[A man from Africa with a swelling in the head and neck region].

A general dental practitioner can, in the daily practice, be confronted with a patient with a swelling in the head and neck region. For such swellings an extensive differential diagnosis exists. Often such a swelling is caused by one or more enlarged lymph nodes due to a bacterial or viral infection. If a swelling in the head and neck region has been present for some time--longer than 4 weeks--then there is a considerable chance, especially in adults, that it is the result of the metastasizing of a malignancy, such as a squamous cell carcinoma in the oral mucosa or another mucosal site in the

Rifampicin causes false-positive immunoassay results for urine opiates.

The treatment of tuberculosis usually includes the antibiotic rifampicin, especially in patients with concomitant human immunodeficiency virus infection. Some of these patients are in withdrawal therapy for drug abuse. When opiate screening is carried out in patients receiving rifampicin, false positive results are detected with the kinetic interaction of microparticles in solution method. We evaluated this interference in a Cobas-Integra analyzer and found a 12% cross-reactivity of rifampicin for antibiotic concentrations ranging from 0.19 to 6.08 mumol/l (156 to 5000 micrograms/l). This effe

The hamster cheek pouch: an immunologically privileged site suitable to the study of granulomatous infections.

The hamster cheek pouch is an invagination of oral mucosa, characterized histologically as skin-like. In this paper we describe anatomical, histological and embriological features of the pouch and comment on the pouch as an immunologically privileged site since it lacks lymphatic drainage and has few Langerhans cells. We present the review from literature and our observations after inoculation in the pouch of mycobacteriae (BCG, Mycobacterium tuberculosis and Mycobacterium leprae) and a fungus (Paracoccidioides brasiliensis). Lesions in the pouch were granulomatous but smaller and long lasting

Ketoconazole: a new drug for the treatment of paracoccidioidomycosis.

Ketoconazole was administered orally to 13 patients with paracoccidioidomycosis. Therapy consisted of 200 mg per day for periods ranging from three to 12 months. All patients improved markedly, with healing of pulmonary, mucosal, and skin lesions. Patients exhibiting the latter two manifestations began to improve two weeks after therapy, and healing occurred within three months. Pulmonary lesions regressed at a slower pace and healed by fibrosis. Mycologic tests have corroborated the clinical improvement. Adverse effects were not observed; monitoring of renal, hematologic, and hepatic function

High dose oral rifampicin to improve survival from adult tuberculous meningitis: A randomised placebo-controlled double-blinded phase III trial (the HARVEST study).

Background: Tuberculous meningitis (TBM), the most severe form of tuberculosis (TB), results in death or neurological disability in >50%, despite World Health Organisation recommended therapy. Current TBM regimen dosages are based on data from pulmonary TB alone. Evidence from recent phase II pharmacokinetic studies suggests that high dose rifampicin (R) administered intravenously or orally enhances central nervous system penetration and may reduce TBM associated mortality. We hypothesize that, among persons with TBM, high dose oral rifampicin (35 mg/kg) for 8 weeks will improve survival compa

Intravenous Steroid Days and Predictors of Early Oral Steroid Administration in Tuberculous Meningitis: A Retrospective Study.

Intravenous (IV) dexamethasone is recommended for 14 days in stage 1 and 28 days in stage 2/3 tuberculous meningitis (TBM). We used a different steroid protocol. We shifted TBM patients to oral steroids after 48 hours of sustained improvement on IV steroids (oral group). Patients who worsened after shifting to oral steroids were reinitiated on IV steroids. Once they showed a consistent improvement for 48 hours, the IV steroids were overlapped with oral steroids for 7-10 days to taper off IV steroids (overlap group). We compared total IV steroid days in our patients with the recommended treatme

Extrapulmonary tuberculosis: atypical presentation in otorhinolaryngology.

