The knowledge about the influence of chloroquine phosphate (CQ) on the iron metabolism in porphyria cutanea tarda (PCT) is still insufficient. In a study on 138 PCT patients treated with CQ, we observed decreasing serum iron concentration, transferrin saturation, and hepatic siderosis, as well as an increasing level of transferrin. After a one-year therapy with CQ, the patients showed normal porphyrinuria. Since there were no statistical correlations between the degree of hepatic siderosis and porphyrinuria, and since we repeatedly found remission of the PCT in spite of the continued existence

The course of arthritis with eosinophilic fasciitis is poorly documented. We report a patient in whom polyarthritis preceded the clinical appearance of eosinophilic fasciitis and progressed over a 14-year period to a destructive process with inflammatory and degenerative characteristics. Porphyria cutanea tarda and transient extramedullary pancytopenia complicated this patient's disease.

We report a case of familial porphyria cutanea tarda (FPCT) in a 7-year-old girl. The condition was exacerbated by low dose of hydroxychloroquin, but controlled by venesection. The favorable prognosis of FPCT should be further established by long-term prospective studies.

Two elderly patients with sporadic porphyria cutanea tarda (PCT) and severe associated diseases were treated with high weekly intravenous doses of desferrioxamine. This therapy proved more effective than slow subcutaneous desferrioxamine infusion and no adverse effect was recorded. We consider high dose intravenous desferrioxamine a suitable treatment for those PCT patients with severe associated diseases who are unable to apply the technique of subcutaneous desferrioxamine infusion.

We have measured C3, C4, CH50 and complement cleavage products C3a and C5a in in sera and plasma from PCT patients and normal controls 10 min and 1, 4 and 24 h after UVA irradiation. We found elevated C3a concentrations in PCT patients immediately after UVA irradiation and 24 h later. The same was true for CH50, whereas C3, C4 and C5a did not change significantly. No such changes occurred in normal controls. Our data suggest that activation of the complement cleavage product C3a by porphyrin and UV light triggers a series of events that cause tissue damage.

We investigated the effect of UV light (320-460 nm) on total hemolytic CH50 activity and C3 cleavage in sera obtained from 14 patients with porphyria cutanea tarda. Irradiation with 5, 10, or 50 J/cm2 resulted in a 12%-60% loss of CH50 and a 5%-30% cleavage of native C3 as estimated by planimetric evaluation of the immunoelectrophoretic C3 pattern. In 74 patients with porphyria cutanea tarda, 11 (14.9%) cases of beta-thalassaemia were found. The incidence of beta-thalassaemia in porphyrics is not greater than in non-porphyrics and it seems that beta-thalassaemia is not a factor triggering the development of porphyria cutanea tarda. The phlebotomy treatment is also a convenient method for porphyria cutanea tarda combined with beta-thalassaemia.

The increased incidence of cases with porphyria cutanea tarda (p.c.T.) in women is correlated to preceding hormonal contraception. This is our reason to report on 16 females suffering from p.c.t. in the last years (1975 to 1979), including 8 females younger than 40 years affected after use of oral contraceptives for a long time. Stopping the hormonal anticonception represents the most important therapeutic measure. This is a good supposition to prevent recidives and to cure definitely. Cure is demonstrated in a young woman by normal pregnancy with a healthy boy.

Male and female F 344 rats were dosed every other day for 103 days with 50 mumole of hexachlorobenzene (HCB)/kg. Females developed a hepatic porphyria, the urine and liver levels of porphyrins being 40- and 310-fold higher respectively than those of males. Urine was periodically hydrolysed and analysed for the three metabolites pentachlorophenol, 2,3,5,6-tetrachlorobenzene-1,4-diol and pentachlorothiophenol (derived from the mercapturate). The acute porphyric crisis is a characteristic clinical feature common to all hereditary hepatic porphyrias (acute intermittent porphyria [AIP], porphyria variegata and hereditary coproporphyria). In thirty-four patients with porphyria cutanea tarda treated with small doses of chloroquine, liver biopsies were performed before and after treatment. In seventeen cases (50%) the morphological patterns before treatment corresponded to unstabilized fibrosis, while in eleven (32.4%) there were non-specific changes in the form of focal fatty change, haemosiderosis, and mild fibrosis of the portal tracts.
