We present the cross-sectional imaging and angiographic findings of hyaline vascular-type Castleman disease located in the retroperitoneum. The diagnosis was made postoperatively. This entity can simulate a malignant neoplasm. The histologic subtypes and presentations of Castleman disease and the differential diagnosis of retroperitoneal masses are discussed.

The authors present the case-history of a patient with multiple injuries where the dominating injury was haemorrhage into the retroperitoneum and subsequently into the abdominal cavity. The solution after CT and sonographic diagnosis was laparoscopy and subsequent arteriography with embolization. The authors consider the described procedures as the most suitable one in the given case.

There continue to be several controversies surrounding the role for retroperitoneal lymphadenectomy (RPL) in the management of patients with germ cell cancer of the testis. The initial treatment options for those with clinical stage I disease are surveillance (orchiectomy only), RPL or chemotherapy. Survival rates are similar with RPL and surveillance. Surgical morbidity has been reduced as techniques for RPL continue to improve. The likelihood of early or late (> 2 years) recurrence in the retroperitoneum is almost eliminated by RPL. BACKGROUND: Retroperitoneoscopic surgery was performed on a 41-year-old man with a retrocaval ureter.
METHODS: The retrocaval segment of the ureter was retroperitoneoscopically separated and the ureter was reanastomosed using an automatic suture device.
RESULTS/DISCUSSION: He returned to normal activities four days postoperatively. Postoperative excretory urography showed improvement of hydronephrosis and no residual stenosis.

We report an unusual case of left common iliac artery and vein injury during laparoscopy. The injury was recognized 3 h after the completion of the laparoscopy. The patient was promptly treated by explorative laparotomy and repair of the lacerated vessels. Major vascular injury during laparoscopy is usually caused by the Verres needle or the first laparoscopic trocar. Chyloperitoneum is a rare complication of abdominal or retroperitoneal surgery, and the combined occurrence of chylothorax and chyloperitoneum has been reported in only a few cases. We describe a case of this complication, which became clinically apparent 20 days after a Warren shunt operation. The large chylous effusion compromised breathing and required chest tube drainage, pleurodesis, medium chain triglyceride (MCT) diet, and, later, total parenteral nutrition (TPN). The patient made a full recovery on TPN for 5 weeks.

Laparoscopic approaches to adrenalectomy have been limited by the retroperitoneal (RP) location of the adrenal glands and their relative inaccessibility transabdominally. We developed a technique for endoscopic adrenalectomy in a domestic swine model using insufflation of the RP space with CO2 and retroperitoneoscopy. Ultrasonographic investigation of the kidneys and efferent urinary tract as well as juxtarenal organ regions has become an indispensable screening method in urology today. Various diseases of the kidneys can be detected rapidly and noninvasively. Some of these clinical pictures are discussed in detail. Above and beyond simple imaging, ultrasonography also allows special diagnostic interventions (biopsy) and therapeutic measures (i.e. nephrostomy).

Percutaneous Greenfield filter placement through a femoral vein approach resulted in errant retroperitoneal placement in a patient with pulmonary emboli. A properly positioned filter was then placed surgically through the right internal jugular vein. The patient subsequently required removal of the errant retroperitoneal filter because of genitofemoral nerve irritation. Problems associated with percutaneous Greenfield filter placement and pertinent literature are discussed.

Percutaneous balloon dilatation was attempted in 15 consecutive neonates (mean age 7.3 (range 1-27) days and weight 3.2 (range 2.5-4.1) kg) with critical stenosis of the pulmonary valve. A patient with the rare presentation of posterior mediastinal echinococcosis is reported. Magnetic resonance imaging (MR) demonstrated the extension of a primary retroperitoneal cyst into the posterior mediastinum which was subsequently confirmed surgically to be of echinococcal origin. Such a case of phrenomediastinal echinococcosis has been described only once before in literature.

A case of Whipples disease presenting with polyarthropathy and weight loss is presented. Ultrasound revealed unusual distinctive diffusely echogenic retroperitoneal lymphadenopathy which resolved after treatment. These appearances, particularly in a young male, support this diagnosis.

Two unusual cases of primary hydatid disease are described. In one of them a retroperitoneal cyst, presenting as a palpable abdominal mass, was strongly adherent to the inferior vena cava and dislocated the right kidney. Partial cystectomy and drainage were performed. The other cyst, of the right ischiorectal fossa, was initially misinterpreted as a perineal of sciatic hernia. It was completely removed. No other site of hydatid disease was found and the patients remain well 3 and 4 years postoperatively.
