Mild head injury: a plea for routine early CT scanning.

We reviewed the records of 1538 mild head injury patients admitted during a 4 1/2-year period to the Southern New Jersey Regional Trauma Center. All patients had experienced brief loss of consciousness or amnesia, but had a normal or near normal neurologic examination on admission, with Glasgow Coma Scale (GCS) scores of 13-15 and no focal neurologic deficit. Routine urgent cranial CT scans were obtained on all patients, and correlations between skull fractures and intracranial lesions investigated. Two hundred sixty-five patients (17.2%) harbored 340 lesions on CT scans, of which 131 were fra

Initial resuscitation and assessment of patients with multisystem blunt trauma.

In the vast majority of blunt trauma victims, shock is due to blood loss. Whereas the diagnosis of circulatory collapse is clear-cut, the rapid identification and control of the bleeding source may not be. Such patients often have injuries to several body systems and thus have numerous possible sources of hemorrhage. Ideally, the injury pattern should be defined during resuscitation, preferably by a trauma team using standardized protocols. Airway patency and adequacy of ventilation have top priority. The cervical spine must be immobilized until fracture has been ruled out. In concert with res

ESR Essentials: trauma team and the role of Interventional Radiology-practice recommendations by the Cardiovascular and Interventional Radiological Society of Europe.

Trauma-related hemorrhage remains a leading cause of preventable death, requiring rapid diagnosis and timely intervention. Interventional Radiology (IR) plays a central role in the management of non-compressible bleeding, especially in solid organ injuries and pelvic trauma. This article outlines three key recommendations for integrating IR into trauma care. First, IR must be embedded in trauma teams with 24/7 availability at Level I trauma centers and structured access at Level II and III centers. Second, whole-body contrast-enhanced CT should be performed in hemodynamically stable or initial…

Twelve were level-1 trauma centers, five of which were university hospitals. RESULTS: The response rate was 86.5% (45/52). A protocol of trauma care in the emergency service including MDCT imaging.

Trauma is the principal cause of young peo ple's death. Care should be appropriate to the severity of the injuries and involves a multidisciplinary team around the clock. In this article, we examine a protocol of care based on ATLS (Advanced Trauma Life Support) recom mendations, including advances in imaging technology, which have greatly increased quality care in our institu tion. The protocol is based on patient stability and on injury mechanisms. The challenge is to avoid wasting time in the definitive treatment of lesions. Multi-detector computed tomogra phy (MDCT) has brought complete an

Surgical strategy for femoral shaft fractures in severely injured patients: A 13-year experience from a tertiary trauma centre.

BACKGROUND: The treatment strategy of femoral shaft fractures in polytraumatised patients has evolved over the years and led to improved outcomes for these patients. However, there is still controversy regarding the optimal treatment strategy and surgical care can differ markedly from one country to another. We investigate the surgical treatment strategy (Early Definitive Care (EDC) or Damage Control Orthopaedics (DCO)) implemented in the care of severely injured patients with femoral shaft fractures treated at a single tertiary trauma centre in southern Finland and factors affecting decision

The management of polytraumatized patients in Germany.

Polytrauma care in Germany is well organized and follows clear-cut demands: (1) to reduce the therapy-free interval, (2) to ensure qualified … e and advanced shock and trauma treatment at the scene. The patients then are carried to high level trauma centers by air transport. In the trauma centers, standardized diagnostic and treatment protocols are applied by trauma surgery teams who care for the entire trauma, including orthopaedic trauma. In addition to stopping mass bleeding, basic concepts include …

Medical management of severe head injury: present and future.

The comparative efficacy of various treatment algorithms in improving outcome from severe head injury (SHI) has never been tested in a prospective, randomized, controlled trial. Indeed, there are few hard data on the influence on outcome of most of the individual treatment modalities used alone. The medical management algorithm presented here is an exercise in evaluating the strength of what studies do exist and attempting to balance the relative risk/benefit ratios of the various treatment modalities. This algorithm, based on the information contained in this issue of New Horizons, divides th

Posttraumatic renovascular hypertension after occult renal injury.

OBJECTIVE: Hypertension secondary to renal injury is an unusual problem, but one that occurs with some frequency in an active trauma unit. The incidence and management of posttraumatic renovascular hypertension at our Level I trauma center was reviewed. METHODS: A retrospective review of a trauma database was performed on patients treated by our trauma service between 1977 and 1996. Seven patients were identified who developed arterial hypertension as a direct result of a renal injury. RESULTS: All of the patients sustained multiple injuries, with five requiring celiotomy to control bleeding

Early care of the patient with cervical spine injury.

For almost two decades, great strides have been made in improving the early care of trauma patients both at the trauma scene and in the hospital. Highly skilled providers in the field rapidly extricate, stabilize, and transport patients to trauma centers. In those centers, teams of clinicians can quickly resuscitate, evaluate, and treat life-threatening conditions. These improvements should lead to an increase in survival and functional capacity of all trauma victims, including those with cervical spine injury.

Determining the impact of culture on venous thromboembolism prevention in trauma patients: A Southwestern Surgical Congress Multicenter trial.

INTRODUCTION: Venous thromboembolism (VTE) remains one of the principal causes of morbidity and death in trauma patients that survive the first 24 h. Recent literature on VTE prevention focuses on choice of chemoprophylaxis, specifically unfractionated heparin (UFH) versus low molecular weight heparin (LMWH). This singular focus on a multifactorial process may be inadequate to fully understand the optimal approach to VTE prevention. We hypothesized that variations in care between trauma centers could be used to identify key components of VTE prevention associated with better outcomes. METHODS…

[Costs and proceeds from patients admitted via the emergency room with mild craniocerebral trauma].

The introduction of the diagnosis-related groups (DRG) in 2003 radically changed the billing of the treatment costs. From the very beginning, trauma surgeons questioned whether the introduction of the DRG could have a negative impact on the care of the severely injured. "Trauma centers in need" was the big catchword warning against shortfalls at trauma centers due to the billing via DRG. This situation was confirmed in the first publications after introduction of the DRG, showing a clearly deficient level of care of polytrauma cases. Over the years, adjustments have led to an improvement in th…

Tropical cyclone Fani-perspective from the trauma and emergency department of an affected tertiary hospital.

PURPOSE: To explore the epidemiological and clinical profile of patients admitted to the trauma and emergency department (TED) of a tertiary care hospital due to tropical cyclone Fani and highlight the challenges faced by the hospital in this natural disaster. METHODS: A retrospective study was conducted in the TED in the affected zone. Data of all victims affected by the cyclone Fani on May 3, 2019 were obtained from disaster records and medical case sheets. All patients except death on admission were included. Clinical variables included anatomical sites and severity of injuries which was a

[Missed injuries in pediatric trauma care : Still a challenge].

Missed injuries (MI) in children and adolescents with multiple injuries represent a major challenge in pediatric trauma care despite the availability of standard examination protocols and imaging procedures. In the initial phase of care the primary survey is essentially important for the detection and treatment of life-threatening injuries and is well established. After stabilization of the patient in the trauma room and the first phase of treatment, a secondary survey and a tertiary survey should be carried out in order to detect initially MI in the further course and avoid longer term conseq
