Sudden cardiac death is a common mechanism of demise in association with congenital cardiac abnormalities. The varied mechanisms may include failure of the transitional circulation, arrhythmias, postoperative or perioperative complications in the neonate and coronary ischaemia, arrhythmias, sepsis, thrombosis, or pulmonary hypertensive crisis in the older child. Knowledge of the natural history of unoperated and operated forms of congenital heart disease and long term follow up with the detection and treatment of underlying hemodynamic abnormalities should improve outcomes. There are few patie

The diagnosis "hiatal hernia" does not equate with surgery, but if it is combined with disturbed function of the gastrooesophageal transitional area or reflux oesophagitis it needs therapy. Therefore, the diagnosis has to be made with care. Based on over 800 cases the intraluminal layered pH-metry proved to be a very helpful method. Paraoesophageal hernias should be surgically treated due to their tendency towards strangulation and incarceration. From 1956 to 1976 287 patients suffering from hiatal hernia were operated on, mortality rate and late results are analysed.

BACKGROUND: Surgical and perioperative improvements permit earlier repair of partial and transitional atrioventricular septal defects (AVSD). We sought to describe contemporary outcomes in a multicenter cohort.
METHODS: We studied 87 patients undergoing primary biventricular repair of partial or transitional AVSD between June 2004 and February 2006 across seven North American centers. One-month and 6-month postoperative data included weight-for-age z-scores, left atrioventricular valve regurgitation (LAVVR) grade, residual shunts, and left ventricular ejection fraction. Paired methods were us

INTRODUCTION: In health services research, there is a special emphasis on the transition from adolescence into adulthood. During this transition period, adolescents change from pediatric to adult medical care. This process must be carefully structured, particularly when special medical care is required. Challenges and difficulties become apparent particularly in the case of rare diseases. This is increasingly so when the rare disease affects the adolescence-specific development of patients, such as Mayer-Rokitansky-Küster-Hauser syndrome (MRKHS), also known as Müllerian agenesis.
METHODS: A s

BACKGROUND: The transition from pediatric to adult care is a critical time when adolescent patients and their families face many challenges. This period can be associated with an increase in disease-related morbidity and mortality. The aim of our study is to identify gaps in transition-related care to help guide areas for improvement.
METHODS: Patients (14-19 years) with juvenile idiopathic arthritis or systemic lupus erythematosus and one of their parents were recruited from the McMaster Rheumatology Transition Clinic. Both were asked to complete the Mind the Gap questionnaire, a validated t

Survival to adulthood for people with Spina Bifida now exceeds 85% due to improvements in medical and surgical management. Rates remain lower than expected for community participation, healthy lifestyle choices, employment and independent living. The importance of transition programming to help adolescents with disabilities prepare for adult life roles is now understood. Literature currently is mainly conceptual or descriptive, but informs the process of developing transition program models. The need for competent and effective adult care providers is discussed. Both the transition to adulthoo

BACKGROUND/OBJECTIVES: The transition from pediatric to adult healthcare is a critical and vulnerable period for adolescents and young adults with type 1 diabetes, often associated with increased risks of acute complications and long-term adverse outcomes. Despite its recognized clinical importance, the concept of "transition readiness" remains inconsistently defined and operationalized in research and practice. This study aimed to clarify the concept using Walker and Avant's method of concept analysis.
METHODS: A comprehensive literature search was conducted across multiple databases, comple

INTRODUCTION: Women living with HIV are required to transition into the prevention of mother-to-child transmission of HIV (PMTCT) services when they become pregnant and back to ART services after delivery. Transition can be a vulnerable time when many women are lost from HIV care yet there is little guidance on the optimal transition approaches to ensure continuity of care. We reviewed the available evidence on existing approaches to transitioning women into and out of PMTCT, outcomes following transition and factors influencing successful transition.
METHODS: We searched PubMed and SCOPUS, a

