A machine for continuous hypothermic pulsatile perfusion preservation of rat kidneys is described in detail. The use of longwave ultraviolet irradiation during hypothermic pulsatile kidney perfusion using Euro-Collins solution containing 8-MOP did not prolong graft survival compared with untreated controls. Systemic application of 8-MOP to the kidney donor may be necessary for effective action of the combined photochemical treatment as a method of immunoalteration.

The main clinical problems that follow heart-lung transplantation are opportunist infections of the lungs and pulmonary rejection. Of 23 patients undergoing heart-lung transplantation, eight had opportunist infections and 12 had at least one episode of pulmonary rejection. Cardiac rejection occurred in only one patient, who did not need treatment. Of the 12 patients who had pulmonary rejection, nine recovered fully after augmented immunosuppression with high dose corticosteroids, although one patient required additional low dose corticosteroids for eight months before making a full recovery. These experiments were designed to determine whether transplants of fetal spinal cord tissue into lesioned spinal cord in newborn rats provide a terrain that supports the growth of serotonergic (5-HT) axons across the site of the lesion. Although descending serotonergic axons can regenerate after chemical lesions in adult animals, they show little regrowth after surgical lesions. The findings in this series of investigations indicate that the passenger leukocyte concept applies to islet transplants. Six methods have been developed which prevent rejection of islet allografts in rats and mice without requiring the continuous use of immunosuppressive drugs in the recipient. The ultrastructure of the pinealocytes of Snell dwarf mice (dw/dw) was examined morphometrically and compared with that of both phenotypically normal (dw/+ or +/+) animals of the same strain and dwarf mice with ectopic pituitary grafts. Thymic epithelium from neonatal DBA/2 mice (H-2d) was placed under the kidney capsule of 10- to 12-wk-old female NZB/W mice (H-2d/z). Donor epithelium, equivalent to 1-2 lobes of neonatal thymus, was either irradiated (1300 rad.) or cultured for 7 days in order to minimise host-versus-graft reactions. To investigate the influence of matching for HLA-DR antigens in renal transplantation, we assessed the outcome of 170 prospectively HLA-typed cadaveric kidney transplantations performed since 1977 in one center. We found a beneficial effect on graft survival of HLA-DR compatibility between donor and recipient (P < 0.05). A possible effect of matching for the HLA-A and B antigens could be seen only in the HLA-DR-mismatched combinations.
