Combined Use of Savi Scout and Magtrace for Non-Palpable Breast Cancer: A Retrospective Study.

BACKGROUND: Non-palpable breast cancer management has become less invasive and more efficient, driven by advances in tumor localization and sentinel lymph node (SLN) identification. Savi Scout, using a radar-based localization system, can be implanted into the tumor before neoadjuvant systemic therapy (NST). Magtrace, a superparamagnetic iron oxide nanoparticle for SLN detection, can be administered preoperatively. This study evaluated combining Savi Scout and Magtrace for breast-conserving surgery and SLN biopsy in non-palpable breast cancer. METHODS: Patients who underwent breast-conserving

Wearable activity data can predict functional recovery after musculoskeletal injury: Feasibility of a machine learning approach.

Delayed functional recovery after injury is associated with significant personal and socioeconomic burden. Identification of patients at risk for a prolonged recovery after a musculoskeletal injury is thus of high relevance. The aim of the current study was to show the feasibility of using a machine learning assisted model to predict functional recovery based on the pre- and immediate post injury patient activity as measured with wearable systems in trauma patients. Patients with a pre-existing wearable (smartphone and/or body-worn sensor), data availability of at least 7 days prior to their i

siRNA-mediated BCR-ABL silencing in primary chronic myeloid leukemia cells using lipopolymers.

Despite development of effective tyrosine kinase inhibitors for treatment of chronic myeloid leukemia (CML), some patients do not effectively respond to the therapy and can display resistance in response to the drug therapy. To develop an alternative approach to CML therapy, we are exploring siRNA mediated silencing of the primary CML oncogene, BCR-ABL, by using non-viral (polymeric) delivery systems. In this study, a group of lipopolymers derived from low molecular PEIs substituted with linoleic acid (LA), α-linolenic acid (αLA) and cholesterol (Chol) was investigated for the first time for s

High-accuracy morphological identification of bone marrow cells using deep learning-based Morphogo system.

Accurate identification and classification of bone marrow (BM) nucleated cell morphology are crucial for the diagnosis of hematological diseases. However, the subjective and time-consuming nature of manual identification by pathologists hinders prompt diagnosis and patient treatment. To address this issue, we developed Morphogo, a convolutional neural network-based system for morphological examination. Morphogo was trained using a vast dataset of over 2.8 million BM nucleated cell images. Its performance was evaluated using 508 BM cases that were categorized into five groups based on the degre

Diagnostic outcomes and temporal changes in skin tests and specific immunoglobulin E levels following Hymenoptera sting reactions: single-center analysis of 194 patients.

Hymenoptera stings are a common occurrence worldwide, with a significant proportion of the general population experiencing at least one sting during their lifetime. The order Hymenoptera includes several species, such as honey bees, hornets, bumblebees, and yellow jackets, that cause hypersensitivity reactions. The most common form of hypersensitivity is systemic sting reaction mediated by specific immunoglobulin E (sIgE) to venom components. The prevalence of systemic sting reaction in the general population is estimated to be approximately 3%, while gross local reactions are reported in 2.4-

Multi-omics approaches to parkinsonism: genomic, proteomic, and non-coding RNA perspectives.

Parkinson's disease (PD) is a progressive neurodegenerative disorder with significant variability associated with substantial loss of dopaminergic neurons in the substantia nigra. Currently, there are limited opportunities for intervention. There is a lack of reliable biomarkers to identify patients with PD, which poses a challenge for clinicians. In this review, recent advances in PD biomarker research, including genomic, epigenomic, proteomic, and non-coding RNA, will be highlighted, with particular emphasis on integrating omics for precision medicine. In addition, the current understanding

Development and validation of multivariable prognostic machine learning models to identify patients at risk for inadequate bowel preparation when undergoing colonoscopy: a prospective, multicentre study.

OBJECTIVE: Inadequate bowel preparation impairs the accuracy of colonoscopy and increases the burden on patients and healthcare systems. Consequently, the quality of bowel preparation is an important quality indicator. We aim to develop and validate multivariable prognostic models for the identification of patients at risk of inadequate bowel preparation using machine learning (ML). METHODS: Demographic and clinical data from consecutive patients ≥18 years of age who underwent colonoscopy at six centres in Germany were prospectively collected. Adequate bowel preparation was defined as Boston

Echocardiographic Assessment Before, During, and After Impella Positioning: State of the Art.

Echocardiographic assessment is essential for evaluating patients with cardiogenic shock (CS) and determining their potential need for mechanical circulatory support (MCS) implantation. The use of Impella devices has increased significantly in recent years, paralleling the growing recognition of their hemodynamic benefits in selected patient populations. As the clinical experience with these devices has expanded, the need for a more standardized imaging approach has emerged. Both transthoracic echocardiography (TTE) and transesophageal echocardiography (TEE) play complementary roles in guiding

CPGPrompt: translating clinical guidelines into large language model-executable decision support.

OBJECTIVE: Clinical practice guidelines (CPGs) provide evidence-based recommendations for patient care; however, integrating them into artificial intelligence (AI) remains challenging. Previous approaches, such as rule-based systems or black-box AI models, face significant limitations, including poor interpretability, inconsistent adherence to guidelines, and narrow domain applicability. To address this, we develop and validate CPGPrompt, an auto-prompting system that converts narrative clinical guidelines into large language models (LLMs). MATERIALS AND METHODS: Our framework translates CPGs

Precision therapeutic strategies for Alzheimer's disease: Amyloid β-targeted foundations and multimodal next-generation approaches.

Alzheimer's disease (AD) is the leading cause of dementia and a significant unmet medical challenge, pathologically characterized by amyloid β (Aβ) aggregation, tau hyperphosphorylation, synaptic dysfunction, and chronic neuroinflammation. Although Aβ has long been a central therapeutic target, clinical translation has historically been hindered by late-stage intervention, inadequate blood-brain barrier (BBB) penetration, and the molecular heterogeneity of AD. Recent advances with Aβ-targeted monoclonal antibodies, particularly lecanemab and donanemab, have provided the first clinical evidence

Recurrent pancreatitis and sepsis in glycogen storage disease type Ia caused by complex heterozygous mutations in 2 sisters: Case report.

RATIONALE: Glycogen storage disease (GSD) is a glycogen metabolism disorder caused by congenital enzyme defects, with type I being the most common. Owing to the rarity of glycogen storage disease type Ia (GSD Ia) and the involvement of diverse systems, patients are prone to delayed diagnosis and inappropriate treatment. Additional studies are required to standardize the diagnosis and treatment of GSD Ia. PATIENT CONCERNS: We report 2 cases of GSD Ia that occurred in 2 sisters. The elder sister also had recurrent pancreatitis, and the pancreatic pseudocyst rupture resulted in sepsis, portal hy
