Spaceship Earth Revisited: The Co-Benefits of Overcoming Biological Extinction of Experience at the Level of Person, Place and Planet.

Extensive research underscores that we interpret the world through metaphors; moreover, common metaphors are a useful means to enhance the pursuit of personal and collective goals. In the context of planetary health-defined as the interdependent vitality of all natural and anthropogenic ecosystems (social, political and otherwise)-one enduring metaphor can be found in the concept of "Spaceship Earth". Although not without criticism, the term "Spaceship Earth" has been useful to highlight both resource limitations and the beauty and fragility of delicate ecosystems that sustain life. Rene Dubos

Brucellosis in the Middle East: Current situation and a pathway forward.

Brucellosis is a bacterial endemic zoonotic disease of global significance with detrimental impacts on public health and food animal production. It is caused by Brucella spp., an expanding group of pathogens able to infect various host species. Bovines and small ruminants, which excrete the bacteria in milk and in reproductive discharges, are major sources of infection for humans and other animals. Contact with contaminated animals and consumption of unpasteurized dairy products are the main routes for human infection. In spite of the considerable progress of knowledge gained and success achie

The Dallol Geothermal Area, Northern Afar (Ethiopia)-An Exceptional Planetary Field Analog on Earth.

The Dallol volcano and its associated hydrothermal field are located in a remote area of the northern Danakil Depression in Ethiopia, a region only recently appraised after decades of inaccessibility due to severe political instability and the absence of infrastructure. The region is notable for hosting environments at the very edge of natural physical-chemical extremities. It is surrounded by a wide, hyperarid salt plain and is one of the hottest (average annual temperatureDallol: 36-38°C) and most acidic natural systems (pHDallol ≈0) on Earth. Spectacular geomorphologies and mineral deposits

Choose and Book: a sociological analysis of 'resistance' to an expert system.

In 2004, the English Department of Health introduced a technology (Choose and Book) designed to help general practitioners and patients book hospital outpatient appointments. It was anticipated that remote booking would become standard practice once technical challenges were overcome. But despite political pressure and financial incentives, Choose and Book remained unpopular and was generally used reluctantly if at all. Policymakers framed this as a problem of 'clinician resistance'. We considered Choose and Book from a sociological perspective. Our dataset, drawn from a qualitative study of c

Beyond 'vulnerable groups': contexts and dynamics of vulnerability.

This paper reviews approaches to vulnerability in public health, introducing a series of 10 papers addressing vulnerability in health in Africa. We understand vulnerability as simultaneously a condition and a process. Social inequalities are manifest in and exacerbate three key dimensions of vulnerability: the initial level of wellbeing, the degree of exposure to risk, and the capacity to manage risk effectively. We stress the dynamic interactions linking material and social deprivation, poverty, powerlessness and ill health: risks or shocks and their health impacts are intimately interconnect

Enabling implementation of the Global Vaccine Action Plan: developing investment cases to achieve targets for measles and rubella prevention.

Global prevention and control of infectious diseases requires significant investment of financial and human resources and well-functioning leadership and management structures. The reality of competing demands for limited resources leads to trade-offs and questions about the relative value of specific investments. Developing investment cases can help to provide stakeholders with information about the benefits, costs, and risks associated with available options, including examination of social, political, governance, and ethical issues. We describe the process of developing investment cases for

Unpacking the concept of patient satisfaction: a feminist analysis.

AIM: The aim of this paper is to present a feminist critique of the concept of patient satisfaction. BACKGROUND: Fiscal restraint, health care restructuring, shifting demographics, biomedical technological advances, and a significant shortage of health care professionals are stretching health care systems across North America to the breaking point. A simultaneous focus on consumerism and health service accountability is placing additional pressure on the system. The concept of patient satisfaction, with roots in the consumer movement of the 1960s, has both practical and political relevance in

Protectionist politics: A postcolonial analysis of the discourse introducing the UK's medical training prioritisation bill.

INTRODUCTION: Rising nationalist sentiment and backlash against equity, diversity and inclusion are reshaping Western political discourse, increasingly framing international medical graduates (IMGs) as threats despite health systems' reliance on them. In January 2026, the UK Parliament introduced the Medical Training (Prioritisation) Bill, prioritising UK graduates over IMGs-a stark reversal of the UK's previous equity commitments, with implications beyond the UK. Using postcolonial theory, we conducted a Critical Discourse Analysis of media, documents and parliamentary transcripts to examine

The State of Transfusion Medicine in Low- and Middle-income Countries: A Critical Narrative Review.

An annual global shortfall of over 100 million blood units is borne almost entirely by low- and middle-income countries (LMICs), with measurable consequences for patient survival. Despite sustained advocacy, the evidence on transfusion medicine across these settings has not been systematically and critically reviewed. A narrative review was conducted across 9 electronic databases, applying strict LMIC-specific, date-bound eligibility criteria. Studies conducted in LMIC settings addressing any aspect of transfusion medicine and published within the preceding 15 years were eligible. Seven studie

Epidemiological Challenges and Health Disparities in the Eastern Mediterranean Region: A Region in Crisis.

