[Injured tongue due to bite of fer-de-lance (author's transl)].

A nine-year-old boy was bitten in the tongue by an Asiatic fer-de-lance, a Trimenesurus wagleri. No generalized toxic reaction developed; there was only locally a massive edema of the tongue leading to a severe dyspnea. The child was intubated and administered antiphlogistics, antibiotics, cortison, a tetanus booster shot as well as parenteral nutrition. After three days the local status was unchanged. Because of the danger of the development of necrosis in the laryngeal region as a result of the toxic effect and the sustained intubation, a tracheotomy inferior was carried out. The status loca…

[Surgical plan selection and efficacy analysis in 32 cases of laryngotracheal stenosis].

Objective: To explore the optimization of surgical procedures for laryngotracheal stenosis and its effect analysis. Methods: The data of 32 patients with acquired laryngotracheal stenosis who received surgical treatment from October 2015 to December 2021 were analyzed retrospectively. The age ranged from 19 to 72 years, with an average of (34.0±9.0) years. The medical history ranged from 1 to 32 months (median 3 months). As for etiology, there were 30 cases of iatrogenic laryngotracheal stenosis, including 20 cases of tracheal intubation and 10 cases of tracheotomy (7 cases of percutaneous tra…

Practical approach to respiratory emergencies in neurological diseases.

Many neurological diseases may cause acute respiratory failure (ARF) due to involvement of bulbar respiratory center, spinal cord, motoneuro … h assistance, noninvasive ventilation, endotracheal intubation, invasive mechanical ventilation, or tracheotomy. This review provides practical recommendations for prevention, recognition, management, and treatment of respiratory emergencies in neurological diseases, mostly in teenagers and adults, according to type and severity of baseline disease.

[Surgery with laryngotracheal stenosis. Rib cartilage interposition for airway reconstruction].

The surgical management of this problem is a major challenge with various optional surgical procedures. The Chinese authors show in their retrospective analysis the success of laryngotracheal reconstruction (LTR) by cartilage interposition and facial cutaneous flap in patients with complex laryngotracheal stenoses and / or anterior cervical defect.

[Diagnosis and treatment of esophageal perforation induced by esophageal foreign body in children].

OBJECTIVE: To discuss the diagnosis and treatment of esophageal perforation induced by esophageal foreign body. METHOD: Ten patients, who were diagnosed as esophageal foreign body and esophageal perforation, were retrospectively analyzed. One patient was operated in other hospital and transferred to our department post-operation. The foreign bodies were removed through rigid esophagoscope in 7 cases and through the tracheotomy in 1 case. The last case was admitted into hospital for abscess around the esophagus. The foreign body wasn't find during the examination of rigid esophagoscope and the…

[Botulism and dysphagia].

BACKGROUND: Botulism is a severe neuroparalytic, of an acute characteristic, afebrile and is caused by the action of a toxin produced by Clostridium botulinum. This toxin links itself to the receptors of the axon membrane of the motor neurons, preventing the release of acetylcholine in the neuromuscular junction, causing flaccid paralysis of the cranial nerves and skeleton musculature. AIM: To present the speech therapy procedures adopted with a patient with botulism and who was presenting dysphagia. METHOD: A male adult, with botulism, sent for a speech-language evaluation due to the presen

Ventilator-associated pneumonia: issues related to the artificial airway.

Pooling of contaminated secretions above the cuff of the endotracheal tube predisposes patients to ventilator-associated pneumonia (VAP). Su … s implemented on a large scale. A patient who has a colonized airway and who undergoes percutaneous tracheotomy has an increased risk of VAP, particularly due to Pseudomonas aeruginosa, in the week following the procedure. As many studies suggest that incidence of VAP is highly dependent on the strategies of airway management, health care workers should be al …

Tracheal release and thymus wrapping of the tracheoplasty anastomosis through mini-sternotomy.

Despite breakthroughs in general thoracic surgery, tracheoplasty remains a technically difficult procedure. The authors performed tracheoplasty on a 75-year-old woman diagnosed with recurrent thyroid cancer and tracheal invasion. Through an L-shaped unilateral mini-sternotomy added to a collar incision, we performed tracheal mobilization and release followed by resection of six tracheal rings. The tracheoplasty anastomosis was then wrapped with the right lobe of the thymus. Using this procedure, tracheoplasty was successfully performed with no complications related to the tracheal anastomosis.

[Pleomorphic adenoma of the tongue base. A new case of big size].

The pleomorphe adenomas (PA) of the tongue base are rare (there are only 6 cases published until now) We present one case of a male 42 years old with a PA of the tongue base very big but with few symptoms. The FNAB and MRI were the key for the diagnosis and therapy. The treatment was surgical with a trans oral resection. The patient is free of tumor after seven years.

Anesthesia for perinatal surgery.

A 30 year gravida 3 para 2002 woman presented at 28 weeks' gestation with polyhydramnios. Ultrasound revealed a single fetus with a large anterior neck mass. With halothane general anesthesia providing uterine relaxation and maintaining placental and umbilical blood flow, an otolaryngologist attempted to secure the airway. We discuss the rationale for our anesthetic management and suggest its use in similar situations in which surgical intervention may be required in the immediate perinatal period.

[Concomitant narcolepsy and sleep apnea: report of a case].

A case of concomitant narcolepsy and obstructive sleep apnea evaluated in a sleep disorder center is reported. Tracheostomy decreased the frequency of the apneas but the clinical and polysomnographic features of narcolepsy remained on 3 and 6 months post-surgical follow-ups. These findings support the need for systematic use of all-night polysomnography and multiple sleep latency test in patients with excessive daytime sleepiness.

Abnormalities of breathing control and airway maintenance in infants and children as a cause of cor pulmonale.

Respiratory control abnormalities may result in cor pulmonale. This report summarizes the clinical history, diagnostic evaluation, treatment, and outcome of 16 infants and children presenting with cor pulmonale subsequently found to be due to sleep-dependent hypoventilation. Eleven patients had cardiomegaly and electrocardiographic evidence of right ventricular hypertrophy (RVH) while 5 had only severe RVH or biventricular hypertrophy (BVH). Four infants with central hypoventilation syndrome (CHS)--absence of sleep-related ventilatory drive--had severe sleep-dependent asphyxia and resultant ac

Prosthetic support of laryngotracheoplasty for acquired subglottic stenosis in infants and children.

Using readily available materials and familiar techniques, prosthetic stents and T-tubes can be made to provide invaluable support in the treatment of subglottic stenosis in infants and children. A method of constructing these stents and tubes has been described along with an explanation of their surgical purpose and use.

Acute epiglottitis in adults.

Epiglottitis occurs more often in adults than is generally recognized. Six adults with epiglottitis were treated at Evanston (III) Hospital from March to August 1978. Two required tracheostomy because of delayed diagnosis. There were no deaths. This experience suggests that any patient with acute, painful dysphagia should have indirect laryngoscopy or a carefully interpreted lateral roentgenogram to the neck to rule out epiglottitis.
