First experiences with a new adjustable plate for osteosynthesis of scaphoid nonunions.

BACKGROUND: Plate osteosynthesis of the scaphoid, as reported earlier by Ender, has lost its importance in the past few years, after Herbert's introduction of the simple and successful technique of screw osteosynthesis. Only in rare cases does one encounter failed healing or instability of the fragments. Even with a vascularized bone chip, it is not always possible to achieve consolidation. Particularly in these situations, poor interfragmentary stability seems to be the reason for failed healing. METHODS: Between January 2007 and August 2009, we treated 7 men and 4 women of mean age 37 years

Percutaneous fixation of scaphoid fractures.

The scaphoid proximal pole and waist fractures presented here were treated by a novel dorsal percutaneous technique with arthroscopic assist … ome articles document extraordinary rapid healing by standard radiographs; however, we caution that scaphoid bone healing cannot accurately be determined without CT scan. Percutaneous, arthroscopically assisted internal fixation by a dorsal approach may be considered in all acute scaphoid fractures selected for surgical fixation. The dorsal guidewire permits do …

[Osteosynthesis using a buttress plate--a new principle for stabilizing scaphoid pseudarthroses].

Deformity with palmar bone resorption can be observed in 20-50% of scaphoid pseudarthroses. A palmar wedge-shaped defect develops after repositioning and debridement of the pseudarthrosis. In the present paper 16 cases in which operations with cancellous bone grafting and palmar buttress plate osteosynthesis in addition were performed are presented. In all but 1, the pseudarthrosis united. All patients were pain-free at follow-up examination a minimum of 9 months (mean 28 months) only slightly reduced. Grip strength and mobility were after operation. The persisting pseudarthrosis necessitated

Radiographic clues for determining carpal instability and treatment protocol for scaphoid fractures.

The magnitude of carpal instability following scaphoid fracture is closely related to the fracture location. Middle-third fractures of the scaphoid are classified into B1 (distal oblique fractures) and B2 (complete waist fractures). Deciding preoperatively whether a fracture is B1 or B2 is clinically important, because several studies have revealed that B1 is more stable than B2. Dorsal intercalated segment instability deformity often develops in B2, creating a large, wedge-shaped bone defect, while minimal humpback deformity develops in B1, and the bone defect is much smaller, even after long

High-resolution bone scintigraphy of the adult wrist.

Bony anatomic landmarks of the wrist (e.g., pisiform, hook of hamate, radioulnar joint, and styloid processes of the radius and ulna) were routinely identified in 28 adult patients examined for wrist pain. With the wrists prone and immobilized, bone scintigrams were obtained for 500,000 counts with an asymmetric (133 to 161 keV) Tc-99m energy window and either a converging (best choice) or straight-bore, high-resolution collimator. High-resolution scintigraphy precisely localized degenerative joint disease (nine patients), scaphoid fractures (five), pisiform fracture (one), lunate avascular ne

Bipartite trapezium - rare congenital accessory bone: a case report of two bilateral cases among related patients and description of new accessory bone - os scaphocapitatum anterius.

BACKGROUND: Trapezium typically maintains consistent morphology, and its anatomical variants are rare. We present the first known familial cases of multiple accessory carpal bones, including bipartite trapezium, and a previously undescribed volar accessory carpal bone located between the scaphoid and capitate. CASE PRESENTATION: A 69-year-old woman was evaluated for a distal radius fracture. Further X-ray and CT imaging revealed a bilateral bipartite trapezium, bilateral os styloideum, os hamuli proprium all having smooth margins and regular cortico-medullar ratio. Cystic changes in the capit

Computed tomography osteoabsorptiometry alterations in stress distribution patterns through the wrist after radial shortening osteotomy for Kienböck disease.

PURPOSE: The distribution pattern of subchondral bone density is considered to closely reflect the stress distribution across a joint under physiological loading conditions. Our purpose was to determine alterations in the distribution pattern of subchondral bone density across the distal articular surfaces of the radius and the ulna in patients with Kienböck disease after radial shortening. METHODS: We collected preoperative and postoperative computed tomography (CT) image data from 7 wrists of 7 patients who had undergone radial shortening for Kienböck disease. We measured the distribution o

[Treatment of old scaphoid fracture with absorbable screw].

OBJECTIVE: To explore a new approach to treat old scaphoid fracture by grafting bones from distal radius and fixing absorbable screws. METHODS: From January 2002 to May 2007, 18 patients with old scaphoid fractures were treated by grafting bones from distal radius and fixing absorbable screws. All of the 18 patients (13 males and 5 females, aged from 17 to 41 years old) had the injury history of the palm of hand touching the ground. They felt tenderness on the anatomical snuff-box and tubercle of scaphoid bone. The grasping force decreased. The range of motion of the wrist was (36 +/- 2) degr…

Retrograde compression screw fixation of acute proximal pole scaphoid fractures.

