Clinical and radiological assessment of grape seed extract versus mineral trioxide aggregates in primary molar pulpotomy: A randomized controlled clinical trial.

BACKGROUND: This study aimed to assess the radiological and clinical outcomes of mineral trioxide aggregates (MTA) versus grape seed extract (GSE) when used as a dressing material in primary teeth undergoing pulpotomy. METHODS: This was a prospective split-mouth randomized clinical study. A total of 32 primary teeth were included and split into 2 groups: MTA was used in group I, and GSE was used in group II. A clinical and radiographic follow-up was conducted for 6 months postoperatively. Periapical radiographs were taken for all treated teeth at 3- and 6-month follow-up visits. Radiographic

Rapidly Progressing Radicular Cyst in an Endodontically Treated Tooth: Diagnostic Challenges, Advanced Imaging, and Surgical Management.

Radicular cysts in teeth that have undergone root canal therapy (RCT) are uncommon but clinically important, often presenting unique challenges for diagnosis. Although rare, radicular cysts can progress rapidly, leading to symptoms, such as discomfort or swelling shortly after the initial treatment. In this case, a 29-year-old female experienced mild discomfort localized to the maxillary right first premolar, which had received RCT several months prior. The patient had no history of systemic conditions or chronic infections. Upon clinical examination, tenderness was noted in the periapical reg

Treatment of invasive cervical resorption: an analysis of results using topical application of trichloracetic acid, curettage, and restoration.

OBJECTIVE: The purpose of this study was to carry out a clinical evaluation of the treatment of invasive cervical resorption. METHOD AND MATERIALS: Topical application of a 90% aqueous solution of trichloracetic acid, curettage, nonsurgical root canal treatment where necessary, and restoration with glass-ionomer cement were performed on 94 patients with a total of 101 affected teeth. A minimum of 3 years' follow-up was required, unless failure occurred before that time, in which case that treatment was included. Teeth were divided into four classes, depending on the extent of the resorptive p

Dental X-ray status of patients admitted for total hip replacement.

The dental status of 50 patients undergoing total hip replacement (THR) was evaluated. Fourteen of the patients had no teeth. The teeth of the remaining 36 patients were in poor condition. Decay was severe in 12. Periapical infectious foci were evident in the panoramic radiographs of 24 patients. Nearly all of the 36 patients with teeth had advanced periodontal disease, as evidenced by horizontal and vertical destruction of alveolar bone. Thirty-four of the 36 dentate patients were considered to be at risk of bacteraemia because of their dental condition. The study shows the value of preoperat

Combined Piezoelectric and Laser Therapy for Severely Compromised Teeth: Two Case Reports.

BACKGROUND: Deep periodontal defects (probing depths: 10-15 mm) are traditionally considered hopeless and are frequently recommended for extraction. This report describes two patients demonstrating substantial periodontal tissue healing using a minimally invasive, nonsurgical approach combining piezoelectric instrumentation with Nd:YAG laser therapy, without bone grafts or biologic adjuncts. METHODS: Two systemically healthy, nonsmoking adults with periodontally hopeless teeth underwent subgingival scaling with chlorhexidine irrigation, followed by Nd:YAG laser therapy. Case 1 received delaye

Clinical trials using dental stem cells: 2022 update.

For nearly 20 years, dental stem cells (DSCs) have been successfully isolated from mature/immature teeth and surrounding tissue, including dental pulp of permanent teeth and exfoliated deciduous teeth, periodontal ligaments, dental follicles, and gingival and apical papilla. They have several properties (such as self-renewal, multidirectional differentiation, and immunomodulation) and exhibit enormous potential for clinical applications. To date, many clinical articles and clinical trials using DSCs have reported the treatment of pulpitis, periapical lesions, periodontitis, cleft lip and palat

Sidt2 Knockdown Aggravates Apical Periodontitis Via Autocrine Tumour Necrosis Factor-Alpha.

OBJECTIVE: Apical periodontitis (AP) is characterised by inflammatory infiltration and periapical bone loss, accompanied by high expression of tumour necrosis factor-alpha (TNF-α). SID1 transmembrane family member 2 (Sidt2) is a lysosomal membrane protein associated with inflammation. However, the effect of Sidt2 on AP remains unknown. METHODS: We established an AP mouse model, and then the adenovirus-associated virus was used to inhibit Sidt2 expression in the AP model. Periapical lesions were detected using micro-computed tomography. Haematoxylin and eosin staining, tartrate-resistant acid

Direct Clinical Evidence of Long-term Hard-tissue Repair after Miniature/Tampon Pulpotomy: An Accidental Re-entry Case after Six Years.

