Reoperative Augmentation Mammoplasty: An Algorithm to Optimize Soft-Tissue Support, Pocket Control, and Smooth Implant Stability with Composite Reverse Inferior Muscle Sling (CRIMS) and its Technical Variations.

BACKGROUND: Reoperative augmentation mammoplasty (RAM) is a challenging procedure, with the highest rates of complications and revision. Complications include implant malposition, lateral displacement, bottoming out, and rotation. These deformities can be addressed with various procedures, but the pocket control and stability of the new smooth implant surface may present limitations. OBJECTIVES: This study revisits a previously described predictable approach in primary breast augmentation and defines a surgical treatment algorithm for RAM technique selection. METHODS: Between 2017 and 2021,

Impact of aluminosilicates on productivity, carcass traits, meat quality, and jejunum morphology of broiler chickens.

The aim of the study was to compare production results and quality of meat, as well as histological features of the jejunum in broiler chickens administered feed with 1% addition of zeolite or halloysite, with the addition of aluminosilicates to litter (4.50 kg/m2) throughout the rearing. In the experiment, 300 male broilers were used for 6 wk. They were divided into 3 groups, each of 10 repetitions (10 birds each). Group 1 was a control, halloysite was added to feed and litter in group 2, while zeolite was applied instead of halloysite in group 3. After rearing, 10 selected birds from each gr

Standard bipolar surface EMG estimations mischaracterize pectoralis major activity in commonly performed tasks.

The pectoralis major assists in several shoulder movements, such as humeral vertical and horizontal adduction, flexion, extension, and internal rotation. Despite its involvement in numerous functional activities, its role in typical shoulder function is ambiguous. Due to this, its purpose in arm movement is largely diminished. However, mounting evidence associates pectoralis major injuries to long-term debilitating arm disability. Therefore, a more deliberate investigation of its role in typical shoulder function is paramount. The purpose of this paper is to outline the current limitations in

Spontaneus subscapular abscess. A clinical case.

Infections of the subscapular space are very infrequent entities, which is why their diagnosis (for which it is crucial to carry out magnetic resonance imaging or, failing that, computerized tomography) can prove complicated. This difficulty in making the diagnosis conditions the speed of treatment (surgical draining that can be accompanied by antibiotherapy), which is crucial for the medium and long-term prognosis. We present the case of a patient who developed a spontaneous subscapular abscess that was drained using a delto-pectoral approach, with the subscapular space accessed via a medial

Functional integrity of the shoulder joint and pectoralis major following subpectoral implant breast reconstruction.

Subpectoral implants for breast reconstruction after mastectomy requires the surgical disinsertion of the sternocostal fiber region of the pectoralis major. This technique is associated with significant shoulder strength and range of motion deficits, but it is unknown how it affects the underlying integrity of the shoulder joint or pectoralis major. The aim of this study was to characterize the long-term effects of this reconstruction approach on shoulder joint stiffness and pectoralis major material properties. Robot-assisted measures of shoulder strength and stiffness and ultrasound shear wa

The pectoral fin muscles of the coelacanth Latimeria chalumnae: Functional and evolutionary implications for the fin-to-limb transition and subsequent evolution of tetrapods.

To investigate the morphology and evolutionary origin of muscles in vertebrate limbs, we conducted anatomical dissections, computed tomography and kinematic analyses on the pectoral fin of the African coelacanth, Latimeria chalumnae. We discovered nine antagonistic pairs of pronators and supinators that are anatomically and functionally distinct from the abductor and adductor superficiales and profundi. In particular, the first pronator and supinator pair represents mono- and biarticular muscles; a portion of the muscle fibers is attached to ridges on the humerus and is separated into two mono

Effect of monochromatic light stimuli during embryogenesis on muscular growth, chemical composition, and meat quality of breast muscle in male broilers.

This study was conducted to evaluate the effect of monochromatic light stimuli during embryogenesis on breast muscle growth, chemical composition, and meat quality of male broilers. Fertile broiler eggs (Arbor Acres; n = 1,320) were preweighed and randomly assigned to 1 of 3 treatment groups in 3 modified incubators: 1) control group (in dark condition), 2) monochromatic green light group (560 nm), and 3) monochromatic blue light group (480 nm). The monochromatic lighting systems sourced from light-emitting diode lamps and were equalized at the intensity of 15 lx at eggshell level. After hatch

An unusual union of the intercostobrachial nerve and the medial pectoral nerve.

