Conservative treatment of adolescent idiopathic scoliosis: the effectiveness of rigid bracing.

BACKGROUND: The use of rigid braces and specialized exercises for the treatment of adolescent idiopathic scoliosis (AIS), is the most common non-surgical approach used to prevent curvature progression. The study aims to assess the efficacy of a rigid brace (the Chêneau brace), in conjunction with SEAS (Scientific Exercise Approach to Scoliosis), as a conservative approach to the treatment of AIS. METHODS: The study involves a retrospective analysis of data collected prospectively from 119 patients with AIS who underwent treatment with the Chêneau brace and SEAS. Patients with AIS were eligibl

Perinatal factors associate with vertebral size and shape but not lumbar lordosis in 10-year-old children.

The intrauterine environment is known to influence foetal development and future health. Low birthweight has been linked to smaller vertebral canals in children and decreased adulthood spine bone mineral content. Perinatal factors affecting lumbar spine curvature have not yet been considered but could be important for adult spinal health, as lumbar movement during lifting, a risk factor for backpain, is associated with lordosis. To investigate this, lumbar spine magnetic resonance images at age 10 years and perinatal and maternal data (birthweight, placental weight, gestation length, crown-hee

[Respiratory treatments in neuromuscular disease].

In a previous article, a review was presented of the respiratory pathophysiology of the patient with neuromuscular disease, as well as their clinical evaluation and the major complications causing pulmonary deterioration. This article presents the respiratory treatments required to preserve lung function in neuromuscular disease as long as possible, as well as in special situations (respiratory infections, spinal curvature surgery, etc.). Special emphasis is made on the use of non-invasive ventilation, which is changing the natural history of many of these diseases. The increase in survival an

[Kyphosis and kyphotic posture in children and adolescents].

Kyphosis is a sagittal deviation of the spine that presents as an anterior spinal curvature. It is a frequent reason for consultation during adolescence. It is necessary to know how to differentiate between a simple postural kyphosis that is flexible and reducible, and requires no further investigation, from a true kyphosis that requires at minimum a radiological evaluation with films of the entire spine, in AP and lateral views. If vertebral epiphysitis is most often at the origins of the sagittal vertebral deviations encountered in daily practice, the other causes pose diagnostic and therape

Skeletal overgrowth and deafness in mice lacking fibroblast growth factor receptor 3.

Fibroblast growth factor receptor 3 (Fgfr3) is a tyrosine kinase receptor expressed in developing bone, cochlea, brain and spinal cord. Achondroplasia, the most common genetic form of dwarfism, is caused by mutations in FGFR3. Here we show that mice homozygous for a targeted disruption of Fgfr3 exhibit skeletal and inner ear defects. Skeletal defects include kyphosis, scoliosis, crooked tails and curvature and overgrowth of long bones and vertebrae. Contrasts between the skeletal phenotype and achondroplasia suggest that activation of FGFR3 causes achondroplasia. Inner ear defects include fail

Report on two cases of Freeman-Sheldon syndrome ("whistling face).

The Freeman-Sheldon syndrome is a rare hereditary disorder that was described by these authors in 1938 under the name of "cranio-carpotarsal dystrophia". It is characterised by a combination of congenital malformations of which the principal are: deformities of the face and skull, flexion and ulnar deviation of the fingers and talipes. The appearance of the face is the most striking feature, the small dimension of the oral fissure giving rise to the term "whistling face" proposed by Burian (1963). Spinal curvature is often present, and can show marked progression, as in one of our cases. The c

Spinal Sagittal Alignment Changes During Childhood: Results of a National Cohort Analysis of 1,059 Healthy Children.

BACKGROUND: In the past decades, it has been recognized that sagittal alignment of the spine is crucial. Although the evolution of spinal alignment with growth has previously been described, there are no data for key parameters such as the exact shapes (extent and magnitude) of spinal curvatures. The goals of this study were therefore to determine normative values of spinopelvic sagittal parameters and to explore their variation during growth, based on the analysis of a large national cohort of healthy children. METHODS: The radiographic data of 1,059 healthy children were analyzed in a retro…

Scoliosis in children with juvenile rheumatoid arthritis.

