[Complex fractures of the proximal end of the radius and ulna in adults: a new classification].

Unlike injuries involving only one of the forearm bones, complex lesions of the proximal end of the radius and the ulna are particularly unstable. Various situations-Monteggia fracture, transolecraneal dislocation, or fracture-dislocation-are encountered. The classification systems proposed to date and recalled here are insufficient, making it difficult to provide optimal therapy and also hindering comparison between published series. We propose a descriptive classification including all the anatomic varieties of complex fractures of the proximal end of the radius and the ulna. This classifica

The effects of distal radius fracture malalignment on forearm rotation: a cadaveric study.

Seven fresh cadaveric specimens were used to determine the loss of forearm rotation with varying distal radius fracture malalignment patterns. Uniplanar malunion patterns consisting of dorsal tilt, radioulnar translation, or radial shortening were simulated by creating an osteotomy at the distal end of the radius, orienting the distal fragment position using an external fixator, and maintaining the position with wedges and a T-plate. Rotation of the forearm was produced by fixing the elbow in a flexed position and applying a constant torque to the forearm using deadweights. Forearm rotation wa

Operative treatment of volar intra-articular fractures of the distal end of the radius.

We retrospectively reviewed the results of operative treatment of forty-nine volar marginal intra-articular fractures of the distal end of the radius. According to the Comprehensive Classification of Fractures, there were two B3.1 fractures (characterized by a small volar fragment, with the sigmoid notch intact), three B3.2 fractures (characterized by a large volar fragment that included the sigmoid notch), and forty-four B3.3 fractures (characterized by comminution of the volar fragment). Although all fractures healed and only nine patients had evidence of osteoarthrosis on follow-up radiogra

Minor axial shortening of the radius affects outcome of Colles' fracture treatment.

Functional disability after a Colles' fracture was correlated to the degree of axial shortening of the radius measured at the site of distal radioulnar joint. According to the sum of demerit points, the functional end result was unsatisfactory (fair or poor) in 4% of the patients with an acceptable anatomic result, in 25% of the patients with grade 1 shortening (corresponding to a shortening of 3 to 5 mm), and in 31% of the patients with grade 2 shortening (shortening of more than 5 mm). The results suggest that even a minor axial shortening of the radius, without concomitant malalignment of t

[Osteosynthesis using plates in fractures of the lower end of the radius with anterior displacement].

A series of 55 distal radial fractures with anterior displacement have been fixed by an anterior plate. Results were excellent or good in 78 per cent, moderate in 18.1 per cent and poor in 3.7 per cent. There was no radio-clinical correlation. However when the reduction was good, clinical result was always good or excellent. On long-term follow-up, results were unchanged, except when an arthritis appeared (2 cases out of 14 with 5 or more years follow-up). Surgical technic must be strict to get a stable and anatomic reduction with all fragments maintained, to prevent tendinous and nervous comp

[Instability of fractures of the lower end of the radius: apropos of a series of 167 cases].

One-hundred-sixty-seven consecutive cases of distal radius fracture have been treated in our department from October 1985 until November 198 … ological results were observed following surgical management. This mode of classification of distal radius fractures is helpful for determining prognosis and evaluating the risk of secondary displacement and in the course, should help in the selection, perhaps earlier of the most appropriate treatment.

[Distal radius fracture--indications for conservative, partial surgical and surgical therapy].

460 fractures of the distal radius including 91 which are treated surgically are investigated in a retrospective study. There were 260 fractures of the left and only 200 of the right radius. 66% of the patients were female, especially striking is the superior number of women older than 45 years. The evaluation of the final radiological results referring to anatomical joint angles and normal radius length shows a high correlation to the evaluation of the final functional results. 70% of the latter are valued as excellent or good, 19% as tolerable and 11% as poor. The classification according to

Acute volar compartment syndrome of the forearm secondary to fractures of the distal radius.

We present five cases of acute volar compartment syndrome developing as a complication of fractures of the distal radius. The patients were all males under the age of 50 years who sustained comminuted or displaced fractures of the distal radius. We suggest that this indicates an 'at risk' group. The interval between injury and the onset of symptoms of volar compartment syndrome varied between 12 and 48 hours. The diagnosis of each case was made clinically and confirmed at operation. With early extensive decompression full neurovascular recovery can be expected.

Fracture of the upper end of the ulna associated with dislocation of the head of the radius in children.

Dislocation of the head of the radius associated with fracture of the upper end of the ulna is a common elbow injury in children. The literature on diagnosis, however, is relatively limited. The injury can be misdiagnosed and consequently mismanaged. In the present series of 46 children, the age range was from 2.5 to 12 years. The dislocation of the radial head was lateral in 50% of the cases, posterolateral in 28.2%, anterolateral in 17.4%, and anterior in 4.3%. Thorough clinical and radiological examinations are essential. Simple bending of the upper end of the ulna may help to avoid missing

Fractures of the distal radius. Intermediate and end results in relation to radiologic parameters.

The purpose of this study was to quantify the functional impairment following distal radial fractures and to identify the factors affecting prognosis. One hundred fifteen patients were assessed six months and two years following initial injury. On final assessment, subjectively, 56% had good, 39% had fair, and 5% had poor results. Median grip strength improved from 51% to 78%, range of movement from 87% to 94%, and wrist torque from 93% to 100%. Redisplacement occurred in 59%; only 33% clinically and 19% radiologically had perfect cosmetic results. Radial malunion was important functionally. O

Closed rupture of extensor digitorum communis tendon following fracture of the radius.

A case of closed rupture of the extensor digitorum communis tendon following a fracture of the lower end of the radius with anterior displacement, is described. This rare complication was treated successfully by abductor pollicis longus tendon transfer.

Distal Radius Interventions for Fracture Treatment (DRIFT) trial: study protocol for a multicentre randomised clinical trial of completely translated distal radius fractures at paediatric hospitals in North America.

INTRODUCTION: Distal radius fractures are the most common fractures seen in the emergency department in children in the USA. However, no established or standardised guidelines exist for the optimal management of completely displaced fractures in younger children. The proposed multicentre randomised trial will compare functional outcomes between children treated with fracture reduction under sedation versus children treated with simple immobilisation. METHODS AND ANALYSIS: Participants aged 4-10 years presenting to the emergency department with 100% dorsally translated metaphyseal fractures of…
