[CT findings in patients with chronic thrombembolic pulmonary hypertension].

Chronic thromboembolic pulmonary hypertension (CTEPH) is thought to be a rare complication of pulmonary embolism. However, it was recently demonstrated that CTEPH is more common than previously thought after pulmonary embolism. Without treatment, CTEPH is associated with a very high mortality rate. Making the correct diagnosis early is essential, because there is a potential curative treatment in the form of pulmonary thromboendarterectomy (PTE). Because of the unspecific clinical symptoms of CTEPH, the different imaging modalities play a crucial role in diagnosis making. Since the introductio

A patient with mild respiratory COVID-19 infection who developed bilateral non-hemorrhagic adrenal infarction.

A 76-year-old woman was admitted to the emergency room of Nagano Municipal Hospital with the complain of severe back pain. Chest and abdominal enhanced computed tomography scans showed bilateral adrenal infarction and minute pulmonary nodules, but she had no respiratory symptoms. After admission, a family member of the patient was found to have been in close contact with a coronavirus disease 2019 (COVID-19) patient. Thus, polymerase chain reaction and antigen tests of severe acute respiratory syndrome coronavirus 2 were conducted, and both tests returned positive. D-dimer levels were normal o

Decreased baroreflex sensitivity in patients with stable coronary artery disease is correlated with the severity of coronary narrowing.

Although studies have shown that arterial baroreflex sensitivity (BRS) is decreased in patients with acute myocardial infarction, BRS changes in patients with stable coronary artery disease (CAD) have not been studied extensively. We assessed BRS by the phenylephrine method in 55 normotensive and nondiabetic patients with chronic effort angina, old myocardial infarction, or both. The control group consisted of 24 age-matched patients without coronary lesions. To identify factors that determine BRS in stable CAD, we performed multivariate analysis using age, sex, left ventricular ejection fract

Cardiac arrhythmias and myocardial ischemia after thoracotomy for lung cancer.

The records of 598 patients undergoing a thoracic surgical procedure for lung cancer from 1975 through 1989 were reviewed for occurrence of cardiac arrhythmias and myocardial ischemic events. Atrial tachycardias occurred in 16% (94/598); atrial fibrillation was preponderant (87%), followed by supraventricular tachycardia and atrial flutter. Patients with recurrent episodes of dysrhythmias had a significantly higher mortality rate than those without episodes or with a single episode only (17% versus 2.4%; p less than 0.01). Transient ischemic electrocardiographic changes were documented in 23 p

Left ventricular performance after acute myocardial infarction.

Current knowledge concerning the major hemodynamic features of acute myocardial infarction has been reviewed and discussed in relation to present concepts of cardiac pathophysiology. The physical examination provides a great deal of information and new, noninvasive methods promise to supplement the bedside appraisal of left ventricular function. Direct hemodynamic methods of serially monitoring patients with acute myocardial infarction are finding increasing application and recently have added considerably to our understanding of this condition. Certain limitations in the use of the central ve

Effect of hip fracture on prognosis of acute cerebral infarction.

OBJECTIVES: Hip fractures are a worldwide public health problem. The incidence of hip fracture is high among the elderly, and it is an important cause of death and disability in this population. This observational study aimed to investigate the effect of acute hip fracture on the recovery of neurological function and the prognosis of patients with acute cerebral infarction, as well as whether surgical treatment of combined acute fracture can improve the prognosis of patients. METHODS: Thirty patients with acute cerebral infarction combined with acute hip fracture, who were hospitalized in two

Alveolar hemorrhage: an underdiagnosed complication of treatment with glycoprotein IIb/IIIa inhibitors.

OBJECTIVE: Alveolar hemorrhage (AH) is a rare complication of treatment with GP IIb/IIIa inhibitors. Hemoptysis, a constant sign, lacks in specificity, and may occur in confounding syndromes such as pulmonary edema, pulmonary infarction, and pneumonia. Nonspecific symptoms and signs often delay the diagnosis, thereby allowing serious or even fatal disease progression. Here, we performed a large-scale retrospective analysis to define the incidence and risk factors of AH in the setting of GP IIb/IIIa inhibitors therapy. BACKGROUND: Randomized controlled trials demonstrate that treatment with gl…

Frequency and treatment of venous thromboembolic events in patients with space-occupying brain infarction and decompressive craniectomy.

INTRODUCTION: Venous thromboembolic events (VTEs) like deep vein thrombosis or pulmonary embolism are frequent complications in (neuro) critical ill patients. Anticoagulation for VTE after space-occupying brain infarction is a therapeutic dilemma. The aim of this retrospective study was to investigate the frequency of clinically apparent VTE in patients with acute ischaemic stroke (AIS) due to large vessel occlusion (LVO), its treatment, and the rate of complications. METHODS: Patients with first AIS due to LVO were assigned to one of the following groups: space-occupying brain infarction wit

A case of cerebral infarction in association with free protein S deficiency and oral contraceptive use.

