Anesthesia for major general surgery in neonates with complex cardiac defects.

Centers with large cardiac workloads may be presented with neonates who need major general surgery before correction or palliation of a serious cardiac defect. This is still a rare situation with only three short case reports available in the medical literature (1-3). We have reviewed the anesthetic and analgesic regimens of 18 such neonates who presented to the Birmingham Children's Hospital in the 4-year period 2004-2007. These children require meticulous preoperative evaluation and although it might be anticipated that they would pose a serious challenge to anesthetists, in reality with tho

A comparison of parenteral and electronic dental anesthesia during operative procedures in children.

The effectiveness of electronic dental anesthesia (EDA) for pain control during restorative procedures was compared with local anesthesia (LA) in 108 pediatric patients aged 6-12. One tooth was treated with LA and another with EDA at the same appointment. The pain levels during restorative treatment were assessed using a color scale. Behavior also was assessed using sound, eye, and motor (SEM) scale. Although the success rate of EDA was less than that of LA, there was no significant difference between the two methods. On the other hand, 63% of the patients preferred EDA while 27.8% preferred L

Epidural versus Patient-Controlled Analgesia after Pediatric Thoracotomy for Malignancy: A Preliminary Review.

Introduction The use of thoracic epidural is standard in adult thoracotomy patients facilitating earlier mobilization, deep breathing, and minimizing narcotic effects. However, a recent randomized trial in pediatric patients who undergo repair of pectus excavatum suggests patient-controlled analgesia (PCA) produces a less costly, minimally invasive postoperative course compared with epidural. Given that thoracotomy is typically less painful than pectus bar placement, we compared the outcomes of epidural to PCA for pain management after pediatric thoracotomy. Methods A retrospective review of 1

Shifting the pediatric choledocholithiasis paradigm: Development of a surgery-first clinical practice guideline at a tertiary children's hospital.

BACKGROUND: Pediatric choledocholithiasis has traditionally been managed with a two-stage endoscopy-first (EF) approach involving ERCP followed by cholecystectomy. This strategy increases hospital length of stay (LOS), procedural complexity, and exposure to anesthesia. A growing body of evidence supports a surgery-first (SF) paradigm. METHODS: A multidisciplinary team at a tertiary pediatric center developed a clinical practice guideline (CPG) in 2023 using the Pediatric DUCT score to risk-stratify patients and prioritize early surgical consultation. The guideline was informally piloted in Ap

Ultrasound-guided Techniques for Perioperative Analgesia in Pediatric Lower Abdominal Surgeries: Quadratus Lumborum Block with Bupivacaine versus Caudal Bupivacaine and Neostigmine.

BACKGROUND: Ultrasound-guided  regional anesthesia techniques for perioperative analgesia in pediatric patients scheduled for lower abdominal surgeries can be achieved either by quadratus lumborum block (QLB) or caudal block (CB). Neostigmine was co-administered with caudal bupivacaine to shorten the onset and extend the duration of analgesia. OBJECTIVES: This study aimed to compare between 2 ultrasound-guided techniques used for perioperative analgesia (QLB with bupivacaine vs. CB with bupivacaine/neostigmine) regarding the total amount of rescue analgesic (acetaminophen mg/kg) used for pain

GNAO1 Mutation-Induced Pediatric Dystonic Storm Rescue With Pallidal Deep Brain Stimulation.

Dystonic storm or status dystonicus is a life-threatening hyperkinetic movement disorder with biochemical alterations due to the excessive muscle contractions. The medical management can require pediatric intensive care unit admission and a combination of medications while the underlying trigger is managed. Severe cases may require general anesthesia and paralytic agents with intubation and may relapse when these drugs are weaned. Deep brain stimulation of the globus pallidum has been reported to terminate dystonic storm in several pediatric cases. We present a 10-year-old boy with a de novo G

Developing a pediatric endovascular thrombolysis program: a single-center experience.

Deep venous thrombosis (DVT) is being increasingly recognized as a significant issue in children. Despite the low incidence of DVT, the risks of pulmonary embolism and death in children are significant. Post-thrombotic syndrome, a syndrome of chronic venous insufficiency, can have long-term adverse consequences in children and adolescents. Adult studies have shown that catheter-directed therapy can reduce the incidence of post-thrombotic syndrome. Safety of catheter-directed therapy in adolescents has also been demonstrated. These reasons compelled us to institute a pediatric endovascular thro

Endoscopic ultrasound-guided pancreatic pseudocyst drainage in pediatric patients.

BACKGROUND: Pancreatic pseudocyst (PPC) is a well-encapsulated peri-pancreatic fluid collection that occurs at least 4 weeks after acute pancreatitis. The prevalence of PPC is 8%-41% among patients with a history of pancreatitis. Management includes conventional surgery and percutaneous radiological or endoscopic approaches. Endoscopic ultrasound (EUS)-guided drainage of PPC appears to be a safe and less invasive treatment option. Its efficacy is well established in the adult population. However, data regarding its usage in the pediatric population are limited. AIM: To determine the safety an

Rates, Outcomes and Predictors of Immediate and Early Extubation After Pediatric Liver Transplantation.

