A 12-yr-old Guamanian girl presented with seven premolars displaying dens evaginatus. The pulp of one tooth was necrotic secondary to fracture of the evagination. Calcium hydroxide treatment was used to induce closure of this root apex, and subsequent routine canal obturation resulted in clinical success. Three teeth had fractured evaginations and compromised coronal pulps, but the radicular pulp tissue was vital. Coronal calcium hydroxide pulpotomy procedures were performed, and root development continued on all three teeth. In one of these teeth, endodontic treatment was completed, and in th

Before realizing a new indirect restoration on an endodontically treated tooth, present restorations and possible posts need to be assessed. In the case of a metal post, the mismatch in flexural modulus between the dentine and the post might increase the risk of a vertical root fracture. Is it therefore prudent to replace such a metal post, before the fabrication of a new indirect restoration? Two scenarios might be considered to answer this question. First, would a glass fibre post result in a lower risk of failure? Two systematic reviews of the literature comparing glass fibre and metal post

Condylar fractures are the most common injury seen in pediatric mandibular trauma. These injuries often cannot be adequately stabilized by conservative techniques such as splinting. The pediatric condyle fracture often requires a period of intermaxillary fixation. Because of the characteristics of the developing dentition, circumdental wiring is often not possible. Surgeons commonly achieve interdental stabilization by the connection of a circum-mandibular wire and a second wire placed through a drill hole in the piriform aperture. This method can be problematic in the young patient whose pala

Post-traumatic complications occasionally lead to tooth loss as well as the need for future implants. However, rehabilitation with endosseous osseointegrated implants does not protect the patient from the risk of suffering a new trauma. Implant fracture and the damage of the hexagon are post-traumatic complications that guide the clinician to preparing a more intricate treatment plan. The authors present a clinical case of a recurrent trauma of maxillary implant fracture. The treatment plan was to remove the implants followed by autogenous bone grafting to correct the defect. Two titanium impl

Background/purpose An ideal post material should have physical and mechanical properties similar to dentin. Another problem when restoring primary teeth that have undergone root canal treatment is the availability of materials that are resorbed in the exfoliation process in a manner similar to the structure of the natural tooth, allowing the normal eruption of a permanent tooth. This study aimed to evaluate the effect of using dentine posts on the fracture resistance of endodontically treated primary incisors in comparison to glass fiber posts. Materials and methods The study sample consisted

Mandibular distraction osteogenesis is an increasingly accepted treatment option for severe upper airway obstruction in grade 3 Robin sequence. Complications are rarely reported but can include fracture, pin dislodgement, tooth bud damage, and temporomandibular joint ankylosis. Operative correction of these complications can carry inherent risks of their own. We present a patient who incurred carotid artery dissection and stroke after release of postdistraction coronoid-zygomatic ankylosis for the treatment of mandibular micrognathia.

Extrusion of teeth may be necessary in cases of delayed eruption, primary retention, traumatically intruded teeth, or subgingivally fractured teeth. Removable appliances are advantageous, as anchorage is not as tooth-dependant as in the case of fixed appliances. They are cost-effective, operator friendly, and a valuable treatment option to consider in cases where extrusion of anterior teeth in the transitional dentition is necessary. The purpose of this paper was to describe a simple, cost-effective technique using a removable appliance for extrusion of incisors in the transitional dentition.

Osteogenesis imperfecta, or "brittle bone disease," is a type I collagen-related condition associated with osteoporosis and increased risk of bone fractures. Using a combination of homozygosity mapping and candidate gene approach, we have identified a homozygous single base pair deletion (c.1052delA) in SP7/Osterix (OSX) in an Egyptian child with recessive osteogenesis imperfecta. The clinical findings from this patient include recurrent fractures, mild bone deformities, delayed tooth eruption, normal hearing, and white sclera. OSX encodes a transcription factor containing three Cys2-His2 zinc

It is known that fractures are more likely to occur in altered teeth, particularly following restoration or endodontic repair; consequently, it is important to understand the structure of altered forms of dentin, the most abundant tissue in the human tooth, in order to better define the increased propensity for such fractures. Transparent (or sclerotic) dentin, wherein the dentinal tubules become occluded with mineral as a natural progressive consequence of aging, is one such altered form. In the present study, high-resolution transmission electron microscopy is used to investigate the effect

Etiological and predisposing factors related to traumatic injuries to permanent teeth were studied in an epidemiological material comprising 1610 children with 286 registered traumatic dental injuries. The results showed that (1) postnormal occlusion, (2) an overjet exceeding 4 mm, (3) short upper lip, (4) incompetent lips, and (5) mouth breathing were all factors which significantly increased susceptibility to traumatic dental injury. The average overjet in children with injuries limited to enamel fracture was 4.3 mm. In cases exhibiting more severe injuries (dentine fracture, pulp lesion, ro

This study evaluates the efficacy of 3D-printed band and loop space maintainers (3D-BLSMs) to mitigate concerns caused by early primary tooth loss in children when compared to their conventional equivalents. Over 9 months, 62 participants aged 6 to 12 years participated in a randomized clinical study. This study evaluated their failure rates (de-cementation, debonding, solder breakage, loop breakage, band breakage, and abutment tooth fracture), gingival health, and patient overall satisfaction. Random assignments were made to place the participants in two groups: traditional band and loop spac

Sampling of healthy multi-rooted teeth is recommended for the genetic identification of human skeletal remains. However, this may not always be possible, as in the reported case consisting of an isolated human cranium found in an aggregate crushing and processing plant in Piedmont, Northwest Italy. The cranium displayed significant weathering, suggesting a post-mortem interval of several years, and was edentulous with the exception of the apical root fragment of the upper left canine, consequence of an antemortem horizontal fracture. Prolonged decalcification of the root fragment followed by p

Hypophosphatasia is a rare inherited metabolic bone disease caused by low activity of tissue-nonspecific alkaline phosphatase. Adult-onset hypophosphatasia is increasingly recognized, but fracture patterns and optimal management remain unclear. We report a 52-year-old woman who presented with progressive pain and bilateral atypical femoral fractures. She was diagnosed with hypophosphatasia based on low alkaline phosphatase, markedly elevated pyridoxal 5'-phosphate, a history of metatarsal fracture, premature tooth loss, and pathogenic ALPL variants. Low-dose asfotase alfa was started because s

To assess the fracture resistance of computer-aided design and computer-aided manufacturing (CAD/CAM) ultratranslucent multilayered zirconia (5Y-YZP) veneers with varying facial thickness. Sixty translucent zirconia veneers were designed and milled using a chairside CAD/CAM system for maxillary central incisors. The butt joint incisal veneer tooth preparations consisted of 1.00 mm incisal reduction, 0.40 mm chamfer margin, and three different facial reductions; 0.50, 0.75, and 1 mm, respectively. The ceramic veneers were cemented to printed resin dies and subjected to thermal cycling. Subseque