Seventeen consecutive patients with acute unstable proximal pole scaphoid fractures were managed over the past 5 years with open reduction and internal fixation. Four fractures were displaced, with greater than 1 mm of fragment offset and intercarpal malalignment. The operative technique consisted of a dorsal approach to the scaphoid, radius bone grafting, and freehand retrograde Herbert compression screw fixation. The patients were evaluated at an average of 37 months (range, 12-63 months) after surgery. All fractures healed within 13 weeks (average, 10 weeks). Functional wrist range of motio

Arthroscopic treatment of scaphoid nonunion, a new algorithm after six years practice.

INTRODUCTION: Scaphoid nonunion is still a challenging problem in hand surgery. Till now most of the patients have been treated with open revision, bone grafting and internal stabilization. Arthroscopy plays an increasing role in hand surgery since the last decade. In this retrospective study, we want to present our results and current treatment protocol, adapted after midterm analysis. MATERIAL AND METHODS: In 2017, arthroscopic treatment of scaphoid nonunion was started at our department. Debridement is done arthroscopically as well as insertion of radius cancellous bone graft. Fixation has

Proximal row carpectomy combined with primary flexor pollicis longus tenorrhaphy: An effective treatment alternative for a rare case of flexor pollicis longus rupture due to scaphoid nonunion advanced collapse.

This study describes a case of flexor pollicis longus rupture resulting from long-term scaphoid nonunion advanced collapse. The tendon rupture mechanism was attrition due to sharp bone protuberances from the scaphoid in the carpal tunnel. Although this extremely rare complication has already been reported in the literature, our study is original in showing that proximal row carpectomy with consequent wrist shortening allowed primary tendon repair without transferring the flexor digitorum superficialis tendon of the ring finger or grafting the palmaris longus tendon, which besides using two-lev

Ununited proximal pole scaphoid fractures. Treatment with a Herbert screw in 16 cases followed for 0.5-8 years.

We treated 16 cases of delayed union and nonunion of proximal one-third scaphoid fractures with cancellous bone grafting and retrograde insertion of a Herbert screw through a dorsal approach. Definite radiographic union was obtained in 13 of 16 patients after a median of 2 (0.5-8) years of follow-up. Using Cooney's clinical scoring system, 5 cases were excellent, 5 good, 5 fair and 1 poor. The treatment of ununited proximal pole scaphoid fractures with retrograde insertion of the Herbert screw offers the advantages of a short period of immobilization and a good function.

Scaphoid malunion.

We reviewed 10 patients with symptomatic malunion of a carpal scaphoid fracture. All had displacement with dorsiflexed intercalated segment instability, and suffered from pain, restricted range of movement at the wrist and decreased grip strength. The restriction of flexion-extension and the decreased grip strength correlated with the severity of the DISI deformity. Seven patients had a corrective osteotomy, using an anterior wedge-shape bone graft with internal fixation by Herbert screw, and all had satisfactory results. We believe that symptoms associated with scaphoid malunion are related t

Scaphoid interfragmentary motions due to simulated transverse fracture and volar wedge osteotomy.

BACKGROUND: Our goal was to determine 3-dimensional interfragmentary motions due to simulated transverse fracture and volar wedge osteotomy of the scaphoid during physiologic flexion-extension of a cadaveric wrist model. METHODS: The model consisted of a cadaveric wrist (n = 8) from the metacarpals through the distal radius and ulna with load applied through the major flexor-extensor tendons. Flexibility tests in flexion-extension were performed in the following 3 test conditions: intact and following transverse fracture and wedge osteotomy of the scaphoid. Scaphoid interfragmentary motions w

Scaphoid Fracture Fixation in a Nonunion Model: A Biomechanical Study Comparing 3 Types of Fixation.

PURPOSE: Union of a scaphoid fracture after fixation is influenced by various factors, one of which is fracture stability. This study aims to compare the biomechanical stability of 3 different types of scaphoid fracture fixation in a scaphoid nonunion model. METHODS: Thirty cadaveric scaphoid specimens were assigned to one of 3 different fixation groups: (1) single 3.0-mm compression screw, (2) two 2.2-mm screws, and (3) scaphoid plate. A 3-mm volar wedge osteotomy was created at the scaphoid waist to simulate a nonunion model. The primary outcome measure was load to failure, whereas secondar
