Accurate estimates of Mycobacterium tuberculosis infection in young children provide a critical indicator of ongoing community transmission of M. tuberculosis. INTRODUCTION: The diagnosis of latent tuberculosis infection (LTBI) is currently based on the century-old tuberculin skin test (TST). However a positive reaction can result from infection by Mycobacterium tuberculosis, BCG vaccination or cross-reaction with nontuberculous mycobacteria. We assessed associations between hazardous alcohol use and latent tuberculosis infection (LTBI) among adults living with human immunodeficiency virus (HIV) in Uganda. We compared tuberculin skin test positivity across medium, high, and very-high alcohol use levels, classified by AUDIT-C scores. In multivariable analysis, very high use was associated with LTBI (adjusted odds ratio 1.61, 95% confidence interval: 1.03-2.50).

We found that optimal conditions for the beryllium transformation test required the use of purified lymphocyte suspensions in 20% human serum, with beryllium sulphate at concentrations of 10(-6) to 10(-7) M. Using these conditions, all 15 chronic beryllium patients tested gave a stimulation index of more than 2. Despite the above, 10(-5) and 10(-6)M, BeSO4 inhibited spontaneous thymidine incorporation and lymphocyte transformation to PHA and PPD in normal subjects. Alternative methods, using whole blood, venous and thumb-prick samples, gave fewer and less reproducible positive results.

M. tuberculosis specific interferon-gamma tests have been established lately as an important additional instrument in the diagnosis of the latent tuberculosis infection (LTBI). In several studies these tests had a higher specificity compared to the tuberculin skin test (TST). In this report we describe the case of a woman without increased risk for LTBI and with discordant results for the interferon-gamma tests QuantiFERON-TB Gold In-Tube and T-SPOT.TB and for the TST. The findings suggest repeated false-positive results of the QuantiFERON-TB Gold In-Tube test.

The effect of bilirubin on one of the basic cell-mediated immunity reactions, i.e. the migration inhibition test was assessed. For this purpose the model of tuberculin hypersensitivity in rabbits was employed and the effect of bilirubin was studied on the migration inhibition test and its two main phases; the process of MIF production and the effect of already preformed MIF on the effector system. It was found that the neutralizing effect of bilirubin on the migration inhibition (caused by specific antigen) was due to the effect of bilirubin on the process of MIF production.

This report describes a rare case of solitary pulmonary tuberculoma in a pediatric patient. A 13-year-old male presented with a progressively enlarging solitary nodule in the left lung, identified via chest computed tomography (CT).
