HIV infection among Brazilian transvestites in a prison population.

Eighty-two male transvestites imprisoned in Casa de Detenção (São Paulo, Brazil) were tested for HIV antibodies, and completed a questionnaire investigating their demographics, arrest and imprisonment records, sexual practices, and drug use. Data were then analyzed to evaluate the incidence of HIV infection and its association with various behavioral and other factors. Sixty-four of 82 (78%, 95% confidence interval [CI], 67-87%) transvestites were positive for HIV infection. The factors associated with significant differences in positivity among these individuals were the time spent in prison

Combat stress reactions in Iraqi enemy prisoners of war.

Combat stress reactions (CSR) are a source of significant morbidity on the battlefield, and they initiate the cascade of psychophysiological dysfunction that results in the posttraumatic stress disorders. During Operation Desert Storm in early 1991, very few allied CSR casualties were observed. However, Iraqi soldiers experienced intense conditions likely to result in CSR. Using semistructured interviews, the authors examined Iraqi enemy prisoners of war for the presence of CSR and compared their responses to two groups of U.S. soldiers. Iraqi prisoners of war experienced many CSR symptoms tha

Accidental and unspecified deaths among prisoners as disguised suicides.

Analysis of 1987 data from the US Department of Justice for prisoners under the jurisdiction of state and federal correctional authorities indicated that suicide rates were not negatively correlated with accidental or unspecified death rates. These findings do not provide evidence of disguised suicides among prisoners. Possible explanations for high rates of unspecified death among prisoners are presented.

Parents in Prison: a child abuse and neglect prevention strategy.

Parents in Prison is an innovative family support service housed at the Tennessee State Prison for Men. Developed in response to inmates' recognition of the need for child abuse and neglect prevention services relevant to men who are in prison, the program model has potential for becoming a major service delivery strategy. Leadership for Parents in Prison is provided by an inmate committee which receives consultation and assistance from a community advisory board and an institutional sponsor. Program components--correspondence and classroom courses, monthly events featuring community guest spe

Absence of drug abuse in prisoners in a major correctional institution.

Laboratory evaluation of illicit drug abuse was conducted in a major correctional institution. Fifty-eight prison inmates were screened for common drugs of abuse in conjunction with a triazolo-benzodiazipine compound tolerance protocol. Urine specimens were collected on days -1, 1, 15, 29, and 49 and assayed for amphetamines, opiates, barbiturates, and alcohol. Quinine was detected in two individuals and was probably derived from cold remedy preparations rather than the illegal use of heroin. While occasional illicit drugs may find their way into a prison population, this study showed no evide

"Looking back to look forward": evaluation of a leadership program for prison staff designed to promote public health-focused culture change in US prisons.

PURPOSE: Prisons are associated with poor health outcomes for incarcerated people and correctional staff. Efforts to remedy harmful prison conditions typically focus on litigation, legislation and administrative policy changes; however, implementing these top-down mandates is often impeded by cultural barriers among prison staff. The purpose of this study was to evaluate a novel intervention grounded in public health and international correctional principles, to educate and motivate frontline prison staff to lead culture change initiatives in US prisons. DESIGN/METHODOLOGY/APPROACH: Guided by…

Chronic hepatitis C elimination prison initiative: HCV-intensive test and treat, a whole prisoner population HCV test-and-treat program in England.

BACKGROUND AND AIM: Prison residents are at high risk for hepatitis C virus (HCV) infection. HCV test-and-treat initiatives within prisons provide an opportunity to engage with prison residents and achieve HCV micro-elimination. The aim of the prison HCV-intensive test and treat initiative was to screen over 95% of all prison residents for HCV infection within a defined number of days determined by the size of the prison population and to initiate treatment within 7-14 days of a positive HCV RNA diagnosis. METHODS: An HCV-intensive test and treat toolkit was developed based on learnings from …

Interferon-gamma release assay for the diagnosis of latent tuberculosis infection in the prison population with a positive tuberculin test: a descriptive study in a prison (Burgos, Spain).

OBJECTIVES: A high prevalence of prison inmates have a positive tuberculin skin test (TST) and sometimes unnecessary treatment for latent tuberculosis infection (LTBI) is prescribed. The prison tuberculosis prevention and control program has not generalized the use of QuantiFERON (QFT) in prisons. We set out to describe the implementation and usefulness of QFT in a population of inmates with positive TST, and to detect false positives and avoid unnecessary treatments. We also analysed the sociodemographic variables of the inmate population. MATERIAL AND METHODS: All the positive TST tests bet…

Non-cancer pain clinic in three English prisons: understanding prisoner needs and prescribing practice in relation to chronic non-cancer pain (CNCP).

PURPOSE: Prisoner access to opiate-based analgesics and gabapentinoids is a concern to prisons through illicit trading. The purpose of this paper is to describe patient needs following introduction of nine pilot chronic non-cancer pain (CNCP) clinics for chronic pain in three prisons (two male and one female) in the South of England. The study evaluated the effectiveness of this model and assessed the wider practical implementation issues. DESIGN/METHODOLOGY/APPROACH: Clinical notes were reviewed for 63 consultations, anonymised and recorded for secondary analysis. FINDINGS: Alongside CNCP, …

A comparison of liver disease mortality with HIV and overdose mortality among Georgia prisoners and releasees: a 2-decade cohort study of prisoners incarcerated in 1991.

OBJECTIVES: We investigated whether eventual causes of death among a cohort of inmates imprisoned in the southeastern United States differed from those in previous prisoner studies. METHODS: We matched 23 510 prisoners in Georgia, a state with historically low levels of heroin consumption but moderate amounts of injection drug use, who were incarcerated on June 30, 1991, to death registries through 2010. Main exposure was 4-year time intervals over 2 decades of observation; main outcome was mortality from liver disease, HIV, and overdose. RESULTS: Although the HIV-related mortality rate exce

Transgender and Gender Diverse Identity, Mental Health Conditions, and Mental Health Care in U.S. Prisons.

Prisons, as total institutions organized around the sex/gender binary, are highly gendered institutions. Individual experiences of prison, therefore, vary dramatically by gender identity due to pains of imprisonment experienced differently by people experiencing incarceration. Building on the gendered pains of imprisonment literature, we use the public-use 2016 Survey of Prison Inmates to expand on previous research exploring the disproportionate mental health burden experienced by transgender and gender diverse (TGD) individuals who are incarcerated relative to cisgender men and women who are…

Serologic Immunity and Vaccination for Hepatitis A Among Incarcerated Adults in California State Prisons, 2024.

BACKGROUND: Hepatitis A is a vaccine-preventable infection which may lead to serious consequences, and which may spread among people experiencing homelessness or substance use disorder. Because these factors are highly prevalent in incarcerated populations, identifying susceptible groups and establishing high levels of immunity are crucial to reducing disease transmission and morbidity. AIM: To describe hepatitis A immunity and vaccination status in the California prison population and to identify predictors of susceptibility. METHODS: This cross-sectional study used secondary data and emplo

Identifying barriers and facilitators to accessing harm reduction services in prisons: A systematic narrative synthesis.

BACKGROUND: Drug use is prevalent in carceral settings globally. Although a comprehensive harm reduction package for people in prison (PIP) is recommended by international agencies, its implementation is limited. The aim of this systematic narrative synthesis was to explore barriers and facilitators to accessing harm reduction services (HRS) in prisons. METHODS: We searched MEDLINE, PsycINFO, SCOPUS, and CINAHL for English and French language articles published before June 26, 2024. Studies evaluating any HRS defined as condoms, pre-exposure prophylaxis, post-exposure prophylaxis, opioid agon…
