Computer-assisted virtual surgical technology in pre-operative design for the reconstruction of calcaneal fracture malunion.

PURPOSE: No computer-assisted pre-operative design for calcaneal fracture malunions has been presented. The aim of the study is to evaluate the intra-operative realization of computer-assisted pre-operative planning (CAPP) and the clinical outcomes based on computer-assisted virtual surgical technology for calcaneal malunions. METHODS: Between 2010 and 2016, 20 patients with 21 calcaneal fracture malunions were retrospectively reviewed with the average follow-up time of 22.3 months (range, 12 to 43 months), which were operatively treated with the help of CAPP. The CAPP steps included the imag

Pubovaginal sling technique utilizing a unique bone anchor instrumentation system.

INTRODUCTION: In the past, the pubovaginal sling (PVS) technique was originally delegated for the treatment of intrinsic sphincter deficiency syndrome (ISD). Today, it has not only undergone a revitalization, but is being recommended for the treatment of all forms of stress urinary incontinence (SUI) as well as the ISD syndrome. In an attempt to combine the best features of the traditional approach plus add the benefits of simplicity, reduction of costs, morbidity, and rapid return to patient normality, a new variation of the PVS has been developed. The technique utilizes pre-threaded bone anc

Anterior extraoral surgery to the upper cervical spine.

STUDY DESIGN: A series of 10 patients surgically treated with prevascular or retrovascular extraoral retropharyngeal approach to the upper cervical spine is examined. OBJECTIVES: In orthopedic surgery, retropharyngeal approach may replace the transoral surgery, obviating the risks of infection and the uncomfortable postoperative course of cases in which median labiomandibular glossotomy was used to accomplish complex bone reconstruction. SUMMARY OF THE BACKGROUND DATA: The transoral approach is reported in literature as the classical anterior access to the upper cervical spine that provides

A Lightweight, 3D-Printable, Low-Cost, "One-Click" Dorsal Skin Window for High-Quality Long-Term Multimodal Subcutaneous Tumor Imaging.

Intravital microscopy (IVM) using dorsal skinfold chambers (DSCs) enables real-time, high-resolution imaging of the tumor microenvironment. Conventional, metal-based DSC systems often cause animal distress and require technically demanding surgical implantation. To address these challenges and adhere to the 3Rs principles of animal welfare, this study presents a novel, stereolithography (SLA)-based, 3D-printed DSC featuring a lightweight (0.69 g) biocompatible resin structure and a suture-free "one-click" fixation system. This affordable device simplifies surgical installation and minimizes po

Implantable Cardioverter-Defibrillator (ICD) Lead-Induced Septal and Left Ventricular Perforation in Hypertrophic Cardiomyopathy: A Case Report.

Left ventricular (LV) perforation caused by an implantable cardioverter-defibrillator (ICD) lead is a rare but potentially fatal complication. We describe the case of a 34-year-old man with hypertrophic cardiomyopathy who underwent single-chamber ICD implantation for primary prevention of sudden cardiac death using an active-fixation transvenous lead positioned in the right ventricular septum. Four days after implantation, he developed acute chest pain, dyspnoea, hypotension, with clinical signs of evolving cardiac tamponade. Transthoracic echocardiography demonstrated a moderate circumferenti

A Study of Inflammatory/Necrosis Biomarkers in the Fracture of the Femur Treated with Proximal Femoral Nail Antirotation.

Pertrochanteric fractures are common injuries in adults and source of morbidity and mortality among the elderly. Different surgical techniques were recommended for their treatment but undoubtedly they add an additional inflammatory trauma along the fracture itself. Many attempts to quantify the degree of approach-related trauma are carried out through measurements of systemic inflammatory parameters. In this study we prospectively analyzed laboratory data of 20 patients over eighty with pertrochanteric fracture of the femur treated with proximal femoral nail antirotation (PFNA). This is an exc

Treatment of unstable extra-articular distal radius fractures by modified intrafocal Kapandji method.

The authors prospectively assess the results of surgically treated, unstable extra-articular distal radius fractures from 29 patients with good bone quality. Mean age was 49 years, ranging from 22 to 69 years; the female gender was the most frequently affected (58.6% of the cases). Surgical fixation was indicated for fractures presenting an angulation above 20 degrees , marked dorsal comminution, and radius shortening in excess of 10 mm on initial x-rays (anteroposterior and lateral views). The Kapandji technique, with intrafocal, nonthreaded Kirschner wires, was employed. Clinical data assess

Salvage pectopexy using detached lateral suspension mesh arms.

BACKGROUND: Laparoscopic lateral suspension is an alternative to sacrocolpopexy when access to the sacral promontory is restricted or unsafe. However, prolapse recurrence may occur due to mesh arm detachment or progressive fascial attenuation. Pectineal fixation is an alternative approach that may provide a stronger, more stable anchoring vector and improved force distribution. OBJECTIVES: To describe a minimally invasive salvage surgical strategy for managing recurrent anterior compartment prolapse in a patient with a history of laparoscopic lateral suspension and inaccessible sacral promont

Atlas-based automatic planning and 3D-2D fluoroscopic guidance in pelvic trauma surgery.

Percutaneous screw fixation in pelvic trauma surgery is a challenging procedure that often requires long fluoroscopic exposure times and trial-and-error insertion attempts along narrow bone corridors of the pelvis. We report a method to automatically plan surgical trajectories using preoperative CT and assist device placement by augmenting the fluoroscopic scene with planned trajectories. A pelvic shape atlas was formed from 40 CT images and used to construct a statistical shape model (SSM). Each member of the atlas included expert definition of volumetric regions representing safe trajectory
