Comparison by Race of Conservative Management for Low-Risk and Intermediate-Risk Prostate Cancers in Veterans From 2004 to 2018.

IMPORTANCE: Conservative management (ie, active surveillance or watchful waiting) is a guideline-based strategy for men with low-risk and intermediate-risk prostate cancer. However, use of conservative management is controversial for African American patients, who have worse prostate cancer outcomes compared with White patients. OBJECTIVE: To examine the association of African American race with the receipt and duration of conservative management in the Veterans Health Administration (VA), a large equal-access health system. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from

Management of women at increased risk for breast cancer secondary to high-risk proliferative lesions and family history of the disease.

Women with breast biopsies showing high-risk proliferative lesions such as atypical hyperplasia (AH) and lobular carcinoma in situ (LCIS) have an increased risk of developing breast cancer. Other factors including age, family history of breast cancer, and extent of AH may play a role in increasing breast cancer risk. In addition to women with AH, there is a subset of women with a positive family history of breast cancer, without a known germline mutation, which places them also at an increased risk for breast cancer. Clinical management, screening, chemoprevention, and surgical risk-reduction

The safety of outpatient management for low-risk pulmonary embolism: A narrative review.

Acute venous thromboembolism (VTE) remains the third most common cause of cardiovascular death, following myocardial infarction and stroke. It represents a global health threat with a significant economic burden. Risk stratification for short-term adverse events is crucial for the management of acute pulmonary embolism (PE). Over the past decade, management of acute PE has evolved toward greater individualization, guided by risk stratification. Supported by the results of recent randomized clinical trials, American and European guidelines recommend considering outpatient therapy for patients w

Beyond clinical complexity: managing risk within a respiratory virtual ward.

BACKGROUND: Community-based models of care, including respiratory virtual wards and Hospital at Home services, continue to expand across the NHS as part of wider efforts to reduce hospital admissions and deliver acute care closer to home. Alongside increasing clinical acuity, community clinicians are increasingly caring for patients with significant psychosocial complexity, including substance use, safeguarding concerns, unstable housing, poor engagement with treatment and environmental risks. While national policy has focused on expanding community-based models of care, comparatively little g

Alcohol in the workplace: an ethnographic study of professional bartender culture and risk management.

BACKGROUND: Alcohol consumption among bartenders is higher than in most other professions. However, the cultural and occupational meanings of drinking in this setting are underexplored. METHODS: Drawing on symbolic interactionism, this ethnographic study explores how bartenders interpret and regulate their alcohol use in their professional environment. Fieldwork was conducted in Marseille, France, in 2023 and included 22 in-depth semi-structured interviews and 320 h of participant observation with 38 bartenders working in 33 independent bars. Data were analysed inductively. RESULTS: Findings

Permanent Complications After Dermal Fillers: Risks, Prevention, and Management Strategies.

BACKGROUND: Dermal fillers are widely utilized in esthetic medicine for soft tissue augmentation and facial rejuvenation. Generally safe, complications are mostly minor. Permanent complications, though rare, are disastrous and overwhelmingly caused by insufficient understanding of facial anatomy, wrong filler selection, technique, and poor diagnosis and management. This review aims to bridge the knowledge gaps that lead to permanent complications, provide guidelines to avoid them, and create best practices to manage them should they occur. METHODS: A PRISMA-guided systematic search of PubMed

Self-Management Support for Cancer Survivors at Risk of Cardiotoxicity: A Scoping Review.

BACKGROUND: Cancer survivorship has significantly increased globally. This success has brought new challenges, including the rise in cardiovascular complications due to cardiotoxic effects of cancer treatments. Self-management (SM) support is increasingly recognised as an important component in overcoming these challenges; however, its implementation into practice remains inconsistent. AIM: This scoping review aims to map the current landscape of SM for cancer survivors at risk of or experiencing cardiotoxicity, with a focus on existing models of interdisciplinary collaboration. Anesthetic Management and Infection Risk in Hip and Knee Arthroplasty: A Narrative Review.

Surgical site infection (SSI) and periprosthetic joint infection (PJI) remain among the most serious complications after total hip and knee arthroplasty, potentially leading to delayed recovery, revision surgery, prolonged antimicrobial treatment, increased healthcare costs, and worse functional outcomes. This narrative review evaluates how perioperative anesthetic management may influence infection risk in patients undergoing hip and knee arthroplasty and summarizes practical measures that anesthesia providers can take to contribute to infection prevention. The review first compares general,

Risks and Management of Glucocorticoid Therapy for Patients with Rheumatic Disease Having Surgery.

