Geometric morphometrics of different malocclusions in lateral skull radiographs.

BACKGROUND: To evaluate the role of craniofacial shape in malocclusion by application of geometric morphometrics to a set of two-dimensional landmarks and semilandmarks obtained from lateral skull radiographs. METHODS: Cephalometric radiograph tracings of 88 untreated Caucasians (age range 7-39 years) were assigned to four groups according to their occlusion: neutrocclusion, distocclusion, mesiocclusion, and anterior open bite. The geometric morphometric shape analysis incorporated 66 landmarks and semilandmarks, which underwent generalized Procrustes analysis, between-groups principal compon…

Strategies for Improving Visualization During Endoscopic Skull Base Surgery.

The nasal cavity has a robust vascular supply, and bleeding is a primary obstacle to the minimally invasive skull base technique. Venous bleeding, including the cavernous sinus, can be managed with various techniques using hemostatic materials and pressure. A thorough understanding of the skull base vascular anatomy is vital for avoiding injury to major arteries and having confidence to control venous bleeding to optimize the endoscopic view and tumor resection.

Imaging in Endoscopic Cranial Skull Base and Pituitary Surgery.

Endoscopic endonasal approaches have widely accepted techniques for managing benign and malignant processes along the entire ventral skull base with similar or better results compared with open procedures, but with lower rates of complication. Managing pathology affecting the skull base can be challenging because of complex anatomy and the proximity of critical neurovascular structures. Postoperative imaging can be challenging, because of surgical alterations of normal anatomy and the now common use of complex reconstruction techniques. Understanding the normal imaging appearance of skull base…

[Oculorrhea after facial fracture and anterior skull base fracture].

Leakage of cerebrospinal fluid (CSF) from a cranio-orbital fistula, i.e. oculorrhea, is rare after skull trauma with fewer than 30 patients reported in Western literature. Here, we for the first time report a Swedish patient. A 75-year-old male sustained a mid-facial injury due to a bicycle accident. The CT-scan showed, among other things, a fracture in the orbital roof, involving lamina cribrosa and lamina papyracea. The patient had epiphora and laboratory analysis showed CSF leakage. This diagnosis should be kept in mind when excessive tearing appears in mid-facial trauma and treatment shoul…

Human Temporal Bone Removal: The Skull Base Block Method.

Objectives To describe a technique for harvesting larger temporal bone specimens from human cadavers for the training of otolaryngology residents and fellows on the various approaches to the lateral and posterolateral skull base. Design Human cadaveric anatomical study. The calvarium was excised 6 cm above the superior aspect of the ear canal. The brain and cerebellum were carefully removed, and the cranial nerves were cut sharply. Two bony cuts were performed, one in the midsagittal plane and the other in the coronal plane at the level of the optic foramen. Setting Medical school anatomy labo…

Multimodal Treatment of Skull Base Inflammatory Pseudotumor: Case Report.

lnflammatory pseudotumor (IPT) is a rare, non-neoplastic inflammatory process. It is most commonly occurs in the orbit, but extension into brain parenchyma is uncommon. In a confirmed case of IPT, most cases show good improvement with steroid theraphy. A 50-year-old man with progressive left-eye visual disturbance and mass lesion was admitted in a hospital. A left orbital mass biopsy revealed what was highly suspected as an inflammatory pseudotumor. Steroid pulse therapy with dexamethasone, radiation therapy, and chemotherapy with amphotericin B were performed, but they were not effective in i

Giant “Panfacial” Chordoma Involving Skull Base With Intracranial Invasion.

Chordomas are rare, low-grade, primary malignant bone tumor arising from primitive notochord remnants of the axial skeleton. They rarely occur in an extraosseus craniofacial location such as nasopharynx, nasal septum, paranasal sinuses, and orbits. We report here an extreme case of "panfacial" chordoma involving middle and anterior skull base with intracranial invasion, which reached considerable size because of a delay in diagnosis and treatment. In this case, a wide surgical excision was performed to improve quality of life.

Perineural tumor spread involving the central skull base region.

