The extrapharyngeal approach to the anterior cervical spine is a safe, rapid surgical exposure. Other surgical exposures such as the posterior, lateral, and intraoral (transpharyngeal) have inherent limitations that this approach avoids. Forty subjects were examined to determine the accuracy and clinical usefulness of nuclear magnetic resonance (NMR) examination of the spine. The authors analyze 14 cases of acute hematogenic osteomyelitis of the vertebral column in children. Three variations of the clinical picture are described: meningitis-like, local and lightening types. Roentgenological symptoms were late to appear and certain patients might have no symptoms at all. Formation of purulent tuberculous abscesses near cartilaginous ends of ribs were observed and considered to be a characteristic feature of osteomyelitis of thoracic vertebrae in infants. A case of fracture dislocation of the dorsolumbar junction together with a dislocation of the cervical spine, complicated by a head injury, is presented. The patient escaped any neurological damage. It is stressed that unstable spinal injuries often present without any leading neurological signs. A high index of suspicion and routine clinical and radiological examination of the entire spine in all patients with head and neck injuries is essential to prevent the tragedy of permanent spinal cord injuries.

Spondylolysis and absence of the pedicle are congenital anomalies of the posterior cervical spine. Their roentgenographic changes may be confused with other more serious entities which may necessitate either emergent therapy or require extensive diagnostic testing and treatment. Four cases are present and the literature is reviewed. A hypothesis for the embryologic etiology of these entities is proposed.

Lumbar vertebral columns from 73 boars were examined for pathological changes. While pathological changes were minimal at five months of age, several animals aged seven to 18 months of age had facet asymmetry and large osteophytes surrounding lumbar facets. Vertebral osteomyelitis is a frequent consequence of disseminated coccidioidomycosis. In a 25-year-old man, the sequela was a single level unilateral osseous bridge between the second and third lumbar vertebra. A review of previous cases has disclosed one case with a similar roentgenographic picture. The occurrence of a single level unilateral reactive process should arouse suspicion of a localized chronic bone infection.

Four cases of tuberculosis of the cervical spine with quadriparesis either from subluxation, and/or compression of the spinal cord are described. Our surgical approach has been laminectomy with excision of abscess and cervical fusion. Pre and post operative cervical traction with Crutchfield tongs on Stryker beds was employed in three of the four cases. Excision of these abscesses and cervical fusion, following improvement of the nutritional status and adjunctive chemotherapy, can reverse paralysis in these patients. Three of our four patients recovered.

A patient with a fracture of an ankylosed thoracic spine was treated initially along conservative lines. Subsequently the fracture displaced, causing paraplegia. Internal fixation proved difficult but stability was obtained by wiring two Sherman plates to the posterior elements. This succeeded in holding the fracture until the death of the patient 13 days postoperatively. The difficulties in the management of these fractures are discussed.

Radionuclide bone scans provide a sensitive, no-risk diagnostic technique for the detection and evaluation of a wide variety of diseases of the spine. Scans and radiographs provide complementary diagnostic information, and should both be considered in the medical imaging portion of the diagnostic plan.
