[Amyloidosis: classification, diagnosis, and treatment].

Amyloidoses are rare protein-misfolding disorders characterized by the deposition of insoluble fibrillar aggregates, leading to potentially life-threatening organ damage. These deposits can be systemic or localized. Systemic amyloidoses are often associated with monoclonal gammopathies, chronic inflammatory conditions, or have a hereditary origin. Treatment strategies primarily focus on reducing the production of amyloidogenic proteins-depending on the specific protein and its origin-through therapies such as chemotherapy, anti-inflammatory agents, or gene-silencing approaches. Early diagnosis

Asian expert consensus on high-quality hypertension management.

High-quality hypertension management is a new concept proposed to improve blood pressure (BP) control in Asia. Out-of-office BP measurements, including ambulatory and home BP monitoring, and wearable BP measurement, are recommended for BP assessment. Long-acting antihypertensive agents at full dose or in combination are priority strategies for achieving 24-h (24-h) BP control, reducing blood pressure variability (BPV) and improving time-in-target range (TTR). High-quality hypertension management across multiple disciplines will be a pragmatic strategy for the reduction in the risk of cardio-ce

Perioperative Anemia and Coagulopathy Management in Pediatric Patients with Medical Complexity.

Perioperative anemia and coagulopathy are frequent yet modifiable conditions in pediatric patients with medical complexity, significantly influencing surgical morbidity and mortality. Iron deficiency anemia is the most common type of anemia, underscoring the importance of early detection, dietary modification, and timely oral or intravenous iron supplementation. Restrictive transfusion thresholds and selective use of erythropoiesis-stimulating agents are recommended to reduce transfusion-related complications. Management of other nutritional (folate, vitamin B12, copper, and zinc) and non-nutr

Hydrogen peroxide pathway in ulcerative colitis: Promises and challenges in translating novel pathogenesis to clinical practice.

This letter addresses Pravda's innovative review, which proposes hydrogen peroxide as the primary pathogenic factor in ulcerative colitis (UC). Although the author presents intriguing mechanistic insights and reports encouraging clinical outcomes with reducing agents, several methodological and clinical considerations require discussion. We examine three key aspects: The selective evidence synthesis approach; the need for rigorous clinical validation of proposed therapies; and the integration of this novel hypothesis with established inflammatory bowel disease pathogenesis. Given the complexit…

Opioid-free anesthesia in enhanced recovery after surgery for gastrointestinal surgery: current status, challenges, and prospects.

The enhanced recovery after surgery (ERAS) protocol has been increasing implementation in gastrointestinal surgeries to optimize perioperative management, mitigate surgical stress responses, and accelerate patient recovery. Although opioid-based anesthesia effectively alleviates pain, it is associated with significant adverse effects, including postoperative nausea and vomiting (PONV), respiratory depression, and intestinal paralysis, which can impeder early recovery. Opioid-free anesthesia (OFA) is designed to alleviate these concerns. This article examines the pharmacological agents and regi

High Bleeding Risk in Patients Undergoing Coronary and Structural Heart Interventions.

Percutaneous coronary and structural heart interventions are increasingly preferred over cardiac surgery due to reduced rates of periprocedural complications and faster recovery but often require postprocedural antithrombotic therapy for the prevention of local thrombotic events. Antithrombotic therapy is inevitably associated with increased bleeding, the extent of which is proportional to the number, duration, and potency of the antithrombotic agents used. Bleeding complications have important clinical implications, which may outweigh the expected benefit of reducing thrombotic events. Herein

Room Temperature Reduction of Nitrogen Oxide on Iron Metal-Organic Frameworks.

Nitrogen oxides represent one of the main threats for the environment. Despite decades of intensive research efforts, a sustainable solution for NOx removal under environmental conditions is still undefined. Using theoretical modelling, material design, state-of-the-art investigation methods and mimicking enzymes, it is found that selected porous hybrid iron(II/III) based MOF material are able to decompose NOx, at room temperature, in the presence of water and oxygen, into N2 and O2 and without reducing agents. This paves the way to the development of new highly sustainable heterogeneous catal…

Considerations in the Treatment of Depression and Anxiety in Women with PCOS.

Women with polycystic ovary syndrome (PCOS) are known to be at a greater risk of depression and anxiety. What is less clear is whether existing treatments for PCOS are effective in managing this increased risk and what the optimal approach to treatment is. In this review, currently available interventions are explored including lifestyle modifications, oral contraceptives, insulin sensitizing agents, psychosocial interventions and psychiatric medications. While data are often conflicting, lifestyle interventions, and cognitive behavioral therapy (CBT) appear most promising in reducing depressi

Case of bullous pemphigoid refractory to corticosteroids by antiepileptic drug-induced CYP3A4.

The most common treatment option for patients with bullous pemphigoid is systemic corticosteroids. CYP3A4, a drug-metabolizing enzyme in the liver, metabolizes synthetic steroids to a varying degree. Although there are many CYP3A4-inducing drugs, several antiepileptic drugs, such as phenytoin and phenobarbital, strongly induce CYP3A4, thereby reducing the effects of corticosteroids. Here, we report a case of refractory bullous pemphigoid that rapidly improved after the discontinuation of phenytoin and phenobarbital. To achieve adequate pharmacological effects of corticosteroids, we must always

Noninvasive Approaches for Anxiety Reduction During Interventional Radiology Procedures.

Periprocedural anxiety is a major cause of morbidity, particularly for interventional radiology procedures that often depend on conscious sedation. Management of anxiety and pain during image-guided procedures has traditionally relied on pharmacologic agents such as benzodiazepines and opioids. Although generally safe, use of these medications risks adverse events, and newer noninvasive, nonpharmacologic techniques have evolved to address patient needs. In this review, we explore the roles of hypnosis, structured empathic attention, anodyne imagery, music, video glasses, and mobile application

Avian Ganglioneuritis in Clinical Practice.

