Peripheral Prosthetic Vascular Graft Infection: A 5-Year Retrospective Study.

Background/Objectives: Peripheral prosthetic vascular graft infection represents a very serious complication after lower limb revascularization, with amputation and mortality rates up to 70% and 30%, respectively. This study was designed to determine the incidence of prosthetic graft infection, amputation, and mortality rate in our institution, analyzing different types of treatment. Methods: A retrospective cohort single institution review of peripheral prosthetic bypass grafts evaluated patient demographics, comorbidities, indications, location of bypass, type of prosthetic material, and cas

Literature review on management of prosthetic graft infections after supra-aortic bypass surgery, based on a case report.

BACKGROUND: Graft infections of supra-aortic bypasses are rare, but often life-threatening. Guidelines for treatment of graft infections recommend systemic antibiotics and complete graft removal, followed by in-situ or extra-anatomic revascularisation. METHODS: Based on case report, literature search was performed on Medline, Google Scholar and Web of Science databases, focussing on infected supra-aortic bypass reconstructions. Inclusion criterium was prosthetic graft infection, excluding stent and carotid patch infections. RESULTS: A 68-year old man with clinically infected subclavian-subcl

Fresh arterial allograft as a replacement for an infected common femoral prosthetic graft and recurrent false aneurysm.

Replacing an infected vascular prosthetic conduit with an allograft is a possible solution of this complication given the low recurrence of infection. It is most commonly utilized for cases where the use of autologous tissue is not an option. We present the case of a 70-year-old patient who had undergone repeated vascular reconstructions in the right lower limb. He was admitted to our department due to a progressively growing mass in the right groin and subsequently placed on the waiting list for a fresh allograft. The patient had the infected false aneurysm and prosthetic material of the femo

Dissection of intima on atrial septum patch after mitral valve replacement in a patient with infective endocarditis after incomplete atrio-ventricular septal defect repair: report of a case.

We encountered a very rare lesion, which included a dissection between a prosthetic patch for repair of an incomplete atrio-ventricular septal defect (AVSD) and a pseudo-intima which had developed. This is the first report of a dissection between an atrial septum patch and pseudo-intima after mitral valve replacement (MVR) in a patient with infective endocarditis following an incomplete AVSD repair. Dissection of the left atrial wall or atrial septum after MVR is extremely rare although some case reports have been published.

Infective endocarditis in the elderly: diagnosis and management.

Infective endocarditis is seen with increasing frequency in older patients. This increase is due to the general aging of the population, improved survival of patients with congenital and valvular heart disease, and the increasing use of catheters and other prosthetic devices with resulting higher incidence of nosocomial endocarditis. In older patients, infective endocarditis frequently develops in the absence of underlying structural heart disease; atheromatous deposits and mitral annular calcification are two important risk factors in this population. Infective organisms in older patients are

The Souter-Strathclyde elbow arthroplasty. A clinical and radiological study of 525 consecutive cases.

We present the results of 525 primary Souter elbow arthroplasties undertaken in 406 patients between 1982 and 1997. There were 372 women and 34 men with a mean age of 57 years; 119 patients had a bilateral procedure. The elbows were affected by chronic inflammatory disease, usually rheumatoid arthritis, which had been present for a mean of 24.7 years (2 to 70). In about 30% the joints were grossly destroyed with significant loss of bone. In 179 elbows the ulnar components were metal-backed and retentive; in the remaining 346, with better bone stock, non-retentive, all-polyethylene prostheses w

Problems, complications, and reinterventions in 4893 onlay humeral lateralized reverse shoulder arthroplasties: a systematic review (part I-complications).

BACKGROUND: Several modifications to the original Grammont reverse shoulder arthroplasty (RSA) design have been proposed to prevent distinctive issues, such as both glenoid and humeral lateralization. The aim of this systematic review was to determine rates of problems, complications, reoperations, and revisions after onlay lateralized humeral stem RSA, hypothesizing that these are design related. METHODS: This systematic review was performed in accordance with the PRISMA statement guidelines. A literature search was conducted (01.01.2000-14.04.2020) using PubMed, Cochrane Reviews, Scopus, an

Moraxella catarrhalis bacteraemia and prosthetic valve endocarditis.

Moraxella catarrhalis, once considered a non-pathogenic coloniser of the oropharynx, has now been recognised as a true pathogen and is reported in cases of bacteraemia. A 63-year-old man with an aortic bioprosthetic valve was brought to the emergency room with altered mental status. Initial blood cultures revealed Gram-negative diplococci on Gram stain; echocardiogram showed a 5 mm vegetation on the aortic bioprosthetic valve. The blood cultures grew M. catarrhalis and the patient was treated medically for prosthetic valve endocarditis with 6 weeks of ceftriaxone and had a favourable clinical

Pasteurella multocida infected total knee arthroplasty: a case report and review of the literature.

Pasteurella multocida is a rare cause of prosthetic joint infection. This infection generally follows significant animal contact, usually licks and scratches. We report a case of P multocida infection that was treated with linezolid with salvage of the implant. Linezolid is generally active against Gram-positive organisms only with the exception of Pasteurella, which is Gram-negative. We extensively review the previous reported cases of implant infection with P multocida.