Background: The World Health Organization's (WHO) Eastern Mediterranean Region (EMR) is marked by socioeconomic diversity, political instability, displacement, and environmental risks. With nearly 745 million people across 22 countries, the region faces a complex "triple burden" of non-communicable diseases (NCDs), communicable diseases (CDs), and injuries. Objective: This paper provides an updated overview of the epidemiological landscape of the EMR by linking current disease burdens with socioeconomic, behavioural, environmental, and health system determinants. It highlights regional dispari

Embodied harm and health consequences: self-reported physiological symptoms among co-victims of cold-case homicides.

BACKGROUND: Families of victims of unresolved ("cold case") homicides endure not only prolonged emotional devastation but also chronic exposure to stressors linked to institutional inaction and uncertainty. While previous studies have addressed trauma, grief, and mental health consequences among co-victims of homicide, minimal scholarly attention has been devoted to the physical and physiological impact of unresolved loss, particularly when justice remains indefinitely suspended. METHODS: This qualitative study draws on in-depth semi-structured interviews with 13 co-victims of cold-case homic

Formal and informal science-policy interaction during public health emergencies of international concern: a scoping review.

BACKGROUND: Interaction between science and policy might be a crucial asset for policy decisions during public health emergencies. Much work has been published about processes and forms of science-policy interaction on the one hand and challenges of public health emergency contexts on the other. However, more knowledge is needed to understand the challenges that arise when these two issues intersect. METHODS: We conducted a scoping review informed by the Johanna-Briggs-Institute protocol and searched seven databases to identify formal and informal approaches to organising science-policy inter

ERA's ABCDE framework for kidney disease prevention: turning the WHO kidney health resolution into action.

In 2025, the World Health Assembly of the World Health Organization (WHO) adopted a resolution on reducing the burden of noncommunicable diseases (NCDs) by promoting kidney health and strengthening the prevention and control of kidney disease. Following the WHO resolution, the United Nations (UN) included kidney health in its 2025 Political Declaration on NCDs. These measures are a clear response to the growing burden of kidney diseases. This achievement for kidney health was facilitated by years of effort by multiple stakeholders and decision-makers, including nephrology associations, particu

Hydrological asymmetry and water stress in Peru: An integrated assessment of resource distribution, anthropogenic pressure, and governance gaps across three drainage basins.

BACKGROUND: Peru presents a paradigmatic case of hydrological asymmetry, where national water abundance (56,887 m 3/capita/year) masks acute regional scarcity. The Pacific basin, hosting 66.4% of the population, possesses only 1.9% of renewable water resources (1,628 m 3/capita/year), falling below the Falkenmark water stress threshold (1,700 m 3/capita/year). Conversely, the Amazon basin holds 97.8% of resources for 30.4% of the population (183,142 m 3/capita/year). This 112.5-fold asymmetry ratio exceeds comparable international cases (Egypt: 49.2; China: 8.0), yet Peru lacks systemic redist

HIV-relevant policies in European countries: A comprehensive landscape of policies hindering or facilitating access to HIV prevention, testing, treatment, and care of key populations.

BACKGROUND: In Europe, there are variations in epidemic patterns, trends, and progress toward achieving the UNAIDS target of ending the AIDS epidemic. Complex social, economic, cultural, and behavioral dynamics make key population groups more vulnerable to HIV, and health and health-related policies play a significant role in ensuring progress towards eliminating HIV. This study aims to characterize and compare how HIV-relevant policies may directly or indirectly hinder or facilitate HIV prevention, testing, treatment, and care of key populations within selected European countries. METHODS: T

The Lenacapavir effect: Triumphalist rhetoric and the displacement of social and structural responses to HIV prevention.

Lenacapavir, a long-acting injectable pre-exposure prophylaxis (PrEP) medication for HIV prevention with proven high efficacy in randomised controlled trials, has been widely positioned as a "game-changing" technology capable of ending the AIDS epidemic. Despite the withdrawal of major donor funding that necessitates a critical reappraisal of HIV prevention priorities, lenacapavir has been rapidly embedded within global policy discourse through claims of exceptional efficacy, anticipated cost-efficiency, and potential epidemiological impact. These narratives reflect a long-standing pattern of

Factors contributing to the dropout of undergraduate nursing curricula according to the year of attendance: A mixed-methods systematic review.

BACKGROUND: The global nursing shortage poses critical challenges for healthcare systems. Understanding nursing student dropout, particularly which factors contribute to this process and when they emerge during the curriculum, is crucial for developing timely and effective retention strategies that support workforce sustainability. Despite growing concern, no systematic reviews have examined the reasons for dropout through a time-sensitive approach. AIM: To understand the factors contributing to dropout from undergraduate nursing curricula with specific attention to the year of attendance. D
