The Effectiveness of Guselkumab by BMI Category Among Patients with Moderate-to-Severe Plaque Psoriasis in the CorEvitas Psoriasis Registry.

INTRODUCTION: Prior studies have demonstrated guselkumab improves disease activity and patient-reported outcomes (PROs) among patients with moderate-to-severe plaque psoriasis. However, the real-world effectiveness of guselkumab across different subgroups [e.g., body mass index (BMI) categories] remains an area of active research. METHODS: This study included patients enrolled in the CorEvitas Psoriasis Registry between July 18, 2017 and March 10, 2020 who had moderate-to-severe psoriasis [Investigator's Global Assessment (IGA) score ≥ 3], initiated guselkumab at a registry visit (index date)…

The Role of EREG, PTPN1, and SERPINB7 Genes in the Pathogenesis of Psoriasis: May SERPINB7 Be Protective and a Marker of Severity for Psoriasis?

INTRODUCTION: Psoriasis is a chronic inflammatory dermatological disease with complex pathogenesis in which many immune system cells, including keratinocytes, play a role. Many genes regulate the proliferation of keratinocytes and other immune cells that have essential roles in the pathogenesis of psoriasis. The expressions of EREG, PTPN1, and SERPINB7 genes were shown as upregulated in psoriatic skins in a few studies previously. OBJECTIVES: We aimed to evaluate the expressions of these genes in psoriatic lesional skin and compared them with non-lesional adjacent skin of the same patients an…

Biological treatment for psoriasis and the risk of herpes zoster: results from the Psoriasis Longitudinal Assessment and Registry (PSOLAR).

Purpose: To describe the risk of herpes zoster (HZ) in patients with psoriasis and its relation to non-biologic systemic therapies or biologic treatment. Materials and methods: Psoriasis Longitudinal Assessment and Registry (PSOLAR) is an international, prospective, registry that follows adult patients with psoriasis eligible to receive non-biologic systemic therapies or biologic therapies. Mutually exclusive therapy cohorts were defined. HZ incident rates were calculated for each therapy cohort and rates between cohorts were compared using hazard ratios (HR) adjusted for potential confounders…

The Role of the Nervous System in the Pathophysiology of Psoriasis: A Review of Cases of Psoriasis Remission or Improvement Following Denervation Injury.

As most efforts in the last decade have focused on the immunologic basis of inflammatory skin disease, there has been less emphasis on the role of the nervous system in the disease process of psoriasis. Evidence in support of the neurocutaneous pathway has come from observations of patients experiencing unilateral improvement and even complete remission following nerve damage in the affected dermatomal region. The aim of this review was to investigate the role of neuropeptides in the intricate pathophysiology of psoriasis. The PubMed database was searched for individual case reports or case se…

Demographic and clinical correlates of extent of psoriasis during stable disease and during flares in chronic plaque psoriasis.

BACKGROUND: Following a previous population-based study, we define a common psoriatic phenotype (A) with a limited area of involvement of stable disease but extensive flares and a less common phenotype (B) with consistently widespread disease. OBJECTIVES: To define these phenotypes quantitatively and to investigate any biological significance through correlations with clinical disease characteristics. Psoriatic plaque thickness was also included in the analyses. METHODS: Two hundred and ninety-four patients who had had chronic plaque psoriasis for at least 5 years were recruited. Area of inv…

[The combination of psoriasis with different types of lipoproteinemias. 2. The characteristics of the dyslipoproteinemias caused by disorders in triacylglyceride transport in psoriasis].

The findings of examination of patients with disseminated psoriasis vulgaris evidence a reduction of high-density lipoprotein (HDLP) cholesterol in women and of low-density lipoprotein cholesterol in men with type IV hyperlipoproteinemia (HLP) as against the normolipidemic patients. Correlation of serum triacylglycerides and HDLP cholesterol concentration was discovered only in men with type IV HLP (r = -0.72, p less than 0.01), administered lipid-reducing and cytostatic drugs. The period of hospitalization was longer and the remissions less frequent in these patients in comparison with normol…

Psoriasis flare confused with drug allergy: A collaborative effort is required to treat tuberculosis in the setting of severe psoriasis.