Late hematogenous infection of first metatarsophalangeal joint replacement: a case presentation.

Late hematogenous infection of previously asymptomatic orthopedic implants is extremely rare and usually associated with total joint replacements, such as those of the hip or knee. We present the case of an otherwise healthy female who developed a deep space infection 18 months after a first metatarsophalangeal joint implant arthroplasty. The patient presented with pain and swelling at the site, and over the course of several days developed fever and tachycardia and leukocytosis. Cultures of the surrounding soft tissues and the implant grew Streptococcus pneumoniae. The patient reported a 1- t

Acute dehiscence of a valve prosthesis 5 years after implantation.

Propionibacterium acnes, a common human skin organism [Perry A.L., Lambert P.A., Under the microscope Propionibacterium acnes, Lett App Microbiol 2006; 42:185-186], mostly considered a contaminant, has rarely been associated with cases of infectious endocarditis [Vanagt W.Y., Daenen W.J., Delhaas T., Propionibacterium acnes endocarditis on an annuloplasty ring in an adolescent boy, Heart 2004; 90:56]. We report on a 48-year old man with a history of mitral valve replacement who acutely developed dyspnoea, tachypnoea and forward failure. Transesophageal echocardiography showed a dehiscence of t

Thrombosis of a mitral valve prosthesis resulting from Staphylococcus epidermidis endocarditis.

A 70-year-old man with a Duromedics mitral valve prosthesis had two episodes of infective endocarditis caused by enterococcus (1994 and 1996). Colonoscopy revealed five polyps. Surgical resection was performed and 2 days later the patient had dyspnea and fever. Because of a suspected valve thrombosis, intravenous heparin was given which resulted in hematic effusion in the Douglas' cul-de-sac. Intravenous heparin was withdrawn but the patient continued to have a worsening dyspnea, hyperthermia and hypotension. The patient was transferred to our Institution in cardiogenic shock. Acute thrombosis

Clinical issues in the prevention of dental-induced endocarditis and prosthetic joint infection.

No issue in medicine or dentistry is the subject of more misunderstanding than the prevention of infective endocarditis and late prosthetic joint infections. To resolve some of the confusion, national medical groups have published guidelines for the prevention of these infections, including the use of antibiotic prophylaxis for certain dental procedures and certain at-risk patients. While these guidelines are helpful, there still are clinicians who either do not recommend antibiotics when indicated or recommend antibiotics without indication. An understanding of these issues helps to prevent n

Surgical management of infected PTFE hemodialysis grafts: analysis of a 15-year experience.

The records of 52 consecutive patients who underwent surgical treatment for 57 episodes of hemodialysis graft infection (HGI) from 1977 to 1993 were reviewed to determine the mortality and morbidity associated with this complication and to clarify guidelines for its management. The study group consisted of 35 women and 17 men whose mean age was 57 years at initial graft placement. Thirty-three (58%) HGIs involved straight grafts in the upper arm, 12 (21%) straight forearm grafts, 11 (19%) loop forearm grafts, and 1 (2%) a loop groin fistula. All of these grafts were constructed with polytetraf

[Bacteremia and localized suppurative Salmonella non-typhi infections in the elderly: a study of 4 cases observed at the Ivrea-Castellamonte Hospital].

Over a period of 60 months 137 cases of intestinal and extra-intestinal salmonellosis were registered in the microbiology department of the Ivrea-Castellamonte hospital. The authors conducted a retrospective study relative to the extra-intestinal salmonellosis manifested in elderly patients. In order to carry out such a study the authors considered only patients who were in the hospital, who were over the age of 65, and who had a clinical history different from the usual one of gastroenterocolitis. Out of 61 patients who were in hospital 50 were with complete registration data; 9 (18%) were ov

Aortic valve replacement with the Toronto SPV bioprosthesis.

The Toronto SPV is a Dacron covered stentless porcine aortic prosthesis used for aortic valve replacement (AVR). We implanted this valve in 53 patients with a mean age of 58 years. The predominant valve lesion was aortic stenosis in 39 patients, insufficiency in 13 and a failed bioprosthesis in one. Six patients also had mitral valve disease and 18 had coronary artery disease. There was one operative death. Patients have been followed for four to 58 months, mean 27 months. There have been two late deaths but neither one was valve related. The actuarial survival at four years was 92% +/- 5%. Tw

TNF-α inhibitor therapy and prosthetic aortic valve endocarditis: a case report.

Tumor necrosis factor-alpha (TNF-α) inhibitors and other biologics used for autoimmune diseases are associated with low-grade immunosuppression. Treatment with these and the presence of prosthetic mechanical cardiac valves both increase the risk of infective endocarditis (IE). However, evidence on the risk of prosthetic valve endocarditis (PVE) among patients treated with TNF-α inhibitors is limited. This study reports a 41-year-old man with a prosthetic aortic valve who was on treatment with golimumab and presented with low-grade fevers and positive blood cultures for Staphylococcus epidermid
