The authors present their experience during a successful operation of an inguino-scrotal giant hernia. The enormous hernia had been enlarging for 8 years. First, the abdominal wall was gradually expanded by creating a pneumoperitoneum. Then the abdominal circumference was increased by 16 cm using a polypropylene mesh implanted in a sub-lay position in the midline - thus enabling the repositioning of the exteriorated hernia content into the abdomen.

Early diagnosis and surgical intervention is important in the management of peptic ulcer perforation. This study looks at the value of a pneumogastrogram in the diagnosis of patients suspected of having a perforated peptic ulcer. One hundred and twenty-nine patients were studied. Pneumogastrogram increased the diagnostic yield of pneumoperitoneum from 66% following plain erect X-ray examination to 91%. The procedure was free of complications.

Cardiorespiratory and blood gas alterations were evaluated in 6 healthy dogs that underwent a laparoscopic procedure using isoflurane anesthesia and carbon dioxide (CO(2)) pneumoperitoneum for 30 min. Heart rate, respiratory rate, body temperature, venous blood pH, partial pressure of CO(2) and oxygen, oxygen saturation, total carbon dioxide (TCO(2)) and bicarbonate were monitored. Significant alterations were hypercapnia, hypoventilation, and respiratory acidosis.

The authors report a case of 32-week-old newborn, admitted to the Intensive Care Unit for Newborns. Mechanical ventilation was needed from the first day of life. The patient was treated with indomethacin on the second day of life due to patent ductus arteriosus. On the seventh day pneumoperitoneum was diagnosed, emergency surgery was performed revealing perforation of Meckel's diverticulum. Perinatal asphyxia and indomethacin administration probably played an important role in this process.
