Postextraction osteomyelitis in a bone marrow transplant recipient.

This report describes a case of mandibular osteomyelitis after a dental extraction in a patient who subsequently underwent bone marrow transplantation (BMT) for lymphoblastic lymphoma. Surgical guidelines consistent with National Cancer Institute recommendations were followed for the extraction, which was performed before initiation of the myelosuppressive conditioning regimen. However, moderate tenderness developed at the extraction site beginning 10 days after marrow infusion. On day 26 the patient became febrile and blood culture-positive for Staphylococcus epidermidis. Radiographs exposed …

[MRT in the diagnosis of mandibular osteomyelitis following radiotherapy].

A prospective study was performed to assess the diagnostic potential of MRI in mandibular osteomyelitis after radiation therapy. A total of 60 MRI studies were performed in 39 patients with lymphoma and carcinoma of the head and neck. Clinically or histologically proven osteomyelitis of the mandible occurred in 10 patients depending on the radiation dose absorbed to bone; 9 of these were unequivocally diagnosed by typical MRI appearance, two cases were false positive. MRI is superior to other diagnostic methods such as conventional radiology and clinical examination. It should be performed rou…

Osteomyelitis due to Mycobacterium haemophilum in a patient with AIDS.

A case of osteomyelitis resulting from infection with Mycobacterium haemophilum in a patient with the Acquired Immunodeficiency Syndrome (AIDS) and Hodgkin's disease is described. The clinical features and response to therapy are examined and compared to previous reported cases. Mycobacterium haemophilum infection in immunocompromised patients usually results in cutaneous lesions but osteomyelitis may be the presenting feature. Tissue samples should be obtained early for microbiological examination and treatment should consist of surgical debridement and appropriate antimicrobial therapy.

[An unusual bone scintigram in osteomyelitis. A case report].

Despite hyperemia in the blood-pool phase of bone scintigraphy with 99mTc-MDP there was an decreased concentration of the nuclide in a 7-year-old boy with acute osteomyelitis of the femur. In addition to inflammatory destruction of the bone tissue, the other reason for this surprising scintigraphic finding might be a reduced blood flow because of thrombotic occlusion or compression of bone-supplying vessels.

Musculocutaneous flaps for the treatment of chronic osteomyelitis of the heel.

Treatment for chronic osteomyelitis with musculocutaneous and muscular flaps is a highly effective modern method. It is effective even in such unfavourable localizations as the calcaneus. The scope of use provided by muscles in the plantar region is wide enough to permit the treatment of foci in the heel bone of diverse localizations. The range of possibilities includes use of m. abductor digiti quinti. A muscular or musculocutaneous flap elevated from the muscle is well suited for the treatment of defects on the external side of the heel. The group consisted of six patients with osteomyelitis…

Malignant fibrous histiocytoma in the course of chronic osteomyelitis.

Malignant fibrous histiocytoma (MFH) was found in the femur of a 28-year-old man who had suffered from chronic osteomyelitis. The patient had undergone surgery for a valgus knee deformity and the procedure was complicated by chronic osteomyelitis which lasted for 12 years. Four months after the onset of painful symptoms related to the occurrence of MFH, gross bone destruction with pathologic fracture and pulmonary metastases were present. The patient died 4 months later. Histopathological and immunohistochemical examinations indicated the diagnosis of MFH.

Nuclear medicine techniques in septic arthritis and osteomyelitis.

Imagers are consulted to confirm the presence of acute osteomyelitis, to evaluate disease activity and chronic osteomyelitis, and to confirm the presence of a sequestrum in active chronic osteomyelitis. Scintigraphy remains the most sensitive and specific modality for the diagnosis of acute osteomyelitis and the evaluation of activity of chronic osteomyelitis. Computed tomography is the best examination for the confirmation of the presence of sequestra. In this article, the authors review the various radionuclide techniques, their applications in the different clinical settings, and the way in…

[Neonatal osteomyelitis. Study of a series of 35 cases].

After a brief literature review, we analyze the results obtained with a retrospective study of 35 neonatal osteomyelitis diagnosed between 1-January-75 and 31-December-87. The valuated frequency was of 0.40% alive newborns. Between the antecedents, we find previous neonatal sepsis in 68% of the cases. The clinical general findings were less apparent, emphasizing among the local symptoms the pain to passive mobilization and swelling. From acute phase reactants, this study rebounds the high sensitivity of C reactive protein and globular sedimentation rate. The most frequently germ isolated was S…

[Biochemical criteria of the inflammatory process in spinal osteomyelitis].

Patterns of collagen metabolism were studied in 36 patients with hematogenic vertebral column osteomyelitis simultaneously with evaluation of C-reactive protein, haptoglobin and ceruloplasmin in blood and consideration of the clinico-morphological steps of the disease. Hyperhydroxyprolineuria proved to be a more informative evidence for inflammation in vertebral column as compared with the proteins of acute phase of inflammation importance of hydroxyproline measurement in biological fluids for evaluation of the osteomyelitis development was elevated as the inflammation activity decreased (whic…

Malignant external otitis and osteomyelitis of the base of the skull.

If malignant external otitis is not treated properly and until all evidence of infection has disappeared, it is likely to recur either in the external auditory canal and/or in the bone marrow at the base of the skull. Unremitting headache is the only early symptom of the latter and may occur in the absence of any signs of local ear disease. A conductive hearing loss is the most common additional symptom and is caused by fluid in the middle ear. If not diagnosed and treated adequately and properly, the infectious process in the bone marrow involves the soft tissues at the skull base and results

Ketoconazole in the treatment of osteomyelitis due to Candida albicans.

Osteomyelitis caused by Candida albicans is a rare condition. The authors report its occurrence as an infective complication in the fractured phalanx of a 3-year-old boy. The infection was first thought to be due to Staphylococcus aureus, but a course of cloxacillin was unsuccessful, and when cultures of the injured finger grew C. albicans, a 3-month course of ketoconazole orally was begun. Ketoconazole was prescribed because, unlike amphotericin B, the antibiotic usually used in such cases, it is not nephrotoxic, it can be taken orally and it has proved successful in other reported cases of o…

Neonatal cranial osteomyelitis: a complication of fetal monitoring.

Use of a scalp electrode facilitates and adds precision to electronic fetal monitoring. We report an instance of neonatal osteomyelitis due to infection of the electrode insertion site. Electrode-associated soft and bony tissue infections are most commonly mediated by cervicovaginal microflora and appear clinically and microbiologically distinct from non-electrode-associated cases. Risk assessment, antiseptic care, and observation of the electrode insertion site are recommended. This report emphasizes the potential morbidity of scalp electrode use and suggests means to prevent or mitigate this…

Acute hematogenous osteomyelitis: a model with trauma as an etiology.

The etiology of acute hematogenous osteomyelitis (AHO) remains unknown, and the disease has no laboratory model. We describe an AHO model in the rabbit which resembles the human disease. The model was developed on the premise that previous attempts had omitted an essential etiologic factor other than the bacteria. Six-week-old white New Zealand rabbits were divided into three different groups. Group I received an injury to the proximal tibial physeal plate, group II received a standardized bacteremia with Staphylococcus aureus, and group III received both the fracture and the bacteremia. The p…