Perineural spread of tumor is defined as extension of the primary tumor along tissues of the neural sheath (epineurium and perineurium) of a named nerve. Given the density of cranial nerves in the central skull base region and their extracranial communications, perineural tumor spread from a variety of sources can affect the central skull base region. Common malignancies with perineural tumor spread to central skull base include mucosal squamous cell carcinoma, adenoid cystic carcinoma, and cutaneous malignancies including melanoma. The presence and extent of tumor spread influence selection o…

Skull parosteal lipoma with reactive hyperostosis: a case report.

A 50-year-old female presented with more than 20-year history of a large subcutaneous mass in the left parieto-occipital portion. Magnetic resonance (MR) imaging revealed the lipomatous mass to show a high signal intensity in both T1- and T2-weighted images. A part of the lipomatous lesion progressed into the underlying hyperostosis and skull. The preoperative diagnosis was skull invasion of a well-differentiated liposarcoma. The tumor was removed completely, including the underlying hyperostosis and skull. Microscopy confirmed a lipoma without any lipoblasts, which was firmly attached to the …

Mastoid pneumocoele with skull base and cervical spine pneumatisation.

Very uncommonly, Eustachian tube dysfunction can lead to symptomatic chronic elevation of middle ear pressure with aeration of bone and adjacent structures in the skull base and upper cervical spine, and an increased risk of fracture. We present a case demonstrating the natural history of this condition over 11 years before treatment and 10 months after treatment.

Biomechanical analysis of skull fractures after uncontrolled hanging release.

In forensic research, biomechanical analyses of falls are widely reported. However, no study on falls consecutive to uncontrolled hanging release, when a hanging body is cut down, has ever been published. In such cases, the presence of cranial trauma can raise interpretation issues, and there may be doubt as to whether the fall was an accident or a crime disguised as suicide. The problem remains as to whether or not a fall after a free hanging release can lead to a skull fracture. To address this question, numerical simulations, post-mortem human subject tests and parametric studies were perfo…

Skull base injury leading to death after conventional septoplasty.

Treatment of nasal obstruction with conventional septoplasty is a very common operation in otorhinolaryngology. There are considerable complications that may become even life threatening. We report a rare complication of septoplasty, namely, skull base injury. The possible mechanisms and treatment modality are discussed. Like other similar procedures, prevention of the skull base injury is more important than its treatment. Correct knowledge about anatomy of the region and meticulous manipulation and instrumentation are the most considerable topics.

Late-onset en coup de sabre of the skull.

En coup de sabre is a rare subtype of linear scleroderma that characteristically affects the skin, underlying muscle, and bone of the frontoparietal region of the face and scalp. It typically presents in the first two decades of life, and may be associated with focal neurological deficits. We present a case of late-onset en coup de sabre of the frontal bone where the diagnosis was further complicated by a history of breast cancer, prior trauma to the region, and use of topical medication.

Allergic fungal rhinosinusitis infiltrating anterior skull base and clivus.

Bone erosion and skull base invasion are often suggestive of a malignant mass in paranasal and nasal cavities. Nevertheless, forms of chronic rhinosinusitis, such as allergic fungal rhinosinusitis (AFRS), could mimic malignant features. Here, we report AFRS patient with orbital, anterior cranial fossa, Turkish saddle and clivus erosion. A 48-year-old Caucasian female with history of drug-resistant headache, nasal obstruction and anosmia was referred to our institution. Imaging showed hyperdense featureless tissue with signs of medial orbital wall, cribiform lamina and clivus erosions and encas…

Squamosal suture synostosis: a cause of atypical skull asymmetry.

BACKGROUND: The squamosal suture is markedly different from the major calvarial sutures of the human skull. The unique properties of the suture are a result of the complex developmental biology of the temporal bone and biomechanical force exerted by surrounding structures. The dysmorphic effects of premature fusion of the suture, and possible treatment strategies in cases of synostosis, have received only brief description in the literature. METHODS: A retrospective case series was performed. The study included patients evaluated by one of the senior authors (S.P.B., R.R.R., and D.J.S.) betwe…
