Dynamic correction for forefoot varus in stage II-A adult flatfoot: Technique tip.

Posterior tibial tendon dysfunction (PTTD) is a progressive disorder and a common cause of adult acquired flatfoot deformity, and forefoot varus is a frequent component in advanced cases. The author proposes peroneus brevis-to-longus transfer as an additional step to correct the forefoot varus component of stage II-A posterior tibial tendon dysfunction. We have performed this dynamic correction of forefoot varus in 12 patients at our institution, and observed promising clinical and radiographic improvement. It is a soft tissue procedure that avoids additional incisions and represents a favorab

Biomechanical and Morphometric Properties of the Long Flexor Tendons of the Toes: A Cadaver Study.

BACKGROUND: We sought to show the biomechanical and morphometric properties of flexor hallucis longus (FHL) and flexor digitorum longus (FDL) tendon grafts harvested by specific surgical approaches and to assess the contribution of FHL slips to the long flexor tendons of the toes. METHODS: Thirteen fresh-frozen amputated feet (average age, 79 years) were dissected. The connections between the FHL and FDL tendons and the contribution of FHL slips to the long flexor tendons were classified. The biomechanical properties of the tendons and slips were measured using a tensile device. RESULTS: The

Tendon interposition and ligament reconstruction with ECRL tendon in the late stages of Kienböck's disease: a cadaver study.

BACKGROUND: The optimal surgical treatment for Kienböck's disease with stages IIIB and IV remains controversial. A cadaver study was carried out to evaluate the use of coiled extensor carpi radialis longus tendon for tendon interposition and a strip obtained from the same tendon for ligament reconstruction in the late stages of Kienböck's disease. METHODS: Coiled extensor carpi radialis longus tendon was used to fill the cavity of the excised lunate, and a strip obtained from this tendon was sutured onto itself after passing through the scaphoid and the triquetrum acting as a ligament to pres

The flexible flatfoot in the adult.

The adult acquired flatfoot deformity is characterized by flattening of the medial longitudinal arch with insufficiency of the supporting posteromedial soft tissue structures of the ankle and hindfoot. While the etiology of this deformity can be arthritic or traumatic in nature, it is most commonly associated with posterior tibial tendon dysfunction (PTTD). By one estimate, PTTD affects approximately five million people in the United States. The clinical presentation of adult flatfoot can range from a flexible deformity with normal joint integrity to a rigid, arthritic foot.

[Simple and composite ulnar transplants in reconstructive surgery of the hand].

Over several years, the authors have devised a technique for compound island transplants of ulnar origin, harvested from the forearm's lower third, with reverse flow vascularization. Because of the variety of arterial ramifications, these transplants can consist: either of a simple cutaneous, superficial flexor tendon or ulnar fragment transplant, or, when associated, of compound cutaneous-tendinous, cutaneous-osseous and even cutaneous-tendinous-osseous transplants. These transplants, based on the clinical experience of 192 cases, can be used by retrograde rotation for a variety of indication

[A simple and effective method of the treatment of habitual shoulder dislocation].

The authors propose a method of surgical treatment of habitual dislocation of the shoulder which consists in partial section of the subscapular muscle tendon in 1/2 of its diameter and inserting the long head tendon of the musculus biceps brachii inke is fixed to the humerus by trans-osseous+ sutures. The operation was performed in 207 patients. The observation terms in 181 patients exceed 2 years, and in 26 patients they are less than 2 years. The relapses of the dislocation have been observed in 3 (1.6%) patients, in 2 of them they occurred 5 and 8 years after severe injuries.

Peroneus tertius reconstruction for flexible clawtoes associated with cavus deformity: a preliminary report.

Numerous procedures and techniques are used to correct clawtoe deformity. This paper reports on a new method of correcting clawtoe deformity surgically. Treatment of this condition is not new, and a survey of the literature is offered initially. The theory behind the tendon transposition is discussed and symptoms associated with this deformity are outlined. Variations of this new procedure are listed and illustrated clinically. The authors recommend the peroneus tertius redirection and/or reconstruction procedure as the most adequate way of reducing clawtoe deformity associated with flexible c

Relationships between tibial rotary torque and knee flexion/extension after tendon transplant surgery.

