conduction velocity (CV) with instantaneous mean frequency (iMNF) of surface EMG signals in isometric and explosive dynamic contractions. The muscles investigated were the vastus lateralis and medialis of both thighs in 12 male subjects. The isometric contractions were at linearly increasing force (0-100% of the maximal voluntary contraction in 10s). The explosive

Inching method in compression neuropathy of the sural nerve distal to the ankle.

We present the case of a 29-year-old electric engineer with compression neuropathy of the left sural nerve due to occupational boots. Routine nerve conduction study of the sural nerve was normal. However, the sensory nerve action potential was not detected more than 3 cm distal to the lateral malleolus, although it returned to normal values after three years. Inching method of the sural nerve may be necessary for detecting compression sural neuropathy distal to the ankle.

Clinical nerve conduction and needle electromyography studies.

The electrodiagnostic study, consisting of nerve conduction studies and needle electromyography, is a useful adjunct to the clinical examination of the peripheral nervous system. The three types of nerve conduction study are motor, sensory, and mixed, of which motor is the least sensitive. Electromyography records the intrinsic electrical activity of muscle fibers, thus providing the physiologic status of muscle function. To interpret the electrodiagnostic study results, the clinician must understand the anatomic and physiologic basis of the studies. Peripheral nerve entrapment initially resul

[Physiological approach to peripheral neuropathy. Conventional nerve conduction studies and magnetic motor root stimulation].

In this communication, I first show some points we should mind in the conventional peripheral nerve conduction studies and later present clinical usefulness of motor root stimulation for peripheral neuropathy. CONVENTIONAL NERVE CONDUCTION STUDIES (NCS): The most important point revealed by the conventional NCSs is whether neuropathy is due to axonal degeneration or demyelinating process. Precise clinical examination with this neurophysiological information leads us to a diagnosis and treatment. Poor clinical examination makes these findings useless. Long standing axonal degeneration sometimes

Focal neuropathy in children with critical illness.

Four children are described who developed focal lower limb nerve palsies following critical illness. Two had clinical and/or neurophysiological evidence of simultaneous generalised critical illness polyneuropathy. The diagnosis was delayed in three patients due to the presence of central motor abnormalities and slow motor recovery. Follow-up from seven months to three years showed minimal or no recovery. In three, a vasculitic skin infarct, compartment syndrome and focal myositis could have caused nerve compression. We suggest that in critical illness peripheral nerves have an increased suscep

Blink reflex and discomplete facial nerve palsy.

BACKGROUND: Electrophysiologic findings of the blink reflex in patients with Bell's palsy are usually said to be either prolonged latencies and/or absent early and middle responses of it. METHODS: Facial nerve conduction and blink reflex studies were performed on a 42-year-old male patient with right-side Bell's palsy. Studies were done using protocols previously validated and published elsewhere. RESULTS: The right compound muscle action potential was not found after stimulation of the right facial nerve as expected. Absence of the short (R1) and middle (R2) responses of the blink reflex we

An unusual presentation of tarsal tunnel syndrome caused by an inflatable ice hockey skate.

BACKGROUND: Tarsal tunnel syndrome is a rare form of entrapment neuropathy. In athletes, it is usually the result of repetitive activity, local injury or a space-occupying lesion. Rarely, athletic footwear has been described as the primary cause of this syndrome. METHODS: A 37-year-old male recreational hockey player was examined clinically and electrophysiologically because of spreading numbness in the toes of his left foot while playing hockey and wearing inflatable ice hockey skates designed to promote a better fit. RESULTS: Clinical and electrophysiological studies revealed evidence of l
