Complication risks and costs associated with Ponte osteotomies in surgical treatment of adolescent idiopathic scoliosis: insights from a national database.

PURPOSE: Risks of Ponte osteotomies (POs) used for posterior spinal fusion (PSF) for Adolescent Idiopathic Scoliosis (AIS) are challenging to assess because of the rarity of complications. Using a national administrative claims database, we evaluated trends, costs and complications associated with PO used in PSF for AIS patients. METHODS: Using ICD-9/CPT codes, we identified patients (ages 10-18) with AIS who underwent PSF (± PO) between 2007 and 2015 in the IBM® MarketScan® Commercial Databases. Costs and trends of POs were evaluated. Odds of neurological complications and readmissions withi

Abnormal Tuning in Nucleus Prepositus Hypoglossi of Monkeys With "A" Pattern Exotropia.

IMPORTANCE: Recent European guidance supports a diminished role for exercise electrocardiography (ECG) in the assessment of suspected stable angina. OBJECTIVE: To evaluate the utility of exercise ECG in contemporary practice and assess the value of combined functional and anatomical testing. DESIGN, SETTING, AND PARTICIPANTS: This is a post hoc analysis of the Scottish Computed Tomography of the Heart (SCOT-HEART) open-label randomized clinical trial, conducted in 12 cardiology chest pain clinics across Scotland for patients with suspected angina secondary to coronary heart disease. Between

An atypical presentation of diffuse midline pontine glioma in a middle age patient: Case report.

INTRODUCTION: Diffuse midline glioma is a newly WHO defined entity (grade IV) (Louis et al., 2016) which includes diffuse intrinsic pontine glioma (DIPG) reported in pediatric population and, occasionally, in young adults. Here, we present a detailed description of an atypical case of diffuse midline glioma in a 53 years old woman. CASE REPORT: A caucasian woman aged 53 from Ukraine, was referred to another neurological department complaining of 3 months history of progressive postural instability and gait impairment with frequent falling. Magnetic resonance demonstrated two brainstem lesions

Massive Spontaneous Symptomatic Hemorrhagic Transformation Following Pontine Infarction - A Case Report.

PURPOSE: Spontaneous hemorrhagic transformation is common in anterior circulation infarction, but is rarely seen in brainstem infarction. We report a case of massive symptomatic brainstem hemorrhagic transformation in a patient with pontine infarction treated with only antiplatelet agents. CASE REPORT: A 59-year-old man presented with acute dysarthria and right sided weakness. His Glasgow coma scale was E3 V5 M6. His pupils were pinpoint in size and minimally reactive to light. He showed complete horizontal gaze palsy, right facial weakness, severe dysarthria, dysphagia, and right hemiplegia.

Effects of pontine lesions on REM sleep.

Over the past several decades, the neural mechanisms underlying REM sleep have become increasingly understood. A more detailed understanding of the respective roles of the pontine nuclei in the generation of REM sleep and its related phenomenon has allowed for the recognition of specific effects that brainstem lesions have on sleep. In humans, however, the effects of such lesions are limited to case reports and small case series. This article offers a comprehensive review of the basic neurobiology of REM sleep. In addition, we discuss specific clinical effects that various pontine lesions have

Pontocerebellar angle aspergillosis: clinical and radiological findings.

INTRODUCTION: Cerebral aspergillosis is a rare and severe condition mostly affecting immunocompromised patients. The lesions are usually intra-axial and supratentorial; several radiologic patterns have been reported. CASE REPORT: A 65-year-old patient with chronic lymphocytic leukemia presented with fever, headache, and a pontocerebellar syndrome. A brain magnetic resonance imaging (MRI) showed a ring-enhancing left pontocerebellar mass consistent with an infectious disease. Despite broad-spectrum antibiotic therapy, the patient worsened. A follow-up MRI examination disclosed a concomitant ac

[A case of pontine infarction presenting with conjugate deviation and unilateral conjugate gaze palsy].

We reported a case of pontine infarction presenting with conjugate deviation and unilateral conjugate gaze palsy. A 75-year-old man was admitted to our hospital because of vomiting. On the day of admission, his neurological findings showed only conjugate deviation to the right without consciousness disturbance, hemispatial neglect, and hemiparesis. On day 5 after admission, the conjugate deviation to the right disappeared, but he could not gaze left with either of his eyes. The pathological lesions, therefore, were considered to be a unilateral paramedian pontine reticular formation (PPRF) and

Ataxia after pontine stroke: insights from pontocerebellar fibers in monkey.

Basis pontis lacunes cause contralateral but rarely ipsilateral ataxia. We explored this phenomenon with isotope tract tracing in the rhesus monkey. Labeled pontocerebellar fibers cross midline and disperse widely in the opposite hemipons before coalescing in the brachium pontis. This anatomical arrangement suggests that small pontine strokes spare sufficient decussating pontocerebellar fibers to prevent ipsilateral dysmetria, and that ipsilateral dysmetria after large pontine stroke represents a disconnection syndrome.

[Central pontine myelinolysis in an eighteen year old woman].

Central pontine myelinolysis is sporadically diagnosed disease. The disease has fulminant course and fatal prognosis. Myelinolysis is known for 40 years, but its cause remain still unclear and therapy is not established. A case of 18 year old woman hospitalized in the Department of Infectious Disease Medical Academy of Bialystoku is an example of such a difficulties.

Ponto-cerebellar hypoplasia with dystonia: clinico-pathological findings in a sporadic case.

Microcephaly, absent psychomotor development and dystonic limb movements were the main clinical features of a 3-year-old girl affected by hypoplasia of the pontocerebellar structures. As in the few previously reported cases there are discrepancies between the severity of lesions in the supratentorial and infratentorial compartments. Pathological features such as size reduction of the ventral pons, inferior olive atrophy, dentate nucleus fragmentation, and thinning of the cerebellar cortex suggest an impaired maturation of the involved structures due to a prenatal condition (dated at about 20-2…

[Risk of central pontine myelinolysis in the treatment of severe hyponatremia].

Central pontine myelinolysis is a life-threatening condition involving the demyelination of axons in certain areas of the brain. It has been shown almost invariably to occur in connection with hospital care. In recent years, a connection has been noted between the rapid restitution of low serum sodium and the development of the condition. In this review, the most recent scientific information is summarized. It is concluded that the risk should always be considered in treating a hyponatremic patient. The serum sodium level should be raised slowly and the acute treatment ended before normal seru

[Central pontine and extrapontine myelinolysis: report of an autopsied case and review of the literature].

An autopsied case of central pontine and extrapontine myelinolysis in a 16-year-old diabetic girl is described. Due to dehydration she was treated vigorously with daily intravenous isotonic saline, from the first day of hospitalization. In the first three days the serum sodium level increased by more than 30 mEq/l when compared with the initial level. By the next days the serum sodium level, after a mild drop, rose again and was maintained above normal range for a further 12 days. On the sixth day of this new and sustained serum sodium increase, the patient presented progressive neurological m