The purpose of this article is to describe protein S deficiency as a cause of cerebral infarction in the young adult. We report a 27-year-old previously healthy woman with a primary free protein S deficiency, who developed a left temporoparietal infarction. Protein S is a naturally occurring vitamin K-dependent protein which, in conjunction with active protein C, inhibits the clotting cascade. Protein S deficiency is known to be of clinical significance in patients with deep venous thrombosis or pulmonary emboli and in these patients, treatment is long-term anticoagulation. Protein S deficienc

[Zygomycoses: clinical aspects and pathology in 10 patients].

Deep fungal infections caused by zygomycetes (so-called mucormycoses) are rare. Whereas in the past rhinocerebral mycosis dominated, today a wide range of clinical manifestations must be expected, especially pulmonary infections and invasion of vessel walls with systemic infarction. On the basis of 10 cases of Mucoraceae mycosis and a review of the literature, the predisposing conditions, clinical symptoms and pathologic-anatomic findings are described. Our observations include 7 patients with leukemia and 2 adults and one newborn without malignant neoplasia. Except for the patient with rhinoc

[Exacerbation of associated bronchial asthma in patients with myocardial infarct].

During follow-up of 27 patients with previous long-term history of bronchial asthma it has been established that in 7 of them there was aggravation of broncho-pulmonary illness in the late terms of setting in of myocardial infarction (on the 3rd-6th week, or in 30 days, on the average). Typical features in these patients were blood hypereosinophilia reaching 16-24 per cent and parallel development in some of them of clinical manifestations of the postinfarction syndrome such as pericarditis, pleurisy and transitory lung infiltration. It is believed that this spontaneous aggravation of bronchia

Two-dimensional echocardiographic recognition of papillary muscle fibrosis in pediatric patients.

Two-dimensional echocardiographic examinations were performed in seven infants with anomalous origin of the left coronary artery from the pulmonary artery (ALCA), in one infant with myocardial infarction and left main coronary obstruction, and in eight with critical valvar aortic stenosis (AS). Comparative qualitative echo density assessment demonstrated that each infant had a marked increase in the echo density of one or both left ventricular papillary muscles. The increased echo density was considered to represent fibrosis and scar formation as a result of ischemia and infarction. Pathologic

A stroke patient with persistently intermittent fever treated with gabapentin: A clinical case.

Fever is one of the most common complications in stroke patients and can generally be classified as either infectious or non-infectious. Infectious fevers are commonly caused by pulmonary infections, urinary tract infections, and secondary infections associated with medical interventions such as endotracheal intubation, urinary catheterization, and nasogastric tubes. Non-infectious fevers primarily manifest as central fevers, although in rare cases, they may also result from drug-induced causes. Existing research indicates that the most common cause of central fever is brainstem hemorrhage, fo

The outcomes of a standardized protocol for extracorporeal mechanical circulatory support selection-left ventricular challenge protocol.

There are no criteria for surgical mechanical circulatory system (MCS) selection for acute heart failure. Since 2021, we have utilized cardiopulmonary bypass system to assess patients' heart and lung condition to inform surgical MCS selection. we aimed to retrospectively analyze the outcomes of treatments administered using our protocol. We analyzed the data of 19 patients who underwent surgical MCS implantation. We compared patients' characteristics across the biventricular-assist device (BiVAD), central Y-Y extracorporeal membrane oxygenation (ECMO), central ECMO, and left VAD (LVAD) systems

Case Report: The Coronavirus Disease 2019 (COVID-19) Pneumonia With Multiple Thromboembolism.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) broke out in Wuhan, China, in late December 2019 and has since spread rapidly around the world. Severe coronavirus disease 2019 (COVID-19) pneumonia patients have abnormal blood coagulation function, but their thromboembolism prevalence is still unknown. We reported a case of a 49-year-old man infected with COVID-19, presenting with fever, chest pain, limb weakness, myalgia, and dyspnea. The patient was diagnosed with severe COVID-19 pneumonia, pulmonary thromboembolism (PTE), deep vein thrombosis (DVT), and cerebral infarction. He r

[Post operative embolism and left atrial enlargement in the old-aged ASD (atrial septal defect) patients].

As the operation risk of ASD is little today, the unexpected postoperative complication such as embolism is a miserable concern. We experienced two cases of embolism, which were cerebral infarction, in old-aged patients after ASD correction. It has been reported that low cardiac function, atrial fibrillation and foreign body in the left atrium should be risk factors of the embolism after ASD correction. However, blood stasis in the left atrium due to its enlargement may be an essential risk factor of the embolism. Accordingly we evaluated the left atrial volume in 87 ASD patients without compl

Phentolamine for vasodilator therapy in left ventricular failure complicating acute myocardial infarction. Haemodynamic study.

In 15 patients with acute myocardial infarction associated with signs of left ventricular dysfunction, phentolamine was infused intravenously in a dose of 10 mg per hour. This therapy induced a substantial reduction in mean right atrial pressure from 10 to 7 mmHg (1.3 to 0.9 kPa) (P) less than 0.001), and in pulmonary capillary wedge pressure from 20 to 13 mmHg (2.7 to 1.7 kPa) (P less than 0.001). The cardiac index increased from 2.5 to 3.0 1/min per m-minus 2 (P less than 0.001) accompanied by a fall in both the systemic and pulmonary vascular resistances (P less than 0.001). On the other ha