INTRODUCTION: Pediatric patients undergoing liver transplantation (LT) often require postoperative mechanical ventilation, which can increase the risk of complications such as ventilator-associated pneumonia (VAP) and higher intensive care unit (ICU) resource utilization. Advances in perioperative care have made immediate intraoperative extubation (IE) and early extubation (EE) feasible and safe; however, evidence in pediatric LT remains limited, and standardized extubation criteria are lacking. This study aimed to evaluate the incidence, outcomes, and predictors of successful IE/EE after pedi

Management of Impacted Primary Molar Obstructing Permanent Tooth Eruption in a Pediatric Patient: A Case Report.

Tooth impaction and eruption failure present common challenges in pediatric dentistry. We report a case of a 10-year-old boy of Indian origin presenting with a missing left mandibular primary second molar and impacted first permanent molar. Radiographic examination revealed an ankylosed primary molar obstructing the path of an unerupted premolar. Surgical extraction under local anesthesia was performed successfully, with postoperative management, including antibiotics and analgesics. Follow-up examinations demonstrated favorable outcomes, prompting referral to an orthodontist for further align

Association of dexmedetomidine use with haemodynamics, postoperative recovery, and cost in paediatric anaesthesia: a hospital registry study.

BACKGROUND: Dexmedetomidine utilisation in paediatric patients is increasing. We hypothesised that intraoperative use of dexmedetomidine in children is associated with longer postanaesthesia care unit length of stay, higher healthcare costs, and side-effects. METHODS: We analysed data from paediatric patients (aged 0-12 yr) between 2016 and 2021 in the Bronx, NY, USA. We matched our cohort with the Healthcare Cost and Utilization Project-Kids' Inpatient Database (HCUP-KID). RESULTS: Among 18 104 paediatric patients, intraoperative dexmedetomidine utilisation increased from 51.7% to 85.7% bet

Effect of Dexmedetomidine on Rocuronium-Induced Neuromuscular Blockade and Intubation Condition in Children: A Randomized Controlled Trial.

BACKGROUND: Dexmedetomidine is an a2-adrenoreceptor agonist with sedative, analgesic, and anxiolytic properties. It has gained remarkable attention in the adult and pediatric population predominantly because of its minimal respiratory depression. However, in the pediatric population, very limited information is available regarding the action of intravenous dexmedetomidine on neuromuscular blockade and on anesthesia induction. The aim of this study was to investigate the impact of dexmedetomidine on rocuronium-induced neuromuscular blockade and on anesthesia induction with propofol in children.

Routine flumazenil administration confounds emergence time comparisons between remimazolam and propofol: a methodological concern.

This letter to the editor identifies a critical methodological concern in a recent clinical trial by Qin et al. that compared the emergence profiles of remimazolam and propofol in pediatric anesthesia. We argue that the study's findings are confounded by a fundamental"asymmetry in interventions". The remimazolam group routinely received flumazenil for pharmacological reversal, while the propofol group emerged spontaneously via natural metabolism.

Propofol-related infusion syndrome in critically ill pediatric patients: coincidence, association, or causation?

Over the past two decades numerous reports have described the development of a propofol-related infusion syndrome (PRIS) in critically ill adult and pediatric patients who received continuous infusion propofol for anesthesia or sedation. The syndrome is generally characterized by progressive metabolic acidosis, hemodynamic instability and bradyarrhythmias that are refractory to aggressive pharmacological treatments. PRIS may occur with or without the presence of hepatomegaly, rhabdomyolysis or lipemia. To date, the medical literature contains accounts of 20 deaths in critically ill pediatric p

Adult Liver Transplant Anesthesiology Practice in the Post-COVID-19 Era in the United States: Survey From the Society for the Advancement of Transplant Anesthesia.

BACKGROUND: Liver transplant (LT) anesthesiology has evolved alongside rising LT volumes in the United States. The formation of dedicated LT anesthesia teams (LTATs), formalization of director of LT anesthesia (DLTA), and standardization of LT anesthesia programs (LTAPs) have advanced the field. However, the current post-COVID-19 state of these initiatives remains poorly characterized. METHODS: From May 1 to June 31, 2024, we conducted a national electronic survey of 101 DLTAs representing 151 active adult LT programs in the U.S., excluding pediatric programs and programs without identifiable

Application of wireless ultrasound for guided caudal anesthesia in children undergoing concealed penis surgery.

Caudal anesthesia alleviates the strong pain endured by children during surgical treatment for concealed penis. In the traditional method, anesthesiologists identify the puncture point using the 'blind probe' method, which leads to anesthesia induction failure in children. Ultrasound has recently gained wide attention for its guidance in peripheral nerve block analgesia. However, the clinical significance of wireless ultrasound - guided caudal anesthesia technology in children remains unexplored. This study investigated the clinical value of wireless ultrasound - guided caudal anesthesia in ch

Perioperative hypersensitivity reactions during childhood and outcomes of subsequent anesthesia.

BACKGROUND: Pediatric perioperative hypersensitivity reactions are rare, and possibly life-threatening. Identification of precise etiology is crucial to circumvent future re-exposures. AIMS: We aim to evaluate the clinical features and triggers of perioperative hypersensitivity reactions in children, and determine the outcomes of subsequent general anesthesia. METHODS: A retrospective study was performed with patients who underwent skin testing for general anesthesia between 2007 and 2019. We noted demographic features and skin tests (neuromuscular blocking agents, induction agents, and anti
