Selective Vagus Nerve Stimulation as a Therapeutic Approach for the Treatment of ARDS: A Rationale for Neuro-Immunomodulation in COVID-19 Disease.

Acute respiratory distress syndrome (ARDS) is the most severe form of acute lung injury. It is induced by sepsis, aspiration, and pneumonia, including that caused by SARS coronavirus and human influenza viruses. The main pathophysiological mechanism of ARDS is a systemic inflammatory response. Vagus nerve stimulation (VNS) can limit cytokine production in the spleen and thereby dampen any systemic inflammation and inflammation-induced tissue damage in the lungs and other organs. However, the effects of increased parasympathetic outflow to the lungs when non-selective VNS is applied may result

A Novel Therapeutic Strategy for Sepsis: The Multiorgan Protective Properties and Clinical Potential of Ulineastatin.

Sepsis, a life-threatening systemic inflammatory response induced by infection, remains a major therapeutic challenge with high mortality rates, especially when progressing to multiple organ dysfunction syndrome. Ulinastatin (UTI), a serine protease inhibitor with anti-inflammatory and antioxidative properties, has emerged as a promising therapeutic candidate for sepsis-induced organ injury. This article systematically reviews recent advances in the multiorgan protective effects of UTI against sepsis, focusing on its mechanisms through modulating key signaling pathways, reducing inflammatory c

Acetaminophen has limited antipyretic activity in critically ill patients.

PURPOSE: Fever occurs commonly in the critically ill patients and may adversely affect outcome. Acetaminophen is one of the most commonly used antipyretic agents in the intensive care unit; however, there is little evidence that it is effective in this population. The objective of this study was to analyze the antipyretic activity of acetaminophen in critically ill patients. MATERIALS AND METHODS: We performed a retrospective study of medical intensive care unit and surgical intensive care unit patients with systemic inflammatory response syndrome and compared the resolution of fever in the p…

[SARS-CoV-2 infection as a thrombo-inflammatory endothelial disease].

Coronavirus disease 19 is characterized by an intense inflammatory response and a high incidence of thrombotic events. Autopsy studies show severe endothelial injury associated with thrombosis of the alveolar capillaries of the lungs and other organs. In the pathophysiology of this disease endothelial injury and dysfunction, inflammation and thrombosis are key factors for the development of severe phenotypes. Therefore, we should consider this entity as a systemic endothelial disease in which an obstructive microvascular syndrome secondary to an intense thrombo-inflammatory response leads to a

The inflammatory response to miniaturised extracorporeal circulation: a review of the literature.

Conventional cardiopulmonary bypass can trigger a systemic inflammatory response syndrome similar to sepsis. Aetiological factors include surgical trauma, reperfusion injury, and, most importantly, contact of the blood with the synthetic surfaces of the heart-lung machine. Recently, a new cardiopulmonary bypass system, mini-extracorporeal circulation (MECC), has been developed and has shown promising early results in terms of reducing this inflammatory response. It has no venous reservoir, a reduced priming volume, and less blood-synthetic interface. This review focuses on the inflammatory and…

Pulmonary manifestations of systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is a systemic inflammatory disease, characterized serologically by an autoantibody response to nucleic antigens, and clinically by injury and/or malfunction in any organ system. During their disease course, up to 50% of SLE patients will develop lung disease. Pulmonary manifestations of SLE include pleuritis (with or without effusion), inflammatory and fibrotic forms of interstitial lung disease, alveolar hemorrhage, shrinking lung syndrome, pulmonary hypertension, airways disease, and thromboembolic disease. Two major themes inform our understanding of SLE-a

Activation of Myenteric Glia during Acute Inflammation In Vitro and In Vivo.

BACKGROUND: Enteric glial cells (EGCs) are the main constituent of the enteric nervous system and share similarities with astrocytes from the central nervous system including their reactivity to an inflammatory microenvironment. Previous studies on EGC pathophysiology have specifically focused on mucosal glia activation and its contribution to mucosal inflammatory processes observed in the gut of inflammatory bowel disease (IBD) patients. In contrast knowledge is scarce on intestinal inflammation not locally restricted to the mucosa but systemically affecting the intestine and its effect on th

The role and mechanism of the annexin A1 peptide Ac2-26 in rats with cardiopulmonary bypass lung injury.

The main causes of lung injury after cardiopulmonary bypass (CPB) are systemic inflammatory response syndrome (SIRS) and pulmonary ischaemia-reperfusion injury (IR-I). SIRS and IR-I are often initiated by a systemic inflammatory response. The present study investigated whether the annexin A1 (ANX-A1) peptidomimetic Ac2-26 by binding to formyl peptide receptors (FPRs) inhibit inflammatory cytokines and reduce lung injury after CPB. Male rats were randomized to the following five groups (n = 6, each): sham, exposed to pulmonary ischaemic-reperfusion (IR-I), IR-I plus Ac2-26, IR-I plus the FPR an…

Statistical Study for the Prognostic Value of NLR Values in Patients with Severe Burns: A Study in the Service of Burns in Albania.

The phases of burn care include initial resuscitation, definitive wound management, and rehabilitation of physical and psychological injuries. In severe burns, this response lasts for the first two to three days post-burn and burns larger than 15% of total body surface area lead to the initiation of systemic inflammatory response syndrome. Neutrophil-lymphocyte ratio (NLR) is an inflammatory biomarker that can indicate systemic inflammation. NLR is defined by the absolute number of neutrophils divided by the absolute number of lymphocytes. Many studies have been done in severe burn patients be…

Metabolic syndrome and its effect on immune cells in apical periodontitis- a narrative review.

OBJECTIVES: Apical periodontitis (AP) is an inflammatory immune response in periapical tissues caused by microbial infections. Failure of root canal treatment or delayed healing is often due to intracanal or extra-radicular bacteria. However, beyond microbial factors, the patient's systemic health can significantly influence the progression and healing of AP. Metabolic syndrome is a risk factor and it is characterized by a cluster of interconnected metabolic risk factors, including abdominal obesity, hyperlipidemia, hypertension, and hyperglycemia. MATERIALS AND METHODS: A comprehensive liter

Advances in molecular agents targeting toll-like receptor 4 signaling pathways for potential treatment of sepsis.

Sepsis is a systemic inflammatory response syndrome caused by an infection. Toll-like receptor 4 (TLR4) is activated by endogenous molecules released by injured or necrotic tissues. Additionally, TLR4 is remarkably sensitive to infection of various bacteria and can rapidly stimulate host defense responses. The TLR4 signaling pathway plays an important role in sepsis by activating the inflammatory response. Accordingly, as part of efforts to improve the inflammatory response and survival rate of patients with sepsis, several drugs have been developed to regulate the inflammatory signaling pathw…

[Immune reconstitution syndromes secondary to effective antiretroviral therapy].

Highly active antiretroviral therapy (HAART) can improve immune system function through suppression of HIV-1 replication. In some cases, a vigorous paradoxical immune response may develop. Individuals treated effectively can, because of low immune reactivity, suffer from diseases which run subclinically until HAART is introduced. These symptoms are termed IRS (immune reconstitution syndrome/immune recovery syndrome), IRD (immune restoration disease), and IRIS (immune restoration inflammatory syndrome). There are some predisposing factors, such as long-lasting HIV-1 infection, severe immunodefi
