Neurotologic Findings of Idiopathic Acute Unilateral Audiovestibulopathy.

BACKGROUND AND PURPOSE: Unlike acute unilateral vestibulopathy or vestibular neuritis (AUVP/VN), of which the diagnosis is made based on well-established neurotologic findings, the diagnosis of idiopathic acute unilateral audiovestibulopathy (iAUAV, also known as labyrinthitis) still requires further elucidation. METHODS: We retrospectively reviewed the medical records of patients with first-onset acute audiovestibulopathy (AAVS) in a referral-based university hospital in South Korea between March 2018 and September 2024. The results of video-oculography, video head-impulse, and other neuroto

The map of dizziness in vestibular schwannoma.

INTRODUCTION: Dizziness is a frequent complaint in patients with vestibular schwannoma (VS). An abnormal vestibulo-ocular reflex (VOR) can explain this dizziness in patients with VS. The video Head impulse test (vHIT) offers a chance to describe specifically the VOR findings in such patients. STUDY DESIGN: Retrospective cases series study in a tertiary referral hospital. METHODS: Fifty consecutive patients with VS were classified in accordance with the morphology of the VOR; gain, covert saccade, and overt saccade were analyzed both in the affected side and in the healthy side. For all patie

Video head impulse testing (VHIT) in the pediatric population.

OBJECTIVES: VHIT is the first vestibular test to evaluate all six semicircular canals. This test has advantages over the rotary chair and caloric tests in evaluating children since it does not require fear-inducing darkness or provocation of dizziness. The goal of this study was to review our initial experience with VHIT in children and adolescents at a pediatric vestibular program. METHODS: Results using the ICS Impulse VHIT device in 33 patients <20 years of age were retrospectively reviewed. Rotary chair testing was used to designate 26 subjects into groups with normal (n=20) and abnormal

Are otolithic inputs interpreted better in figure skaters?

The aim of this study was to investigate whether figure skaters, as individuals who experience intense vestibular stimulation, presented modification of the otolith-ocular reflex. The reflexes of 12 figure skaters were assessed using off vertical axis rotation (OVAR). Horizontal otolith-ocular reflex during OVAR is characterized by two parameters: the eye velocity horizontal modulation, assumed to compensate for perceived lateral linear translation, and the bias, assumed to compensate for the perceived rotation. We observed that skaters presented smaller amplitude of modulation and truly compe

Active versus passive head-shaking nystagmus.

CONCLUSIONS: We recommend performing the test for nystagmus actively, both when the response is analyzed at bedside or with videonystagmography. If no nystagmus is observed with the active test, it should then be performed passively. OBJECTIVE: Head-shaking nystagmus (HSN) is a sign of vestibular dysfunction that is elicited after actively (patient) or passively (examiner) shaking a patient's head. The objective of this study was to analyze the frequency of HSN when the test is performed actively (aHSN) or passively (pHSN), and to determine whether any differences in the results might be due

[Testing of the macula induced vestibuloocular reflex--possibilities and problems in the clinical routine].

INTRODUCTION: During the last years, an increasing number of papers is presented dealing with diseases and diagnostic procedures of the macula function. On the other hand, there are just a few distinct test setups for this purpose. In clinical routine, this kind of test system has to be efficient and effective to diagnostic and therapeutic findings. METHODS: The presented test setup includes examination of the semicircular canals by means of harmonic acceleration pendular rotation test and the bilateral, bithermal test as well as otolith function tests i. e. off vertical axis rotation (OVAR)

Eye movements to yaw, pitch, and roll about vertical and horizontal axes: adaptation and motion sickness.

BACKGROUND: In the search for parameters to predict motion sickness that can be measured in the laboratory, we performed a longitudinal investigation in aviators. Since the vestibular system is involved in the generation of motion sickness as well as eye movements, vestibulo-ocular reflex (VOR) parameters seemed relevant. We investigated three topics: 1) the effect of axis orientation and its orientation to gravity on the VOR; 2) changes in VOR parameters depending on flight experience; and 3) differences in VOR parameters in aircrew with high and low susceptibility to motion sickness. The velocity step test. Its use in the evaluation of the effects of drugs in dizzy patients.

The effect of flunarizine (10 mg/day) was studied in open and double-blind clinical trials. Patients presenting the symptom of dizziness were diagnosed as having the hyperventilation syndrome. They showed vestibular hyperreactivity in the velocity step test. This test was performed before and after 4-6 weeks of treatment. The results of treatment were compared with a control or placebo group. In an open trial, 12 of 14 patients reported alleviation of dizziness during treatment and showed a significant decrease in right-left asymmetry of their velocity step responses. In a double-blind trial,

The influence of sensory function on walking stability in aging and falls through a head-movement perturbation model.

Age-related sensory decline impairs responses to balance disturbances, increasing fall risk. Evaluating gait and balance under sensory challenges can help reveal how this decline affects instability in older adults. This study (Clinicaltrials.gov NCT06345625) incorporates head movements to evaluate the effect of sensory function - cervical sensorimotor control, vestibular function, and visual acuity - on walking stability in aging and its relationship with falls through a novel augmented-reality perturbation model. Sixty-one healthy, community-dwelling older adults (76.2 years; 40 females) wit

Identification of blood-based inflammatory biomarkers for vestibular neuritis using a proximity extension assay.

ObjectiveThe present study aimed to identify blood-based inflammatory biomarkers that would help investigate the underlying pathogenesis in vestibular neuritis (VN) using a proximity extension assay (PEA).MethodsIn this single-center prospective study, we enrolled both a cross-sectional (acute VN vs normal controls) and a longitudinal (acute phase vs recovery phase) cohorts. We quantified 92 plasma inflammatory proteins using the Olink PEA and identified differentially expressed proteins (DEPs) in each cohort using t-tests. The clinical utility and predictive value of each DEP were evaluated u

Case report: Spiller syndrome initially mimicking vestibular neuritis.

Spiller syndrome is a rare subtype of medial medullary infarction (MMI). Herein, we report on a patient with progressing stroke who presented with the initial features of acute peripheral vestibulopathy and MMI (Spiller syndrome), as confirmed by magnetic resonance imaging (MRI). A 42-year-old man experienced acute persistent vertigo with nausea, vomiting, and severe gait instability for 6 h before presenting to the emergency department. He exhibited spontaneous right-beating horizontal-torsional nystagmus that intensified on rightward gaze. The patient fell to the left side during the Romberg

The Different Stages of Vestibular Neuritis from the Point of View of the Video Head Impulse Test.

Vestibular neuritis (VN) is one of the most common causes of acute vestibular syndrome (AVS). Quantifying the vestibulo-ocular reflex (VOR) gain by the video Head Impulse Test (vHIT) could provide useful information to diagnose VN. This study aims to retrospectively evaluate the VOR gain values during the acute and subacute stages of the VN and to correlate these values with the patients' dizziness-related handicap. Medical record of 28 patients with VN were reviewed. Patients were assigned to two groups according to the time since the acute vestibular syndrome (AVS). One group with patients a

[Paradigm shift in management of patients with vertigo and imbalance].

Dizziness is one of the most common causes of medical visits. Management of the dizzy patient may be challenging both for the general practitioner, in emergency departments, and special clinics, as behind a seemingly homogeneous clinical presentation several very different etiologies may occur. Research of the last two century enriched our knowledge about physiology and pathophysiology of the vestibular system. Much knowledge is now available about the labyrinth being able to sense head motions and gravity, processing of afferent vestibular stimuli, reflectory oculomotor and postural control,