Vital pulp therapy (VPT) has become an established biologically-based treatment for preserving pulp vitality in mature permanent teeth with irreversible pulpitis. Although long-term clinical success has been widely reported, direct clinical evidence demonstrating the biological outcome beneath the pulp-capping material years after treatment is exceptionally rare because successfully treated teeth seldom require re-entry. A mature mandibular first molar diagnosed with symptomatic irreversible pulpitis associated with symptomatic apical periodontitis underwent a miniature pulpotomy. Because conv

Minimally Invasive Management of Two Separated Dens Invaginatus Oehler's Type I and II: A Case Report.

Treating coronal dens invaginatus (CDI) with pulp infection typically involves the removal of the invaginatus, which increases the risk of fracture and perforation, compromising tooth structure. Minimally invasive endodontic management of coronal dens invaginatus is highly recommended. This case report presents the management of a 19-year-old female patient with a permanent maxillary lateral incisor exhibiting two distinct dens invaginatus anomalies. Cone-beam computed tomography (CBCT) was used to assist in the diagnosis of tooth morphology and canal location. CBCT confirmed that the buccally

[Maxillary sinus lesions and their association with apical lesions observed by cone beam computed tomography. A retrospective cross-sectional study].

INTRODUCTION: Through cone beam computed tomography, alterations in the maxillary sinuses, such as opacities, space occupation and thickening of the mucosa, can be observed. Some factors contribute to this thickening, standing out among dental factors, periodontitis, apical pathology and endodontic treatments. OBJECTIVE: To evaluate the association between changes observed in the maxillary sinuses and apical lesions using cone beam computed tomography. MATERIALS AND METHODS: It was a descriptive study with a retrospective and cross-sectional, correlational, field, non-experimental design. Th

Management of rapidly progressing periopical pathologies: a case report.

Dentists often treat swellings from odontogenic periapical pathologies. One management option involves immediate treatment with antibiotics, followed by surgical intervention. We report a clinical case in which an 8-year-old patient sought care for such a lesion and received pharmacological therapy alone. The lesion expanded into multiple facial spaces approximating the ocular organ and other vital cranial structures. Eventual treatment of the lesion required a CT-scan, followed by an intubated general anesthetic, incision and drainage, extraction of the involved dentition and an overnight hos

Lymphangioma mimicking apical periodontitis.

INTRODUCTION: Lymphangiomas mimicking apical periodontitis, usually found in soft tissue of children and infants younger than 2 years, are extremely rare in aged patients, particularly in bone tissues. METHODS: An 81-year-old woman was seen in a radiological service to undergo imaging studies for the placement of dental implants. A panoramic radiograph showed a radiolucent multilocular structure in the left mandibular molar region, where teeth #17 and #18 had previously been treated endodontically. A fracture in tooth #17 was detected. Because of the patient's clinical history and the charact

Periodontal bone loss analysis via keypoint detection with heuristic post-processing.

OBJECTIVES: This study proposes a deep learning framework and an annotation methodology for the automatic detection of periodontal bone loss landmarks, associated conditions, and staging. Methods192 periapical radiographs were collected and annotated using a stage agnostic methodology, labelling clinically relevant landmarks regardless of disease presence or extent. We propose a heuristic post-processing module that aligns predicted keypoints to tooth boundaries using an auxiliary instance segmentation model. An evaluation metric, Percentage of Relative Correct Keypoints (PRCK), is proposed to

Optimising Outcomes in Endodontic Microsurgery: Evidence, Uncertainties and Future Directions.

Endodontic microsurgery (EMS) is a reliable treatment modality for managing persistent or recurrent periapical pathologies, particularly when conventional nonsurgical retreatment is infeasible or has failed. Various factors influence EMS outcomes, among which the lesion type is a key determinant. Isolated endodontic lesions generally exhibit favourable outcomes, whereas combined endodontic-periodontal lesions and through-and-through defects are clinically challenging and associated with less predictable outcomes. The adjunctive use of regenerative procedures, such as bone grafts, barrier membr