The hamstring muscles play an important role in decelerating and checking tibial rotation before excessive stresses are applied to knee support structures. The purpose of this study was to determine the relationships between standard flexion/extension rehabilitative techniques and tibial rotary torque among subjects who had received pes anserinus transfers at least 1 year prior. Postsurgical and healthy contralateral limbs of 15 males were tested for maximum, isokinetic knee flexion/extension and internal/external tibial rotation. An adjustable chair and stabilizing boot were used to minimize

Flexor carpi radialis opponensplasty.

Although numerous opponensplasties are described, the options are limited when the median and ulnar nerves and the flexor tendons are injured. We describe a new tendon transfer using the previously injured flexor carpi radialis combined with a fascia lata graft for opponensplasty in a patient who had a wrist injury resulting in division of all his flexor tendons, except flexor carpi ulnaris, and the median and ulnar nerves.

Magnetic resonance imaging glenohumeral joint analysis before and after muscle transfer in children with obstetric brachial plexus palsy: a cross-sectional study of 10 cases.

This study aims to analyze the impact of muscle transfer on the glenohumeral joint in children with obstetric brachial plexus palsy (OBPP) using MRI by comparing preoperative and 5-year follow-up postoperative imaging findings to determine whether tendon transfers affect the alignment and configuration of the glenohumeral joint. Ten children with obstetric brachial plexus palsy (OBPP) participated in our prospective observational study, and we performed a tendon transfer technique. Every patient had an MRI of both shoulders done at preoperative and at the 5-year mark following the procedure. T…

Extensor tendon rupture due to osteoarthritis of the distal radio-ulnar joint.

Five cases of closed rupture of the finger extensor tendon due to osteoarthritis of the distal radioulnar joint were studied. Difficulty in extension began at the little finger and extended to the ring and long fingers. Pain and swelling in the dorsal aspect of the wrist preceded the tendon rupture. Osteoarthritic changes at the distal radio-ulnar joint were more severe than those at the radio-carpal and intercarpal joints. The distal end of the ulna showed the plus variant, as well as dorsal dislocation or subluxation. All patients underwent a tendon graft or tendon transfer, with excision of…

Double lateral band transfer for treatment of traumatic hyperextension instability of the proximal interphalangeal joint: a report of two cases.

Post-traumatic hyperextension instability of the proximal interphalangeal joint may lead to pain, difficulty with initiating finger flexion, and a swan-neck deformity. Most techniques to correct a traumatic hyperextension deformity of the proximal interphalangeal joint require a window in the flexor retinaculum, retraction of the flexor tendons, and manipulation of the joint capsule with a conceivable potential for joint stiffness, tendon adhesions, and tendon bowstringing. We describe an extra-articular lateral band transfer technique that utilizes strips of both lateral bands and preserves t

Predislocation syndrome: a review and retrospective analysis of eight patients.

Predislocation syndrome is defined as an either an acute, subacute, or chronic inflammatory process involving the lesser metatarsophalangealjoints. If left untreated, this process can lead to plantarplate and capsular attenuation and metatarsophalangeal joint instability. A retrospective analysis were performed on eight patients who were treated with a flexor tendon transfer for this condition. A subjective analysis and chart review were performed to determine the final outcome of these patients. All patients had a chief complaint of a painful second metatarsophalangeal joint with seven of eig…

The cavovarus foot deformity. Etiology and management.

The cavovarus foot is a complex deformity of the forefoot and hindfoot, frequently of neuropathologic etiology. A progressive spinal or peri … e and metatarsal osteotomies is reliable. The underlying muscle imbalance must be addressed through tendon transfers to prevent further progression. In the young child, a rigid hindfoot varus will often be corrected by a radical plantar-medial release, but the more mature foot may require midtarsal osteotomy and calcaneal osteotomy or triple arthrodesis. The surg …