PurposeThis study examined how transitions in living arrangements are associated with changes in sleep quality and sleep duration among Chinese older adults.Major findingsUsing four waves of the Chinese Longitudinal Healthy Longevity Survey data (2008-2018), we examined how living arrangement transitions affect sleep among 10,473 observations from older adults (65+). Cox models showed that older adults who remained living with household members (hazard ratio [HR] = 0.66, 95% CI: 0.53-0.81; p < .001) or transitioned to living alone (HR = 0.71, 95% CI: 0.57-0.89; p = .003) had significantly lowe

BACKGROUND AND OBJECTIVES: Hospital-to-home transitions constitute vulnerable periods during which health systems increasingly rely on unpaid care partners to help care recipients execute discharge plans. We examine how mutuality and incongruence within care partner-care recipient dyads may shape discharge preparedness and dyadic self-efficacy during care transitions.
RESEARCH DESIGN AND METHODS: We conducted a secondary qualitative analysis of PROJECT ACHIEVE focus groups and key informant interviews with 248 participants (138 care recipients, 110 care partners) across six U.S. health system

BACKGROUND: People who are prescribed opioids for pain management may undergo transitions of care. However, little is known about transitions of care that involve a prescribed opioid, which may have significant consequences for people with chronic pain. The purpose of this concept analysis was to define opioid-related transitions (OrTs) and characterize their attributes, antecedents, and consequences.
METHODS: Rodger's evolutionary method for concept analysis was used to identify key attributes, antecedents, and consequences of OrTs. A comprehensive literature search was conducted in Septembe

Transition from pediatric to adult care systems is a major challenge in the management of adolescents with epilepsy. The comparison of pediatric and adult physicians' points of view on this issue is scarcely described. The aim of this study was to understand pediatric and adult neurologists' experience and opinions on transition in epilepsy in France. We investigate the age at which they usually transfer patients, their opinion on the factors that positively or negatively impact transition, on the help provided during this transition period, and their propositions to improve this process. We p

The survival of patients with advanced cancer, coupled with the increased presence of end-stage chronic illnesses in an aging population, is leading to a demand in palliative care. Due to the ongoing need for acute-pain and symptom control in hospice/palliative care units, few are able to offer long-stay admission for those whose symptoms have stabilized. When a patient no longer requires specialist palliative care services, transfer from an inpatient palliative care facility may then be necessary. A core component of the role of palliative-care social workers involves working with patients an

BACKGROUND: Transition to adult care occurs at age 18 during a vulnerable adolescent period for pediatric renal transplant (RTx) patients.
METHODS: We examined renal allograft loss and hospitalization for RTx biopsy or rejection before and after transition to adult care using clinical and administrative health records of children who underwent RTx (1992-2002) in Ontario, Canada. Life-table analyses examined event rates/100 person years according to age at first RTx.
RESULTS: A total of 115 patients were included (57% men; mean age at first transplant 13.9+/-3.7 years). Allograft loss rates w

BACKGROUND: Transitioning from paediatric to adult healthcare is a pivotal phase for adolescents and young adults (AYAs) with chronic conditions. Effective healthcare transition (HCT) supports continued healthcare engagement and stable psychosocial outcomes, reducing risks for patients, their families, and the healthcare system. Yet the determinants of a smooth transition are complex, shaped by the interplay of various biopsychosocial constructs.
OBJECTIVE: The ExPeCT study (acronym: Exploring Predictors of a successful healthCare Transition) seeks to: A) examine the relationship between biop

BACKGROUND: There are currently no models for the transition of patients with metabolic bone diseases (MBDs) from paediatric to adult care. The aim of this project was to analyse information on the experience of physicians in the transition of these patients in Spain, and to draw up consensus recommendations with the specialists involved in their treatment and follow-up.
METHODS: The project was carried out by a group of experts in MBDs and included a systematic review of the literature for the identification of critical points in the transition process. This was used to develop a questionnai

BACKGROUND: During transition from children's to adults' healthcare, young adults with long-term conditions may show delays in psychosocial development compared to their peers without long-term conditions, and deterioration of their conditions' medical control.
METHODS: This paper integrates the findings, already published in 10 separate papers, of a 5-year transition research programme.
IMPLICATIONS: There is an important role for funders (commissioners) of adults' services to fund transitional healthcare, in addition to funders of children's services who currently take responsibility. It is