Juvenile rheumatoid arthritis (JRA) is an unusual causative factor of scoliosis in childhood. Among 320 patients with JRA in Sweden, 17 (5.3%) had structural scoliosis of thoracic and lumbar spine. This frequency is considerably higher than in the normal population. The mean age when scoliosis was first noted was 12.4 years, corresponding to a mean duration of the JRA of 7.4 years. A curvature exceeding 20 degrees was found in 12 out of the 17 patients. The scoliosis in our patients was associated with severe and protracted polyarthritis of early onset and involvement of hip joints and cervica

An Integrated Rigid-Flexible Body Dynamic Approach to Computationally Efficient Musculoskeletal Modeling and Muscle Recruitment Simulation of the Lumbosacral Spine and Torso.

OBJECTIVE: In silico biomechanical models of the spine traditionally follow either rigid body dynamic (RBD) modeling (multibody modeling) or finite element (FE) modeling techniques. While RBD models lack robust representation for flexible tissues, FE models are computationally expensive. This study proposes an integrated rigid-flexible body dynamic (RFBD) architecture to address these limitations, and develops a full-torso human model, focusing spinal mechanical stability. METHODS: The model consisted of L1-L5 lumbar vertebrae, pelvis, sacrum, a lumped thoracic spine with ribcage as rigid bod

On growth and scoliosis.

PURPOSE: To describe the physiology of spinal growth in patients with adolescent idiopathic scoliosis (AIS). METHODS: Narrative review of the literature with a focus on mechanisms of growth. RESULTS: In his landmark publication On Growth and Form, D'Arcy Thompson wrote that the anatomy of an organism reflects the forces it is subjected to. This means that mechanical forces underlie the shape of tissues, organs and organisms, whether healthy or diseased. AIS is called idiopathic because the underlying cause of the deformation is unknown, although many factors are  associated. Eventually, howe

The correlation between spinal and chest wall deformities and pulmonary function in Marfan syndrome.

PURPOSE: Scoliosis, chest wall deformities and pulmonary involvement are common features of Marfan syndrome (MFS). We aimed to assess the impact of spinal and chest wall deformities on pulmonary function in paediatric MFS patients with a surgically managed spinal deformity. METHODS: In this multicentre retrospective study, spirometry, lung volumes and radiographic imaging were performed on 26 MFS patients between the ages of seven and 18 years who were undergoing planned spinal fusion surgery for spinal deformity. A correlation analysis assessed the relationship between radiographic measureme

Limited laminectomy and foraminal decompression combined with internal fixation for treating multi-segment cervical spondylotic myelopathy: Does it effectively improve neurological function and prevent C5 palsy?

Laminectomy is an effective surgical treatment for multi-segment cervical spondylotic myelopathy (M-CSM) but usually results in C5 palsy. Some surgical techniques to restore the spinal sequence, increase the intervertebral foramen diameter, and limit the spinal cord drift distance have been proposed; however, it is unclear whether these procedures can avoid this complication. To investigate the clinical efficacy of limited laminectomy and foraminal decompression with fixation (LLFDF) for improving neurological recovery and preventing C5 palsy.A total of 71 patients with M-CSM were retrospective

Scoliosis and Kyphosis Prevalence in Turner Syndrome: A Retrospective Review at a Pediatric Tertiary Care Medical Center.

BACKGROUND: The prevalence of major coronal and sagittal spinal curves (scoliosis and kyphosis) in Turner syndrome (TS) is not well established due to limited reporting. The relationship between growth hormone (GH) therapy and its effect on TS spinal curve incidence is also not well established. METHODS: A retrospective chart review of 306 TS patients from 2007 to 2021 evaluated major coronal and sagittal spinal curves, progression of the curve, and treatment with GH. Statistical significance (defined as P <0.05) between curvature rates and curve progression was compared between GH-treated pa

The Influence of Bilateral and Unilateral Flatfoot on Coronal Spinopelvic Alignment in Asymptomatic Young Healthy Males.

