Targeting the Energy-Coupling Factor Transporters: A Novel Antibiotic Drug Target in Streptococcus pneumoniae.

Energy-Coupling Factor (ECF) transporters are a hitherto underexplored target involved in the uptake of several micronutrients in bacteria and are absent in humans. Here, in Streptococcus pneumoniae, we demonstrate that the genes encoding ECF transporters are highly conserved and their expression is crucial for causing bacterial infection in both murine (in vivo) and human (ex vivo) infection models. Next, we demonstrated that the antimicrobial activity of our inhibitors against S. pneumoniae is related to the level of the ECF transporter expressed by the bacterium, confirming the target engag…

Antibiotic resistance patterns and resistance-associated genes in Streptococcus pneumoniae: insights from healthy children.

BACKGROUND: The emergence of resistance to β-lactam and macrolide antibiotics, the main therapeutic agents for Streptococcus pneumoniae infections, poses a major public health challenge. This study aimed to investigate antibiotic resistance patterns and characterize the resistance-associated genes in pneumococcal isolates. METHODS: Nasopharyngeal swab specimens were collected from 318 healthy children younger than 16 years of age. S. pneumoniae isolates were identified using conventional culture, Gram staining, and biochemical tests. Antimicrobial susceptibility was determined by the disk dif…

Streptococcus pneumoniae adaptation to nutrient deprivation and immune modulation drives upper respiratory tract colonization.

Streptococcus pneumoniae is a successful colonizer of the human upper respiratory tract; however, the mechanisms that enable its persistence in this nutrient-limited environment, with numerous immune mechanisms in place, remain enigmatic. Here, we examined how pneumococci adapt to upper respiratory tract conditions and how this affects host interactions. We measured intranasal metal ion and monosaccharide concentrations to create an in vivo-mimicking medium for studying pneumococcal adaptation. Growth in this medium was reduced compared to glucose-rich chemically defined media (CDM). Proteome …

Impact of Hyperglycemia on Streptococcus pneumoniae Virulence: Insights into Capsular Dynamics and Clinical Implications.

Hyperglycemia, a diabetic condition, profoundly affects the pathogenicity of Streptococcus pneumoniae ( S. pneumoniae ), a leading etiologic pathogen of pneumonia. This review addresses a systematic understanding of how hyperglycemia affects the pathogenicity of S. pneumoniae , especially by immune dysfunction, cytokine dysregulation, and oxidative stress. Although the length of the capsular polysaccharide (CPS) chain plays a role in immune evasion and survival of bacteria, hyperglycemia also affects host immune responses, leading to increased severity of infection in diabetic patients. CPS le…

SARS-CoV-2 and Streptococcus pneumoniae colonization and disease: an observational study in adults.

INTRODUCTION: The COVID-19 pandemic has impacted global health and altered respiratory pathogens. While SARS-CoV-2 vaccines have mitigated COVID-19 severity, emerging variants remain challenging. Co-infection of Streptococcus pneumoniae with respiratory viruses is associated with increased disease severity, but its relationship with SARS-CoV-2 remains unclear. This study aims to analyze their co-occurrence, focusing on disease progression, colonization rates and clinical outcomes. METHODS: To this end, three approaches were used. First, a laboratory-based analysis of invasive pneumococcal dis…

Antimicrobial Resistance of Streptococcus pneumoniae Clinical Serotypes between 2017 and 2022 in Crete, Greece.

BACKGROUND: Pneumococcal disease is still considered a global problem. With the introduction of pneumococcal conjugate vaccines (PCVs) serotype epidemiology changed, but antimicrobial resistance persists constituting a serious problem. The current study aimed to determine the serotype distribution and the antimicrobial susceptibility of recent Streptococcus pneumoniae isolates, following implementation of the 13-valent conjugate vaccine (PCV13). MATERIALS AND METHODS: From January 2017 to December 2022 we evaluated 116 nonduplicate S. pneumoniae isolates collected from adult patients (21 - 98…

Deciphering structural variation upon biotinylation of biotin carboxyl carrier protein domain in Streptococcus pneumoniae.

Streptococcus pneumoniae is a leading cause of community-acquired pneumonia and is responsible for acute invasive and non-invasive infections. Fight against pneumococcus is currently hampered by insufficient vaccine coverage and rising antimicrobial resistance, making the research necessary on novel drug targets. High-throughput mutagenesis has shown that acetyl-CoA carboxylase (ACC) is an essential enzyme in S. pneumoniae which converts acetyl-CoA to malonyl-CoA, a key step in fatty acid biosynthesis. ACC has four subunits; Biotin carboxyl carrier protein (BCCP), Biotin carboxylase (BC), Carb…

Invasive disease caused simultaneously by two different serotypes of Streptococcus pneumoniae: a microbiological appreciation.

We report a clinical case of a child with an invasive pneumococcal disease caused by two different pneumococcal serotypes that belonged to different sequence types. She was a 15-month-old girl with pneumonia and pleural effusion in which S. pneumoniae colonies with different morphologies grew, one from the blood culture (characteristic greyish appearance) and the other from the pleural fluid (mucoid appearance). The isolate from blood was serotype 22 F (ST698/CC698/GPSC61), while the isolate from the pleural fluid was serotype 3 (ST180/CC180/GPSC12). The patient fully recovered after treatment

Bacteriological characteristics and changes of Streptococcus pneumoniae serotype 35B after vaccine implementation in Japan.

Streptococcus pneumoniae serotype 35B, a non-vaccine type, is a major contributor to the increase in pneumococcal infection post-vaccination. We aimed to understand the mechanism of its spread by characterizing 35B. The serotype, type 1 pilus (T1P) positivity, and antimicrobial susceptibility of 319 isolates in 2018-2022 were analysed and compared with those of isolates in 2014-2017 to find the changes. 35B accounted for 40 (12.5%) isolates. T1P positivity was notably higher in 35B (87.5%) than in the other serotypes. To confirm the role of T1P, an adhesion factor, we compared adherence to A54…

Epidural abscess secondary to Streptococcus pneumoniae. A case report and review of the literature.

While spinal epidural abscess is a well described disease process, this condition is rarely caused by Streptococcus pneumoniae. This case describes a case of spinal epidural abscess secondary to S. pneumoniae in an otherwise healthy, immunocompetent 61-year-old female without a history of spinal procedures, obvious source of hematogenous seeding, or clear risk factors for invasive pneumococcal infection. She was treated with IV and oral antibiotic therapy and made a full recovery.

Intracellular survival of Streptococcus pneumoniae in human alveolar macrophages is augmented with HIV infection.

People Living with HIV (PLHIV) are at an increased risk of pneumococcal pneumonia than HIV-uninfected adults, but the reasons for this are still not well understood. We investigated whether alveolar macrophages (AM) mediated control of pneumococcal infection is impaired in PLHIV compared to HIV-uninfected adults. We assessed anti-bactericidal activity against Streptococcus pneumoniae of primary human AM obtained from PLHIV and HIV-uninfected adults. We found that pneumococcus survived intracellularly in AMs at least 24 hours post ex vivo infection, and this was more frequent in PLHIV than HIV-…
