Intraarterial transplantation of human umbilical cord blood mononuclear cells in hyperacute stroke improves vascular function.

BACKGROUND: Human umbilical cord blood (hUCB) cell therapy is a promising treatment for ischemic stroke. The effects of hyperacute stem cell transplantation on cerebrovascular function in ischemic stroke are, however, not well understood. This study evaluated the effects of hyperacute intraarterial transplantation of hUCB mononuclear cells (MNCs) on cerebrovascular function in stroke rats using serial magnetic resonance imaging (MRI). METHODS: HUCB MNCs or vehicle were administered to stroke rats via the internal carotid artery immediately after reperfusion at 60 min following ischemia onset.

Stroke increases neural stem cells and angiogenesis in the neurogenic niche of the adult mouse.

The unique cellular and vascular architecture of the adult ventricular-subventricular zone (V/SVZ) neurogenic niche plays an important role in regulating neural stem cell function. However, the in vivo identification of neural stem cells and their relationship to blood vessels within this niche in response to stroke remain largely unknown. Using whole-mount preparation of the lateral ventricle wall, we examined the architecture of neural stem cells and blood vessels in the V/SVZ of adult mouse over the course of 3 months after onset of focal cerebral ischemia. Stroke substantially increased th

Hemodynamic comparison of albumin and hydroxyethyl starch in postoperative cardiac surgery patients.

The hemodynamic effects of 2 plasma volume expanders were compared in postoperative open heart surgery patients. Albumin 5% (A) or hydroxyethyl starch 6% (HES) solutions were infused according to indications based on cardiac index (CI) and pulmonary wedge pressure (WP), and their effects evaluated by physiologic profile measurements. Both groups demonstrated significant increases with volume infusion in CI (A from 2.37 to 2.84; HES from 1.97 to 2.49 L/min X m2) and WP (A from 9.4 to 13.7 mm Hg; HES from 11.9 to 13.2 mm Hg). Stroke index and stroke work increased similarly. Mean systemic arteri

Patent Foramen Ovale Closure Decreases the Incidence but Not the Size of New Brain Infarction on Magnetic Resonance Imaging: An Analysis of the REDUCE Trial.

BACKGROUND AND PURPOSE: Randomized patent foramen ovale closure trials have used open-label end point ascertainment which increases the risk of bias and undermines confidence in the conclusions. The Gore REDUCE trial prospectively performed baseline and follow-up magnetic resonance imaging (MRIs) for all subjects providing an objective measure of the effectiveness of closure. METHODS: We performed blinded evaluations of the presence, location, and volume of new infarct on diffusion-weighted imaging of recurrent clinical stroke or new infarct (>3 mm) on T2/fluid attenuated inversion recovery f

Mapping the path to degeneration: How lesion location predicts post-stroke cerebellar atrophy.

INTRODUCTION: Stroke can lead to neurological changes beyond the initial lesion site, including post-stroke crossed-cerebellar degeneration. While traditional methods typically rely on total lesion volume to assess remote effects, the spatial distribution of lesions may more accurately predict cerebellar atrophy and associated functional deficits. This study investigated whether anatomically specific cortical lesions contribute to cerebellar gray matter volume loss, expanding on the hypothesis that cerebellar atrophy may reflect more than global brain injury severity, and instead result from t

Feel it or deal with it: Oral perception and aspiration risk in early stroke.

BACKGROUND: Sensation and perception impairments are significant problems faced by individuals with early-stage stroke. Sensory inputs needed in swallowing guide the perceptual process with the involvement of cognitive functions. In the absence of sensory input, bolus perception may be altered, and swallowing safety may be compromised. OBJECTIVES: This study aims to evaluate the oral perception of volume change and the aspiration risk of individuals with stroke and determine the relationship between oral perception and aspiration risk. METHODS: Total participants were 35. The Gugging Swallow

Characteristic lesion pattern and echocardiographic findings in extra-cardiac shunt-related stroke.

Among embolic strokes of undetermined source, under-recognized etiology such as extra-cardiac shunt could be a potential risk factor. We sought to characterize infarction patterns on diffusion-weighted imaging (DWI) and transesophageal echocardiography (TEE) findings in extra-cardiac shunt-related stroke. We enrolled 96 consecutive patients with cryptogenic stroke who had an extra- or intra-cardiac shunt. Diagnosis of the shunt was performed using TEE with agitated saline contrast and pulmonary vein isolation. Infarction patterns on DWI and total lesion volume were analyzed. Bubble amounts thr

Impact of albuminuria on early neurological deterioration and lesion volume expansion in lenticulostriate small infarcts.

BACKGROUND AND PURPOSE: Albuminuria, a marker of chronic kidney disease, is associated with an increased risk of incident stroke and unfavorable long-term outcomes. However, the association of albuminuria with short-term outcomes and change in infarct volume in patients with acute small subcortical infarction remains unknown. METHODS: We retrospectively reviewed 85 consecutive patients with acute small subcortical infarcts in the lenticulostriate artery territory who were admitted to our stroke center within 24 hours of symptom onset and underwent serial diffusion-weighted imaging (DWI). Autonomic dysfunction as a possible cause of sudden cardiac death in swimming sports.

INTRODUCTION: Human diving reflex is a well-studied phenomenon. However, very little is known about the possible relationship between augmented diving reflex and autonomic dysfunction. METHODS: We retrospectively studied a group of four swimmers who underwent a diving reflex test as part of the examination due to symptoms related to autonomic dysfunction during swimming. The control group comprised 11 healthy swimmers with no history of these symptoms. A standardized diving reflex test was performed for each athlete in both groups. Hemodynamic profiles, including heart rate, stroke volume, an…

[Doppler echocardiography determination of systolic and diastolic function in sequential and ventricular stimulation].

In 16 patients with sequential pacing and partly impaired left-ventricular function (mean shortening fraction 23 +/- 7%), the flow through the mitral and aortic valves were studied with Doppler echocardiography at different AV-intervals, keeping heart rate constant at 70/min. The time-velocity integral of the early and late diastolic filling, the first one-third filling fraction, and the diastolic-filling time were found to be significantly dependent on the length of the AV interval (p less than 0.001). In the left ventricular outflow tract, the maximum stroke volume of 85.6 +/- 28.4 ml was me…