Tuberculosis (TB) continues to be a major health burden globally more so in low/middle-income countries like India. There is an increase in the prevalence of extrapulmonary TB (EPTB) because of HIV epidemics and increased usage of immunomodulating drugs. EPTB constitutes 15%-20% of all patients with TB and >50% of HIV-TB coinfected patients. We present three such atypical presentations of EPTB in head and neck region. EPTB can mimic any disease, hence knowledge of the unusual presentations helps in making early diagnosis and thereby reduces the morbidity and mortality involved with the disease

Antifibrinolytic therapy to reduce haemoptysis from any cause.

BACKGROUND: Haemoptysis is a common pathology around the world, occurring with more frequency in low-income countries. It has different etiologies, many of which have infectious characteristics. Antifibrinolytic agents are commonly used to manage bleeding from different sources, but their usefulness in pulmonology is unclear. OBJECTIVES: To evaluate the effectiveness and safety of antifibrinolytic agents in reducing the volume and duration of haemoptysis in adult and paediatric patients. SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) and the Database

Oral re-vaccination of Eurasian wild boar with Mycobacterium bovis BCG yields a strong protective response against challenge with a field strain.

BACKGROUND: Field vaccination trials with Mycobacterium bovis BCG, an attenuated mutant of M. bovis, are ongoing in Spain, where the Eurasian wild boar (Sus scrofa) is regarded as the main driver of animal tuberculosis (TB). The oral baiting strategy consists in deploying vaccine baits twice each summer, in order to gain access to a high proportion of wild boar piglets. The aim of this study was to assess the response of wild boar to re-vaccination with BCG and to subsequent challenge with an M. bovis field strain. RESULTS: BCG re-vaccinated wild boar showed reductions of 75.8% in lesion scor

Pediatric salivary gland lesions.

The salivary glands comprise three main pairs of glands (ie, the parotid, the submandibular, and the sublingual) and a number of minor glands found in the mucosa of the upper aerodigestive tract. Lesions may be inflammatory or obstructive in nature or may stem from granulomatous or neoplastic disease. As such, establishing a definitive diagnosis is often quite challenging. This article reviews widely used diagnostic approaches and briefly describes various salivary gland lesions within an etiologic framework.

Short course of clarithromycin in an immunocompetent patient with BCG-induced regional complications.

Bacillus Calmette Guerin (BCG) has been used to vaccinate against tuberculosis since 1921. Persistant skin and lymph node lesions yielding Mycobacterium bovis are a rare complication for which there is not standardized treatment. We report an 11 month old child with a progressive cutaneous nodule and lymphadenopathy after BCG vaccination. These rapidly cleared with administration of clarithromycin.

[Urinary excretion of rifampicin in kidney failure when administered alone or together with isoniazid].

Rifampicin excretion with urine and its plasma levels were studied in 46 patients with nephrophthisis treated with rifampicin alone or in combination with isoniazid in doses of 8--10 mg/kg bw with regard to the degree of renal insufficiency. It was shown that excretion of rifampicin with urine decreased with progression of renal insufficiency and had no effect on its plasma levels. The combined use of isoniazid and rifampicin in a single dose was accompanied by a decrease in the latter's excretion with urine and different changes in its plasma concentration. The use of the two drugs for 4--6 w

Molecular determinants of multidrug-resistant tuberculosis in Sierra Leone.

UNLABELLED: Multidrug-resistant tuberculosis (MDR-TB) management has become a serious global health challenge. Understanding its epidemic determinants on the regional level is crucial for developing effective control measures. We used whole genome sequencing data of 238 of Mycobacterium tuberculosis complex (MTBC) strains to determine drug resistance profiles, phylogeny, and transmission dynamics of MDR/rifampicin-resistant (RR) MTBC strains from Sierra Leone. Forty-two strains were classified as RR, 196 as MDR, 5 were resistant to bedaquiline (BDQ) and clofazimine (CFZ), but none was found to

Evaluating newly approved drugs in combination regimens for multidrug-resistant tuberculosis with fluoroquinolone resistance (endTB-Q): study protocol for a multi-country randomized controlled trial.