PURPOSE OF REVIEW: The purpose of this review was to identify and characterize the diverse risks and benefits of glucocorticoid use in the perioperative period in patients with rheumatic diseases. RECENT FINDINGS: Glucocorticoids increase the risk of surgical site infection, wound dehiscence, pneumonia, unplanned intubation and readmissions. The risk of perioperative adrenal insufficiency is rare. Guidelines specifically for knee and hip arthroplasties do not recommend stress dosing of steroids. Continuation of home doses of glucocorticoids is adequate for most patients. Stress dosing of gluc

Management of intermediate- and high-risk pulmonary embolism-a case of a pregnant patient.

Acute pulmonary embolism (PE) has a heterogeneous clinical presentation, varying from mild chest symptoms to shock or cardiac arrest. Optimal management of severe acute PE requires assessment of multiple parameters to confirm the diagnosis and assess the patient's prognosis, preferably by EXPERT-PE care/Pulmonary Embolism Response Teams (PERTs), integrating multidisciplinary expertise to optimize PE management. While management of severe acute PE already can be challenging, this particularly applies to certain patient subgroups, in particular pregnant women. As a result of ruling out pregnant

Evaluation of Risk Factors for Failed Seroconversion in the Management of Potential Lyssavirus Exposures.

BACKGROUND: Although failure of rabies postexposure prophylaxis is rare, several factors, including improper vaccine or rabies immunoglobulin administration, may increase the risk. Where there is the potential for postexposure prophylaxis failure, public health units may recommend serology to confirm a therapeutic immune response. This study evaluates which factors increase the risk of failed seroconversion in cases where serology was indicated after rabies postexposure prophylaxis. METHODS: Potential lyssavirus exposure notifications where serology was collected were identified from 5 Southe

[Advances in the Growth Risk Assessment and Precision Management
of Pulmonary Subsolid Nodules].

Pulmonary subsolid nodules (SSNs) exhibit heterogeneous growth patterns. Most SSNs remain stable or grow only slowly during long-term follow-up, whereas a small subset grows rapidly over a relatively short period. Some of these lesions may ultimately progress to invasive adenocarcinoma (IAC). Growth is an important imaging manifestation of evolving biological behavior in SSNs. It also serves as a key determinant of surveillance strategies and the timing of intervention. Accordingly, accurate assessment of growth risk has become a major focus of current research. Starting from the natural histo

Managing Constipation and Its Risks in Parkinson's Disease: Is There a Role for Dietary Fiber?

A common consequence of Parkinson's disease (PD) is constipation. Beyond its deleterious effect on the quality of life in affected persons, constipation is associated with gut microbiota dysbiosis and microbial-generated inflammatory metabolites, which may further disease progression. Management strategies are needed, and diet, specifically higher fiber dietary patterns or fiber supplements, may be recommended. However, therapeutic effectiveness and potential health benefits have been underexplored, and risks may be unappreciated. This perspective addresses whether high-fiber diets and fiber s

Taiwan Consensus for the Management of Cardiovascular Risks and Complications in Patients with Prostate Cancer.

Prostate cancer (PC) is one of the leading causes of cancer-related deaths among men globally, and it frequently coexists with cardiovascular disease (CVD). With increasing cancer survival rates and an aging population, the incidence of CVD among PC patients is also on the rise. The treatments for PC include androgen deprivation therapy, androgen receptor pathway inhibitors, and docetaxel-based chemotherapy, and the cardiovascular complications include hypertension, metabolic syndrome, heart failure and arrhythmias. As it is important to provide personalized treatment planning and close monito

Risk Management of Valproate and Other Teratogenic Anticonvulsants in the Era of Proliferating Use.

Valproic acid, carbamazepine and topiramate have well-known teratogenic risk and all 3 rank among the top 10 teratogenic medications with the highest prenatal exposure risk. Importantly, pregnancies exposed to valproic acid are not dominated by patients with epilepsy but rather with less serious conditions such as migraine. In the United States, only a weight loss combination product containing topiramate has a mandatory pregnancy prevention program, a so-called Risk Evaluation and Mitigation Strategy (REMS), while prevention of fetal exposure to all three single ingredient products relies on
