Fifty patients underwent modified high varicocelectomy as outpatients. Twelve of the 22 unilateral varicocelectomies were performed under local anesthesia. All patients tolerated the procedures well, and none required admission to the hospital. The modified high approach, which exposes the area above the internal inguinal ring and of the posterior spermatic cord, is straightforward and insures that both internal spermatic and cremasteric veins can be ligated. Use of the operating microscope prevents the inadvertent ligation of the testicular artery and lymphatics.

In order to compare the efficacy of permucosal needle drainage with that of incision and drainage in the outpatient management of peritonsillar abscess, 52 patients with aspiration-proven peritonsillar abscess were entered into a randomized, prospective protocol. Clonidine hydrochloride (an alpha-2 adrenergic agonist) and naltrexone hydrochloride (an opioid antagonist), given in combination, provided a safe and effective treatment of abrupt opioid withdrawal over 5 days in an outpatient/day setting. Maloney's mercury weighted dilators have been used to dilate benign oesophageal strictures in 109 patients in the outpatient department. A total of 524 dilatations have been performed. Following dilatation two patients had a small haematemesis but there have been no oesophageal perforations. Maloney's dilators can be used in the outpatient department effectively to dilate benign oesophageal strictures and the risk of complications is low.

A method of treatment of patients (224) with the postthrombotic disease of lower extremities under conditions of out-patient clinics is described. At the first stage of treatment (a preparatory one) the patients were given drugs blocking biogenic substantias, vasodilative drugs and physiotherapeutic treatment. The second stage included the putting of the Unna-Kefer compressive bundage for a long time. The supporting therapy followed. Remote results were favourable.

One hundred and two patients, presenting at the outpatient departments of two Boston teaching hospitals, underwent clinical examination and venography. History, physical examination and presence of risk factors were unreliable in the diagnosis of deep venous thrombosis.