Avian ganglioneuritis (AG) comprises one of the most intricate pathologies in avian medicine and is researched worldwide. Avian bornavirus (ABV) has been shown to be a causative agent of proventricular dilatation disease in birds. The avian Bornaviridae represent a genetically diverse group of viruses that are widely distributed in captive and wild populations around the world. ABV and other infective agents are implicated as a cause of the autoimmune pathology that leads to AG, similar to human Guillain Barrè syndrome. Management of affected birds is beneficial and currently centered at reduc

Long-term Survival in a Kidney Transplantation Patient With Progressive Multifocal Leukoencephalopathy: A Case Report.

Post-kidney transplantation progressive multifocal leukoencephalopathy (PML) is a rare disease on which there are very few published reports on record. PML is a demyelinating disease caused by a destructive infection of the oligodendrocytes by the JC polyomavirus. No effective therapeutic protocol has been established other than measures to revive the immune function by reducing or discontinuing the administration of immunosuppressive agents. Most cases are progressive and show a poor prognosis. We herein report a case in which renal function has been maintained for 2 years following the onset

Management of Compensatory Sweating After Sympathetic Surgery.

Compensatory hyperhidrosis (HH) is the most common and feared side effect of thoracic sympathectomy, because patients with severe forms have their quality of life greatly impaired. The most well-known factors associated with compensatory HH are extension of manipulation of the sympathetic chain, level of sympathetic denervation, and body mass index. Technical developments as well as the proper selection of patients for surgery have been crucial in reducing the occurrence of severe forms of compensatory HH. Therapeutic options include topical agents, botulinum toxin, systemic anticholinergics,

Metabolic Bone Disease in Primary Biliary Cirrhosis.

Primary biliary cirrhosis (PBC) is a liver-specific autoimmune disease that primarily affects women (female-to-male ratio, 10:1) between 40 and 60 years of age. Metabolic bone disease is a common complication of PBC, affecting 14% to 52% of patients, depending on the duration and severity of liver disease. The osteoporosis seen in PBC seems mainly due to low bone formation, although increased bone resorption may contribute. Treatment of osteoporosis consists primarily of antiresorptive agents. Additional large prospective, long-term studies in patients with PBC are needed to determine efficacy

Visit-to-visit blood pressure variability and classes of antihypertensive agents; associations with artery remodeling and the risk of stroke.

Recent studies have shown that visit-to-visit blood pressure (BP) variability was emerging as an independent risk factor for stroke. Although the mechanism is not fully understood, artery remodeling would be closely associated with the relationship between visit-to-visit BP variability and stroke. In addition, the class of antihypertensive agents is suggested to be an important determinant of visit-to-visit BP variability. This review article summarizes the recent literature on these topics. In the elderly hypertensives, strict BP control using calcium channel blockade would play a crucial rol

The role of sonographic cervical length in labor induction at term.

The purpose of this study is to review the literature examining the role of ultrasound in the induction of labor. Databases including Ovid, PubMed, Web of Science, Google Scholar, and UpToDate were searched and current guidelines from the SOGC, the ACOG, the RCOG, and the RANZCOG were reviewed. Although studies have not demonstrated the superiority of cervical sonography to the Bishop score, the evidence indicates that sonography could be useful in planning induction of labor, significantly reducing the need for cervical ripening agents. A more comprehensive method integrating both sonography

[Environmental virology and sanitation in Brazil: a narrative review].

Sanitation services play a critical role in controlling transmission of numerous waterborne pathogens, especially viruses that cause acute gastroenteritis and hepatitis. The viral agents with the greatest public health impact are hepatitis A virus, rotaviruses and noroviruses, adenoviruses, and enteroviruses, contaminating many Brazilian aquatic ecosystems. Heavy circulation of viruses in the environment has been related to inadequate local sanitary conditions, including incomplete coverage of services or inefficacy of conventional technologies in eliminating or reducing the viral load in wate

Valproate as a treatment for dopamine dysregulation syndrome (DDS) in Parkinson's disease.

It has been previously well established that the use of dopaminergic agents in Parkinson's disease may contribute to behavioral disturbances such as dopamine dysregulation syndrome (DDS), impulse control disorders (ICD), and punding. ICD and punding have been most commonly addressed by reducing dose or by discontinuing the use of a dopamine agonist. Treatment of DDS has proven more challenging, and to date there has been no standard approach. In this paper, we review a series of four patients who met criteria for DDS, who were all refractory to medication adjustments. The DDS symptoms responde

Polycystic ovarian syndrome management options.

Polycystic ovarian syndrome (PCOS) is a disorder of androgen excess and ovarian dysfunction. Hirsutism and elevated free testosterone levels are the most consistent signs of the androgen excess. Irregular, infrequent, or absent menses and infertility are symptoms of ovulatory dysfunction. Obesity is also a feature of this syndrome and contributes to associated metabolic abnormalities. Lifestyle modification should be the first treatment and is effective in reducing the signs and symptoms. The ovulatory infertility associated with PCOS can be overcome in most cases with oral (clomiphene citrate

Regression of paroxysmal atrial fibrillation associated with thiazolidinedione therapy.

Thiazolidinediones (TZDs) represent insulin-sensitizing agents that have several pleiotropic properties, possibly related to their favorable effects on cardiovascular remodeling. We briefly describe 2 diabetic patients who experienced a remarkable improvement in their paroxysmal atrial fibrillation (AF) after treatment with rosiglitazone. Current evidence suggests that atrial remodeling represents a prominent mechanism of AF development and perpetuation while inflammation and oxidative stress are possibly implicated in this process. It could therefore be speculated that the pleiotropic effects
