Fifteen patients presented during a four-year period with a pseudothrombophlebitis syndrome, subsequently confirmed arthrographically as being due to popliteal cyst rupture. Initially the majority of patients (73%) were erroneously diagnosed as having calf vein thrombosis, and were anticoagulated for periods of three to ten days. Clinical differentiation from venous thrombosis was usually impossible. Contrast arthrography was the definitive diagnostic investigation, revealing popliteal cysts in all patients, and an active synovial leak into the calf in 13 cases (87%). The mean delay in perform…

Subdural hematomas are to be expected in about 3% of ruptured intracranial aneurysms. One third of these bleedings causes rapid deterioration as do subdural hematomas of traumatic origin. Otherwise they will show a prolonged or asymptomatic course. On hand of 8 own cases and 51 comparable cases of literature the symptomatology, diagnostic difficulties and the indication for surgery are pointed out. We could show that in more than 70% subdural hematomas in association with bleeding aneurysm are due to aneurysms of the internal carotid artery or the middle cerebral artery. ICA aneurysms are most…

Rupture of the urinary bladder without a history of recent trauma or instrumentation is rare; it is usually associated with bladder disease or outlet obstruction. The patient is usually seen as an emergency case and has an atypical clinical picture. The four patients in this report were initially seen and treated by a general surgeon; in only two was the diagnosis established preoperatively and confirmed by retrograde cystography. Awareness on the part of the consulting surgeon that bladder rupture is possible in a predisposed patient may lead to a correct preoperative diagnosis. In this serie…

Spontaneous rupture of the common bile duct is rare, but may result from increased intraductal pressure with associated calculous erosion and necrosis of the bile duct wall secondary to thrombosis. Two cases are reported; one clearly related to calculous erosion and the other unexplained. Survival depends on early diagnosis. Aggressive supportive therapy, including Swan-Ganz catheter, intensive care unit monitoring, and appropriate antibiotics, are mandatory. Only with early operation to alleviate sepsis and decompression of the biliary system can better survival be expected.

Fifty-eight patients with chordal ruptures of the mitral valve were examined. The main echocardiographic signs of chordal ruptures of the anterior and posterior mitral cusp were described. The authors provide the results of the use of two-dimensional echocardiography for diagnosing chordal ruptures of the mitral valve depending on the etiological factor of chordal pathology (mesenchymal abnormalities, rheumatic fever, infective endocarditis, coronary heart disease).

A 23-year-old woman was involved in an automobile accident and sustained a double traumatic rupture of the aorta, at the isthmus distal to the left subclavian artery and at the base of the innominate artery. A successful repair was accomplished within 4 hours of the injury using profound hypothermia and total circulatory arrest. To our knowledge, this case represents the first successful surgical repair of a double traumatic rupture of the aorta. Followup at 2 years showed an excellent postoperative recovery and the patient gainfully employed.

A case of late rupture of an abdominal aortic aneurysm that had been successfully treated by surgically induced thrombosis is presented. After emergency surgery consisting of ligation of the neck of the aneurysm, the patient survived and is doing well. The literature on surgically induced thrombosis of abdominal aortic aneurysm is reviewed. It is stressed that complete thrombosis of aortic aneurysm induced by distal ligation does not preclude rupture, and if patients present with symptoms of expansion or rupture, they should undergo emergency surgery.

Preterm rupture of the membranes poses perhaps the greatest threat to perinatal survival. Unfortunately, the literature leaves us in a quandry as to the management of the problem. At the Medical University of South Carolina for the past 3 years, aspects of both active and passive management have been used. Vaginal pool phospholipid analysis, the use of corticosteroids whenever appropriate, and tocolysis (in an attempt to gain 48 hours) were used in 96 consecutive patients with premature rupture of the membranes between 26 and 36 weeks' gestation. Decisions about delivery were dictated by spont…

Uncomplicated colonic diverticula have been regarded as weak points in the bowel wall which have a predisposition to rupture during colonoscopy. We attempted to prove or disprove this assumption. Eleven segments of diverticula-containing sigmoid colon were insufflated via a colonoscope and the rupture pressure manometrically recorded. The mean +/- SEM pressure causing serosal tear was 202 +/- 15 mm Hg and mucosal rupture 226 +/- 14 mm Hg. No diverticular blowouts occurred. Intraluminal sigmoid pressures were measured manometrically in 15 patients with and 15 patients without colonic diverticul…

Prolapses of intervertebral disks with intradural localisation are extremely rare. Up to 1981, a total of 41 cases have been published in the world literature. A report is given on two further patients in whom intradural sequesters of intervertebral disks were found during the operation. In one patiente there were at the same time two total intrathecal sequesters from the intervertebral spaces L 3/4 and L 4/5. Until now, no reports have been published on such cases in the world literature.

Tracheobronchial rupture is a rare and serious complication of blunt chest trauma. The cause is often a traffic accident and the rupture may be associated with other, fatal injuries. Three cases were treated at our clinic in the period 1970-1982. In them, as in about 60% of all cases in the literature, the tracheobronchial injury was not recognized in the acute stage. The early clinical picture may vary, depending on the site and extent of the injury, from only minimal mediastinal emphysema to severe dyspnea with extensive subcutaneous and mediastinal emphysema or persistent pneumothorax. The …

Rupture of the extensor tendons is common in longstanding rheumatoid arthritis of the hand. The tendons commonly involved are those of the ring and little fingers and the common site of rupture is over the dorsal ulna and distal edge of the extensor retinaculum. We are reporting two cases of extensor tendon rupture over the metacarpal head of the middle finger due to prominent osteophytes.

A spontaneous rupture of the common carotid artery occurred in a 56-year-old, apparently healthy, man. The patient suffered a sudden, painful swelling on the left side of the neck and bulge of the tonsillar fossa simulating a parapharyngeal abscess. Emergency surgical exploration of the neck revealed a 5-mm rent in the common carotid artery 15 mm proximal from the bifurcation. Such abnormalities as aneurysm, thickness, or thinness of the arterial wall were not detected. The rent was closed by sutures during temporal ligation of the carotid artery. The duration of the temporal interruption of t…