BACKGROUND: Treatment for fluoroquinolone-resistant multidrug-resistant/rifampicin-resistant tuberculosis (pre-XDR TB) often lasts longer than treatment for less resistant strains, yields worse efficacy results, and causes substantial toxicity. The newer anti-tuberculosis drugs, bedaquiline and delamanid, and repurposed drugs clofazimine and linezolid, show great promise for combination in shorter, less-toxic, and effective regimens. To date, there has been no randomized, internally and concurrently controlled trial of a shorter, all-oral regimen comprising these newer and repurposed drugs suf

Anti-inflammatory, antioxidant and anti-Mycobacterium tuberculosis activity of viridiflorol: The major constituent of Allophylus edulis (A. St.-Hil., A. Juss. & Cambess.) Radlk.

ETHNOPHARMACOLOGICAL RELEVANCE: The leaves of Allophylus edulis (A. St.-Hil., A. Juss. & Cambess.) Radlk. The content of essential oil in A. edulis may be the chemical basis to explain its ethnobotanical uses, since infusions of this plant are used to treat inflammation in the traditional medicine in Brazil. AIM OF THE STUDY: This study evaluated the anti-inflammatory, antioxidant and anti-mycobacterial activities of the essential oil (E

Pharmacokinetics of dexamethasone in tuberculosis meningitis.

INTRODUCTION: Dexamethasone is recommended as adjunctive therapy for tuberculosis meningitis (TBM). Co-administration with rifampicin is expected to reduce dexamethasone exposure in TBM, an effect that may be more pronounced with the higher rifampicin doses currently being evaluated in clinical trials. METHODS: This pharmacokinetic study was nested in a randomised controlled trial comparing the safety of high-dose rifampicin (oral, 35 mg/kg; intravenous, 20 mg/kg) plus linezolid, with or without aspirin, vs standard-dose rifampicin (10 mg/kg) for adults with HIV-associated TBM. Potential long-acting tuberculosis treatment for people with HIV: cost-effectiveness benchmarks.

RATIONALE: Long-acting tuberculosis (TB) drugs could improve treatment outcomes in South Africa, where there is high prevalence of HIV and TB coinfection and high attrition from standard TB treatment, but potential benefits must be weighed against costs. OBJECTIVES: To identify scenarios where a long-acting TB treatment would provide clinical benefit and be cost-effective compared with standard TB treatment for people with HIV in South Africa. METHODS: We used a microsimulation model to compare clinical and cost outcomes of (1) standard-of-care 6-month oral TB treatment (SOC); and (2) a long

Association of linezolid interruption and rechallenge with clinical outcomes in patients with rifampicin-resistant tuberculosis in South Africa, 2018-22: an observational cohort study.

BACKGROUND: Linezolid is a key component of treatment regimens for rifampicin-resistant or multidrug-resistant tuberculosis. Toxicity-related linezolid interruption is not uncommon, but little is known about its effect on treatment-related outcomes. Furthermore, there are no data on the frequency of successful rechallenge and its associated predictors. This study aimed to ascertain the relationship between linezolid discontinuation and treatment outcome, and the likelihood of successful rechallenge in patients who discontinued linezolid. METHODS: We conducted an observational cohort study to

Mediastinal tuberculous lymphadenitis causing a broncho-mediastinal-oesophageal fistula.

Mediastinal tuberculous lymphadenitis and tuberculous fistulae are recognised complications of pulmonary tuberculosis (TB); however, we could find no documented cases of broncho-mediastinal-oesophageal fistulae. We describe the case of a patient in their early 20s from a South Asian country with endemic TB who presented to a UK hospital with a productive cough, fevers, reduced appetite, inter-scapular pain and feeling generally unwell. They were initially diagnosed with community-acquired pneumonia but their symptoms were refractory to antibiotic therapy. Following CT and endoscopy with biopsie
