Advances in materials research related to orbital reconstruction: A review.

Orbital injuries or defects caused by various reasons are quite common, such as violent trauma or tumors. If the damaged orbits are not treated in a timely manner and the normal orbital structure cannot be restored, it may lead to ocular nerve injury, embedding or protrusion of orbital contents, and complications such as enophthalmos, diplopia, and eye movement disorders. Therefore, it is particularly important to repair orbital injuries or defects and reconstruct the normal structure of the orbit. Currently, there are various types of implants applied to reconstruct the orbit, which can be ca

[Reconstruction and Reanimation of the Periorbital Region in Facial Paralysis].

Reconstruction of facial paralysis requires the restoration of both functional and aesthetic aspects while considering a satisfactory quality of life. The periorbital region in particular poses major challenges. The main symptoms of the periorbital region include increased tearing, lagophthalmos, recurrent keratoconjunctivitis, asymmetry and inability to blink spontaneously. Diagnostics as well as therapy must be carried out in an interdisciplinary setting. Conservative treatment is initiated immediately after occurrence of symptoms. The indication for surgical therapy depends on the etiology,

Functional reconstruction of lip defects.

The lip is a crucial structure in the oral and maxillofacial region, serving vital physiological functions such as speech, swallowing, chewing, and expression. Due to the complexity of lip anatomy, function, and the various types of defects, the functional restoration and reconstruction of lip defects remain complex and challenging tasks. In this article, we summarize several methods for functional restoration and reconstruction of lip defects using local flaps that carry the depressor anguli oris muscle, as well as some free flaps. We also introduce methods for repairing extensive defects in

Integrated Short-Term and Long-Term Efficacy of Topical Clascoterone Cream 1% in Patients Aged 12 Years or Older With Acne Vulgaris.

BACKGROUND: Clascoterone cream 1% is approved for the treatment of acne vulgaris in patients aged 12 years or older based on results from two identical pivotal Phase 3 trials. Integrated efficacy of clascoterone in patients aged 12 years or older with acne vulgaris from the pivotal trials (NCT02608450 and NCT02608476) and long-term extension (LTE) study (NCT02682264) is reported. METHODS: In the pivotal trials, patients with moderate-to-severe acne vulgaris were randomized 1:1 to twice-daily application of clascoterone cream 1% or vehicle for 12 weeks; they could then enter the LTE

Midfoot Fusion Using Superconstructs for the Charcot Foot: Current Techniques and Complications.

Neuroarthropathy of the foot and ankle presents a series of challenges. The treating physician faces a perfect storm of pathomechanics, deformity, and medical comorbidities. Successful treatment requires a systematic approach in diagnosis, nonsurgical management, surgical management, and long-term maintenance of the affected extremity. Nonsurgical care of the Charcot foot remains the mainstay of treatment and is successful in most cases. Surgery has become more accepted for patients with severe deformity. The concept of a superconstruct has been introduced to describe modern surgical technique

[Sincipital meningoencephaloceles: Clinical and surgical considerations].

Sincipital meningoencephaloceles (MECs) are rare congenital malformations characterized by the herniation of brain or meningeal tissue through an opening in the anterior floor of the skull base. These malformations always affect the frontal bone, specifically the glabellar region and the naso-frontal angle. A collaboration between Médecins du Monde and the Children's Surgical Center in Phnom Penh has enabled the treatment of over four hundred cases over twenty years. Patients typically have not undergone radiological examinations, with surgical strategies based on clinical and intraoperative o

[Carinal Reconstruction].

Carinoplasty can be divided into the one-stoma method, the montage method, the double-barrel method, and the Miyamoto method. The one-stoma method is usually performed with right upper sleeve lobectomy, and with an anastomosis of the intermediate trunk to a carina. On the other hand, in the montage method, the double-barrel method or the Miyamoto method, carina is completely resected and the trachea, left main bronchus and right bronchus are divided into three pieces. In the montage method, a side hole is created in the trachea, left main bronchus, or right bronchus, and an end-to-side anastom

Evolution and Diversity of Medial Sural Artery Perforator Flap for Hand Reconstruction.

The free medial sural artery perforator (MSAP) flap is a recently popularized flap. It has evolved from a composite myocutaneous flap to a pedicled perforator flap for lower limb reconstruction. It is also a versatile free perforator flap for extremity and head and neck reconstruction. The diversity of the flap designs with options for harvest of non-vascularized grafts enhances the versatility for hand and upper limb reconstruction. The adjunctive use of endoscopy and indocyanine green fluorescence imaging studies can assist and demystify the flap anatomy. The authors present their experience

Volume Redistribution of the Buccal Fat Pad After Reduction Malarplasty.

BACKGROUND: During reduction malarplasty, cheek bulging could be found immediately after zygomatic complex is moved inwards, backwards and upwards. As patient is in the supine position during surgery, the effect of gravity is eliminated, so the only reason for the bulge is the redistribution of the soft tissue in the deep facial spaces. The buccal fat pad, with its main body behind the zygomatic arch and buccal extension in the cheek area, is most likely to be responsible for the bulge. METHODS: 3D buccal extension models were reconstructed from preoperative and long-term follow-up CT images

Therapeutic Options for Migraines in the Microsurgical Patient: A Scoping Review.

BACKGROUND: There exists an increasing array of treatments proposed to prevent, alleviate, and abort symptoms of a migraine; however, for patients who undergo reconstructive microsurgery, caution must be taken to preserve vascular integrity. This study is the first-to-date scoping review of vascular and bleeding risk of current migraine therapies, with the purpose of identifying potential therapeutic agents for postoperative migraine management appropriate for microsurgical patients. METHODS: Currently available migraine therapeutics were compiled from the UpToDate software system and the Ame

Hemi one-piece distraction osteogenesis for unilateral coronal craniosynostosis.

INTRODUCTION: It was the aim of the study to assess the efficacy of the hemi one-piece distraction osteogenesis and to compare it to the traditional one-piece distraction osteogenesis technique. METHODS: Two different surgical techniques were used; the one-piece distraction and the hemi one-piece distraction. The principal distinction between the two techniques is that in the hemi style approach, the intact sutures on the contralateral side were left undisturbed, with no osteotomy performed. RESULTS: The hemi one-piece group had a significantly lower median value of plastic surgery time, tot

Reconstruction of Anophthalmic Sockets With Pedicled Nasoseptal Flap.

PURPOSE: Patients who undergo eye removal often present with orbital soft-tissue insufficiency and contraction of the eye sockets. The most commonly used reconstruction strategy is grafting the orbit with free grafts, which is associated with the drawback of harvesting tissue from an unconnected site. This study describes the use of the vascularized nasoseptal flap in the reconstruction and enlargement of the contracted anophthalmic cavity in patients with severe or recurrent contracted eye sockets and evaluates its efficacy. METHODS: A sphenopalatine-pedicled flap from the nasal septum was h