BACKGROUND: This cross-sectional retrospective study aimed to present the influence of unilateral and bilateral flatfoot on coronal spinopelvic alignment in asymptomatic young healthy males. METHODS: This study was performed by examining the medical reports of individuals who applied to the National Health Board to work in positions requiring physical fitness between January 1, 2018, and January 1, 2019. Plain radiographs of the feet, pelvis, and spine were analyzed. The calcaneal pitch angle for flatfoot, pelvic obliquity, and the Cobb angle for spinal asymmetry were measured. After all of t

IoT-Based Solution for Detecting and Monitoring Upper Crossed Syndrome.

A sedentary lifestyle has caused adults to spend more than 6 h seated, which has led to inactivity and spinal issues. This context underscores the growing sedentary behavior, exemplified by extended sitting hours among adults and university students. Such inactivity triggers various health problems and spinal disorders, notably Upper Crossed Syndrome (UCS) and its association with thoracic kyphosis, which can cause severe spinal curvature and related complications. Traditional detection involves clinical assessments and corrective exercises; however, this work proposes a multi-layered system f

Anterior cervical corpectomy decompression and fusion for cervical kyphosis in a girl with Ehlers-Danlos syndrome: A case report.

BACKGROUND: Spinal deformities in Ehlers-Danlos syndrome (EDS; type VI) are generally progressive and severe. Surgical treatment has been described for kyphoscoliosis in the thoracolumbar spine. However, there are few studies describing the consequences of an anterior approach in cervical kyphosis. An anterior approach may not be able to fully decompress the spinal canal and restore the normal curvature of the cervical spine. Therefore, the anterior approach for cervical kyphosis in young children is hard. We describe the first case in an EDS girl with cervical kyphosis who received satisfacto

Asymmetrical intrapleural pressure distribution: a cause for scoliosis? A computational analysis.

PURPOSE: The mechanical link between the pleural physiology and the development of scoliosis is still unresolved. The intrapleural pressure (IPP) which is distributed across the inner chest wall has yet been widely neglected in etiology debates. With this study, we attempted to investigate the mechanical influence of the IPP distribution on the shape of the spinal curvature. METHODS: A finite element model of pleura, chest and spine was created based on CT data of a patient with no visual deformities. Different IPP distributions at a static end of expiration condition were investigated, such

Healthy morphological variability in sagittal alignment modulates loading patterns in musculoskeletal spine modelling.

Spinal sagittal alignment (SSA) is a primary determinant of spinal loading and stability. While biomechanical research frequently addresses major spinal deformities, the impact of healthy morphological variability remains underrepresented in musculoskeletal modelling, as conventional models typically assume a generic, "one-size-fits-all" curvature for non-deformed alignments. Using SSA4MSK tool, an open-source standalone Matlab application here introduced for the automatic integration of subject-specific SSA into OpenSim musculoskeletal models, this study quantifies the bias in spinal load est

Experience with combined video-assisted thoracoscopic surgery (VATS) anterior spinal release and posterior spinal fusion in Scheuermann's kyphosis.

STUDY DESIGN: Retrospective. OBJECTIVES: To determine whether anterior endoscopic release and posterior spinal fusion could achieve stable correction in Scheuermann's kyphosis. SUMMARY OF BACKGROUND DATA: The initial treatment of choice of Scheuermann's kyphosis is thoracic hyperextension and postural exercises and/or Milwaukee brace. Milwaukee bracing is most efficacious in the early stages when the curvature is flexible and in the skeletally immature. However, it is known that larger curves, vertebral wedging greater than 10 degrees, and skeletally mature patients will not usually respond