BACKGROUND: Psoriasis is a chronic autoimmune disorder. Severe psoriasis is treated with systemic immunosuppressive agents. Systemic immunosuppression increases the risk of tuberculosis (TB) disease. Sudden cessation of immunosuppression seems logical in a TB patient but can lead to psoriasis flares. And when a new rash occurs during TB therapy, drug reaction is usually suspected. This can result in untreated TB disease or intermittent therapy, thus increasing the risk for acquired drug resistance. METHODS: We describe two patients who developed TB disease during immunosuppressive therapy for…

The Definition of Psoriasis Severity and Treatment Target of Psoriasis: Korean Expert Consensus Using the Modified Delphi Method.

BACKGROUND: The emergence of new treatments has significantly elevated treatment expectations for psoriasis. However, existing Korean severity definitions and treatment targets of psoriasis remain primarily centered on traditional Psoriasis Area and Severity Index (PASI) and Body Surface Area (BSA) thresholds, often underestimating the disease burden in high-impact special areas. OBJECTIVE: To develop a revised Korean expert consensus on psoriasis severity and treatment targets that incorporate the clinical significance of special areas and reflect the evolving therapeutic landscape. METHODS…

Is Cumulative Life Course Impairment Considered in Psoriasis Management? A Multinational Survey of People with Psoriasis and Healthcare Professionals.

INTRODUCTION: Delays remain in patients receiving effective treatment strategies that have potential to clear their skin of psoriasis, improve their quality of life (QoL) and change the psoriatic disease course, which, if uncontrolled, can irreversibly alter an individual's life course (i.e. cumulative life course impairment [CLCI]). This study explored current international awareness and consideration of the potential impact of psoriasis over the life course within clinical assessments and decisions about its management. METHODS: Cross-sectional surveys collated insights from people with pso…

Keratinocyte Exosomes for Topical Delivery of Tofacitinib in Treatment of Psoriasis: an In Vitro/ In Vivo Study in Animal Model of Psoriasis.

INTRODUCTION: Exosomes are extracellular vesicles in the range of 40-150 nm released from the cell membrane. Exosomes secreted by keratinocytes can communicate with other keratinocytes and immune cells with specific biomarkers at their surface, which may be effective on inflammation of psoriasis and its pathogenesis. OBJECTIVE: The present study aimed to formulate and study effectiveness of an exosomal delivery system of tofacitinib (TFC). METHODS: TFC was loaded by different methods in exosomes and then characterized for particle size, zeta potential, drug loading efficiency, and release ef…

Psoriasis localization patterns in the Swiss Psoriasis Registry (SDNTT) over 11 years: an analysis by sex and age.

Real-world data on anatomically localized psoriasis and its response to systemic therapy across different age-groups and sexes is limited. This study aimed to evaluate the severity and distribution of psoriasis over time in female and male patients receiving systemic therapies, categorized by age within the Swiss psoriasis registry (SDNTT). Patient-data was obtained over 11 years through the SDNTT. The localized Psoriasis Area and Severity Index (locPASI) of the head, trunk, upper and lower extremities was analyzed over two years following the start of systemic non-/biologic treatment. A total…

Evaluation of Kynu, Defb2, Camp, and Penk Expression Levels as Psoriasis Marker in the Imiquimod-Induced Psoriasis Model.

BACKGROUND: Psoriasis is a noncontagious auto-inflammatory chronic skin disease. So far, some of the inflammatory genes were upregulated in mouse model of psoriasis. This study examined changes in skin mRNA expression of L-kynureninase (Kynu), cathelicidin antimicrobial peptide (Camp), beta-defensin 2 (Defb2), and proenkephalin (Penk) in a mouse model of imiquimod-induced psoriasis. MATERIALS AND METHODS: Tree groups of C57BL/6 female mice were allocated. The imiquimod (IMQ) cream was administered to the mice dorsal skin of the two groups to induce psoriatic inflammation. In the treatment gro…

Approaches to nutrition intervention in plaque psoriasis, a multi-system inflammatory disease-The Diet and Psoriasis Project (DIEPP).

Psoriasis is an immune-mediated inflammatory skin disease affecting approximately 2% of the UK population. Its pathogenesis is suggested to be an outcome of genetic and environmental interplay. People with psoriasis have an increased likelihood of developing other conditions such as type 2 diabetes and cardiovascular disease. Systemic inflammation is hypothesised to be the common link between psoriasis and cardio-metabolic diseases. Emerging evidence shows diet as a potential therapeutic adjunct in the management of psoriasis. The Diet and Psoriasis Project (DIEPP) aims to investigate whether …
