De Quervain disease in volleyball players.

BACKGROUND: Chronic inflammatory tendon diseases in athletes are frequent, and they often result from modifications in normal kinematics of a tendon associated with a patient's anatomical determinants. De Quervain stenosing tenosynovitis is an inflammatory disease of tendons of the first dorsal compartment of the wrist. There is no literature about this disease concerning professional volleyball players. HYPOTHESIS: Limited, multiple trauma on the dorsal radial portion of the wrist, associated with long training times, can be involved in the pathogenetic process of de Quervain disease in prof…

Foot problems in middle-aged patients: keeping active people up to speed.

Most of the common foot problems that bother active middle-aged people are self-limiting and easily treated if detected early. Reviewed here are the causes, symptoms, diagnosis, and treatment of hallux valgus and rigidus, lesser-toe deformities, corns, Morton's neuroma, metatarsal stress fractures, plantar fasciitis, posterior tibialis tenosynovitis and rupture, acquired pes planus, tarsal tunnel syndrome, and foot problems related to rheumatoid arthritis and diabetes. In most cases, conservative treatment will enable patients to return to activity relatively quickly.

Trigger Finger: An Unusual Clue to Hashimoto's Thyroiditis.

Stenosing flexor tenosynovitis, commonly known as trigger finger (TF), is characterized by thickening and inflammation of the flexor tendon sheath (A1 pulley), leading to painful catching or locking of the finger in a flexed position. While often associated with conditions like diabetes and hypothyroidism, this case report presents a unique instance where surgical intervention for pharmacologically resistant TF ultimately led to the diagnosis of Hashimoto's thyroiditis. This case highlights the potential for underlying systemic conditions to manifest as TF and emphasizes the importance of a co…

Ultrasonography in Pediatric Rheumatology.

This review highlights the increasing evidence from the last few years supporting the use of musculoskeletal ultrasonography (MSUS) in the evaluation and management of patients with pediatric rheumatic diseases, particularly focusing on juvenile idiopathic arthritis. Recently developed definitions for the sonographic appearance of healthy and pathologic joints in children are discussed. Further topics explored include how MSUS enhances the diagnosis of inflammatory joint disease (synovitis, enthesitis, tenosynovitis), including the detection of subclinical synovitis. There is a brief summary o…

Imaging of Soft Tissue Infections.

Although superficial infections can often be diagnosed and managed clinically, physical examination may lack sensitivity and specificity, and imaging is often required to evaluate the depth of involvement and identify complications. Depending on the area of involvement, radiography, ultrasound, CT, MR imaging, or a combination of imaging modalities may be required. Soft tissue infections can be nonnecrotizing or necrotizing, with the later having a morbid and rapid course. Infectious tenosynovitis most commonly affects the flexor tendon sheaths of the hand, characterized by thickened and enhan…

Current Perspectives on Imaging for Systemic Lupus Erythematosus, Systemic Sclerosis, and Dermatomyositis/Polymyositis.

Systemic rheumatic diseases frequently pose diagnostic challenges to the clinician. The precise nature of the disorder can be obscure and different disorders can present with similar symptoms, such as joint pain. Plain radiographs provide an appropriate starting point for detection of joint abnormalities. Musculoskeletal ultrasound and MRI offer greater sensitivity in detecting synovitis, tenosynovitis and bony erosions, among other features. However, due to the rarity of these diseases and lack of prospective longitudinal trials, a broader picture of the epidemiology of these findings and the…

Spontaneous Rupture of the Extensor Pollicis Longus Tendon due to Unusual Etiology.

BACKGROUND: The etiology of spontaneous rupture of the extensor pollicis longus tendon includes systemic or local steroid injections, wrist fracture, tenosynovitis, synovitis, rheumatoid arthritis, and repetitive wrist motions. CASE REPORT: We encountered a case of extensor pollicis longus tendon rupture with an unusual etiology, cow milking. In this case, transfer of the extensor indicis proprius tendon was performed successfully. At 1 year after surgery, extension of the thumb was sufficient. CONCLUSION: It appears that patients with occupations involving repetitive motions are at a high r…

Treatment of extensor digitorum brevis manus myalgia with botulinum toxin.