Variations in the branching pattern of the intercostobrachial nerve have been known to complicate dissection during mastectomy and other procedures involving the axilla. We present the case of an 87-year-old Caucasian female, in whom the intercostobrachial nerve joined with a separate branch from the medial pectoral nerve. The clinical consequences of such a variation may include pectoral muscle motor loss, in addition to the commonly reported sensory loss resulting from the accidental injury or intentional sacrifice of the intercostobrachial nerve during axillary dissection.

A case of ductal carcinoma in situ of breast with Poland syndrome.

INTRODUCTION: A 51-year-old woman was diagnosed with a rare case of multi-focal ductal carcinoma in situ (DCIS) of the breast associated with Poland syndrome. CLINICAL PICTURE: Physical examination showed mild hypoplasia of the left breast with microcalcifications on mammogram. Intraoperatively, there was complete absence of the pectoralis major, which precluded the insertion of a breast implant. TREATMENT AND OUTCOME: She underwent a skin-sparing mastectomy and autologous microsurgical free flap reconstruction with deep inferior epigastric perforator (DIEP) flap. Postoperative recovery was

Pectoralis major myocutaneous pedicled flap in head and neck reconstruction: retrospective review of indications and results in 244 consecutive cases at the Toronto General Hospital.

The pectoralis major myocutaneous pedicled flap (PMMPF) has been considered to be the "workhorse" of pedicled flaps for head and neck reconstruction, and several series of PMMPF procedures have been reported in the literature. Between 1983 and 1997, 244 reconstruction procedures using the PMMPF were carried out on 229 patients by the Otolaryngology-Head and Neck Surgery Department at the Toronto General Hospital. Pectoralis major myocutaneous pedicled flap reconstructions were completed after ablation of cancer in the following sites: oral cavity, 113; oropharynx/hypopharynx, 50; larynx, 59; a

Chest radiograph appearances after head and neck flap reconstructive surgery.

The chest radiographs of 17 patients who had undergone pectoralis major myocutaneous flap (PMMF) reconstruction for head and neck cancers were reviewed. Surgical procedure correlated well with radiographic findings, which consisted of the following signs: abnormal axillary and supraclavicular folds, replacement of normal vertical neck fold by an oblique fold, soft tissue mass and loss of facial plane at base of the neck. Unilateral radiolucent hemithorax was present in 41% of cases. In females, loss of breast shadow simulating the post-mastectomy appearance was present.

A simplified approach to the repair of pediatric pectus deformities.

For the past 14 years, a simplified operation utilizing a metal strut for internal fixation has been used to repair pectus anomalies in 123 children. Subperichondrial cartilage resection is preformed through small incisions in the pectoral muscles. No sternal osteotomy required. A malleable strut is passed transsternally and removed in four to six months, frequently under local anesthesia. All children who underwent this procedure were discharged within five to six days, and no transfusions were necessary. The use of the technique has shortened operative time and decreased the necessity for ex

Poland syndrome: a concept of pathogenesis based on limb bud embryology.

A study of 15 patients as well as a review of the literature was made to determine the etiology of Poland syndrome. Neither the associated pathologic features nor any pattern of prenatal factors were helpful in revealing an etiology. A theory of primary nerve root dysgenesis was disproven. However, a concept of a localized insult at a particular embryonic stage based on known limb bud development is presented as a possible explanation of pathogenesis.

The axillary arch - morphological and histological study with clinical importance.

The latissimus dorsi is a muscle of the back which forms the posterior fold of the axilla and its tendon twists to insert into the floor of the intertubercular sulcus of the humerus. Occasionally, the muscle has a muscular slip which crosses the axilla to insert into the pectoralis major. This muscular slip is often termed as "axillary arch." In the present study, we report bilateral axillary arch detected in a 45-year-old male cadaver. The average vertical length of the axillary arch measured 7 cm. The average maximum width of the uppermost, middle and lower part of the arch measured 2, 3.5 a
