The paper describes the clinico-instrumental diagnostic iter in a 57-year-old patient with amyotrophy of the pelvic and pectoral girdles and of the roots of the limbs, with lively and symmetric R.O.T., and without fasciculation, feeling deficit, bulbar involvement and pathological reflexes; EMG was probative for the neurogenic involvement of all four limbs, whereas muscular enzymes were normal. Laboratory controls showed primary hyperparathyroidism caused by a single operated parathyroid adenoma; other test enabled multiple endocrine adenomatosis to be excluded. The paper discusses the involve

Infectious mononucleosis is a relatively common disease. In this paper, three patients with neurological symptoms related to infectious mononucleosis are described. Patient no. 1 had myelitis, one of the most frequent neurological complications. Patient no. 2 had cerebellar symptoms, with ataxia and disturbance of gait. In this case magnetic resonance revealed vermis atrophy, a finding which, to the best of our knowledge, has not been reported before. Patient no. 3 had bilateral optic neuritis. Repeated serologic tests may be necessary to confirm the diagnosis. Infectious mononucleosis should

Neurologic emergencies during pregnancy are not encountered often but contribute significantly to maternal mortality. This chapter reviews neurologic emergencies with an emphasis on pathophysiology and related nursing care for patients with epilepsy, status epilepticus, eclampsia, intracranial hemorrhage, increased intracranial pressure, ischemic stroke, myasthenia gravis, autonomic hyperreflexia, Wernicke's encephalopathy, and chorea gravidarum.

Alcohol is still the most abused drug today and may affect the peripheral, central, and autonomic nervous system in many ways. There are several pathophysiologic mechanisms that include direct effects through intoxication, withdrawal effects, secondary effects from nutritional problems and systemic diseases, and, of course, syndromes of unknown etiology. The alcoholic is also susceptible to trauma to both the central and peripheral nervous system. In many of the clinical entities, multiple factors play a role.

The involvement of the nervous system in malaria is reviewed in this paper. Cerebral malaria, the acute encephalopathy which complicates exclusively the infection by Plasmodium falciparum commonly affects children and adolescents in hyperendemic areas. Plugging of cerebral capillaries and venules by clumped, parasitized red cells causing sludging in the capillary circulation is one hypothesis to explain its pathogenesis. The other is a humoral hypothesis which proposes nonspecific, immune-mediated, inflammatory responses with release of vasoactive substances capable of producing endothelial da

Neurological diseases, such as multiple sclerosis (MS), Sjögren-Larsson syndrome, Reye's syndrome, and Refsum's syndrome (herediopathica atactica polyneuroformis), and many others afflict millions of persons yearly and have no successful treatment available. A common aspect of these diseases appears to be a lipid imbalance involving the essential fatty acids (EFA), linoleic and linolenic, and trace fatty acids which result from faulty lipid metabolism. It is proposed that treatments for these diseases should be sought through diet and metabolic enzymes rather than drugs.

In any patient with leukemia and new neurologic findings, the first entity that must be considered is direct invasion of the nervous system. If that is not a consideration, one must systematically consider cerebrovascular disorders, CNS infections, or metabolic abnormalities as the cause. Finally, one must not overlook radiotoxicity, chemotoxicity, or both in the differential diagnosis. Although some neurologic disorders are stereotyped in their presentation and are relatively easy to recognize, others continue to perplex the physician. Symptoms may be multifactorial in origin, compounding the

This article reviews the clinical manifestations, diagnosis, and current therapy of four neurologic diseases currently believed to be autoimmune in etiology: myasthenia gravis, Guillain-Barré syndrome, multiple sclerosis, and Reye's syndrome. Nursing care of these patients is very challenging and crucial to the survival and prevention of further complications. Nursing interventions for the potential problems faced by these individuals are described.

Paraneoplastic neurological syndromes are of two types: some are more often seen without than with cancers and may therefore be called "occasionally para neoplastic" (e.g. chronic sensorimotor polyneuropathy and polymyositis), while others are fairly regularly associated with cancers, and particularly with small cell lung carcinoma. In this category falls subacute encephalomyelitis, an entity of broad anatomico-clinical spectrum including limbic encephalitis and subacute sensory neuronopathy; the patient's serum and cerebrospinal fluid may contain neuronal antinuclear antibodies. One type of s

Neurological diseases may induce alterations in sleep which are difficult to interpret owing to the multiple interferences introduced by treatments, associated somatic symptoms and time-related changes in causal lesions. The consequences of vascular or traumatic lesions observed in man confirm the data obtained from experimental lesions created in animals, thereby throwing some light on the roles played by the cerebral cortex, the thalamus and the brain stem nuclei. In degenerative diseases polygraphic recordings show subclinical signs of extension which are exceptionally specific of any parti

In this paper, I described clinical and basic problems on neurology of the aged patients. These studies have been done in various institutions with many co-workers. 1) A PET study revealed some age differences on CBF, CMRO2, or CMRgl. But these results are not so rigid in which much of individual variations should be considered in interpretation. Calendar age is not always compatible to biological age. 2) Saccular aneurysms in the brain artery were found in 7.3% of 1200 routine autopsy series of the aged subjects. Aneurysms with external diameter exceeding 6 mm had been fatally ruptured in 14 …

A 37-year-old white woman presented with bilateral symmetrical ascending motor weakness. Subsequent evaluation revealed she was suffering from infectious mononucleosis. This case demonstrates that one should consider mononucleosis when presented with an idiopathic disease of the nervous system. The classic features of mononucleosis, such as pharyngitis, lymphadenopathy or splenomegaly, need not be present.

Assessment of Leukotriene C4 (LTC4) activity in the serum and cerebrospinal fluid (CSF) specimens of patients with multiple sclerosis (MS), Parkinson's disease, amyotrophic lateral sclerosis (ALS), Myasthenia gravis (MG), Behçet's disease and neuro-Behçet, as well as in normal controls were carried out in order to determine its role in the pathogenesis of neurologic disorders. LTC4 levels were found to be elevated in MS and Behçet patient in comparison with controls. Augmentation of LTC4 levels underlines the fact that leukotrienes may be held responsible the pathogenesis of these disorders.

The medical records of 285 patients at Royal Prince Alfred Hospital with a definite diagnosis of sarcoidosis over the period 1973-87 were reviewed. Fourteen (4.9%) had involvement of the nervous system, in 8 of whom the neurological manifestations were the presenting feature of the disease. The age range of neurosarcoidosis was 22 to 79 years and there was a female preponderance (10:4). Neurological complications (some patients experienced more than one complication) were facial nerve palsy (4), myopathy (3), aseptic meningitis (3), deafness/vertigo (2), hydrocephalus (2), hypopituitarism (2)

Neurological visual field defects represent lesions to the visual pathway, some of which may be life-threatening. It is, therefore, crucial that optometrists understand how to diagnose these lesions, so they may know when, and to whom to refer the patient. The following cases illustrate the management of patients with lesions to the prechiasmal, chiasmal, and postchiasmal visual pathway.