Extensor digitorum brevis manus (EDBM) muscles are accessory dorsal hand muscles that are present in 1%-3% of the population. These muscles are not commonly symptomatic, but they can occasionally cause pain and discomfort and may be misdiagnosed as tenosynovitis or a ganglion cyst. In this case, we describe an appropriate workup of dorsal hand masses and myalgia that are suspected to be related to EDBM muscles. The patient's symptoms were controlled with injections of botulinum toxin into the EDMB muscle belly, which allowed the patient to be relatively pain free for a considerable amount of t…

[Infection of a tophaceous nodule of the middle finger].

Gouty involvement of the hand is uncommon and the complications the most described are bony destruction, digital joint instability, tenosynovitis and double ipsilateral tunnel syndrome of the median and ulnar nerves at the wrist. Septic complications, such as infection of a tophaceous nodule, are a rare entity. The authors report a case of an infection of a tophaceous nodule of the middle finger in a 67-year-old man. The diagnosis was suspected on physical examination and radiological findings but final diagnosis is provided by bacteriological examination. This case is presented to heighten aw…

Carpal tunnel syndrome associated with a fracture of a silicone implant for Kienböck's disease: two case reports.

In this report, two cases presenting with CTS including a fracture of the silicone implant for Kienböck's disease after replacement over 20 years postoperatively where CTR ceased the symptoms of CTS. The major symptom characteristic was the median nerve impairment with less emphasis on the limited range of motion of the wrist or wrist pain due to a fracture of the silicone implant. There was an unlikely association between the fracture of the implant and CTS, which was confirmed by the operative findings of tenosynovitis and thickness of the degenerated transverse ligament without a significan…

Repetitive motion hand disorders.

The clinical management of cumulative trauma disorder is based upon the identification and treatment of individual component pathologies and, frequently, referral to a knowledgeable occupational therapist with an understanding of ergonomic behavioral, postural, and workspace modification. Most commonly these individual pathologic entities are carpal tunnel syndrome, cubital tunnel syndrome, trigger finger, and De Quervain's tenosynovitis. In this article, the anatomy, diagnosis, and treatment of each of these disorders will be considered separately. In addition, since these clinical entities a…

Pigmented villonodular synovitis in the hand.

The related disorders of pigmented villonodular synovitis and pigmented villonodular tenosynovitis have very different clinical presentations. The synovial form occurs most frequently in the knee although other lower extremity joints including the hip, ankle and subtalar joints can be involved. Rarely, the shoulder, elbow and wrist have been the joint of origin. In contrast, the tenosynovial form is one of the most common masses in the hand and is found very much less frequently elsewhere. This report documents the synovial origin of four cases involving the small joints of the hands.

Peripheral Spondyloarthritis: Conceptualization and Diagnosis as a Distinct Rheumatologic Subset of Disease.

Peripheral spondyloarthritis (pSpA) encompasses multiple entities affecting the musculoskeletal system with classic findings of synovitis, enthesitis, and tenosynovitis. It remains poorly defined when compared with the axial spondyloarthritides due to nonspecific symptoms, ill-defined imaging findings, and lack of specific testable biomarkers. Previous research has tried to distinguish between the two disease entities using clinical presentations, scoring systems, and radiographic findings; however, overlap remains. We summarize the current classification of pSpA, imaging findings, and the dis…

[Update in Current Care guidelines: repetitive strain injuries of the hand and forearm].

Repetitive strain injuriesof the upper extremities refer to pain in the forearm, wrist and hand, caused by excessive strain. Diagnoses include tenosynovitis, epicondylitis and carpal tunnel syndrome. Treatment is aimed at alleviating pain, restoring normal physical functioning and maintain ability to work. Preventive interventions have shown some attenuation of discomfort but no effects on disease prevalance or sick leave days. Return to work interventions seem to decrease length of sickness absences. Part time work has hastened return to work and decreased sickness absences in musculoskeletal…

Role of ultrasound in the assessment of juvenile idiopathic arthritis.

US has demonstrated higher sensitivity in detecting synovitis, tenosynovitis and enthesitis than clinical examination in JIA. US has also allowed earlier assessment of cartilage and bone abnormalities than conventional radiology. Although it seems that US detects a high frequency of subclinical synovitis in JIA patients with clinically inactive disease, further studies are needed to clarify those findings. In addition, assessment of structural damage changes in growing children should be taken with caution until more information about normal US articular anatomy in healthy children is availabl…
