BACKGROUND Traditional diagnostic methods for tuberculosis (TB) cannot be reliably applied to tuberculous pleurisy. Therefore, this prospective, randomized, controlled trial was performed to compare the diagnostic sensitivity and safety of ultrasound-guided cutting-needle pleural biopsy versus thoracoscopic pleural biopsy in patients suspected of tuberculous pleurisy following inconclusive thoracentesis. MATERIAL AND METHODS A total of 196 adult patients with acid-fast bacillus (AFB)-negative exudative pleural effusions clinically suspected of tuberculous pleurisy were recruited. Enrollees wer

Pneumothorax is a frequent cause of admission in an emergency department. It can be due to a leakage of air from an air-filled lung cavitation into the pleural space. We report the unusual case of pneumothorax in a patient with a pulmonary cavitary infectious process mimicking tuberculosis. A 30-year-old asthmatic man, treated for several years with low-dose inhaled corticosteroids, presented a complete left tension pneumothorax and chronic necrotising pulmonary aspergillosis that mimicked initial pulmonary tuberculosis. Antifungal treatment by voriconazole was started and continued for 1 year

Children with evidence of tuberculous disease registered at the TB Clinic, Institute of Child Health, Madras during the years 1977 to 1992 were analyzed. Progressive primary complex, is the commonest thoracic form of tuberculosis while tuberculous meningitis is the commonest extra thoracic form. The overall prevalence of various clinical forms of tuberculosis has decreased over the last 16 years. There is an increasing trend in the prevalence of progressive primary complex among the BCG vaccinated group. The prevalence of pleural effusion, bone tuberculosis and abdominal tuberculosis is almost

We mention the major pathogenous characteristics of a tuberculosis infection. Immunity to tuberculosis and tuberculin allergy are connected with each other but are not identical reactions of the organism. We describe the well-known complications in primary infections, namely the primary focus and the formation of lymph nodes. The peripheral primary focus generally regresses after treatment. However, the hilar lymph nodes sometimes increase in size even after appropriate medication (autonomous activity of some lymph nodes). If x-ray films reveal segmental changes and if there is evidence of vir

Pulmonary artery aneurysm is a rare condition accounting for less than 1% of all intra thoracic aneurysms. This paper reports a case of giant aneurysm of the trunk and branches of the pulmonary artery, which was discovered incidentally on a computed tomography angiogram in a 48-year-old man with a past history of pulmonary tuberculosis. Besides the aneurysm there was dilation of the pulmonary annulus with massive pulmonary regurgitation, a tight mitral stenosis and sequelae of pleural and pulmonary tuberculosis. The patient is being followed-up with medical treatment and has been stable clinic

We reviewed the radiologic features of 19 children and 26 adults with miliary tuberculosis. Children had a very high prevalence of associated lymph node enlargement compared to the adults (95% vs. 12%). Pulmonary consolidation was more common in children (42% vs. 12%). Pleural effusions were uncommon in both groups. The miliary pulmonary lesions cleared faster in children. Evidence of previous tuberculosis was seen in 31% of the adults but none of the children. Most of these differences in radiologic features between children and adults probably represent differences in pathogenesis for hemato

A combined ++clinico-roentgenological+ examination of 367 patients with a pleurisy syndrome was aimed at the study of a potential establishment of the etiologic diagnosis of the disease. Examination comprised the routine tests and findings of various types of biopsy, including pleural biopsy performed within the first two months from disease onset. The etiologic diagnosis of tuberculous pleurisy was most difficult in 96% of the patients who exhibited prolonged and stable maintenance of exudation or, on the contrary, very rapid regression of the pleural exudate without residual changes. To determine if patients who had lymphocyte-rich pleural effusion and a pleural biopsy without any specific findings could be histopathologically differentiated between those with tuberculous and nontuberculous pleuritis, we histologically re-evaluated the pleural biopsies of all patients whose pleural effusion was predominant with lymphocytes and contained no malignant cells. A total of 40 patients with a nonspecific histologic findings of pleural biopsy specimen were categorized based on their ultimate diagnosis as having tuberculous (n = 15), carcinomatous (n = 10) or nontuberculous, benign

A patient with a 5-month history of pericardial effusion and unsuccessful steroid treatment was found at pericardial exploration to have thickened pericardium adhered to the heart. After anterior pericardiectomy, histologic examination revealed severe granulomatous pericarditis resulting from infection with Mycobacterium tuberculosis. Despite the pericardiectomy and antituberculous therapy, the patient continued to have symptoms, including bilateral pleural effusions, 4 days after discharge from the hospital. After a second exploration (after echocardiography revealed decreased left ventricula

INTRODUCTION: Medical thoracoscopy (MT) is a minimally invasive procedure primarily used for diagnosing pleural diseases. However, its therapeutic role, especially in complex pleural infections, is underexplored. Rising pleural disease incidence and the need for less invasive treatment options have spurred interest in enhancing MT's capabilities. Disposable articulated wristed instruments, initially designed for laparoscopic surgery, could offer improved dexterity and precision, potentially expanding MT's therapeutic use.

INTRODUCTION: Differentiating between malignant pleural effusion (MPE) and tuberculous pleural effusion (TPE) remains challenging in clinical practice. The cancer ratio plus (CR+), a potential diagnostic tool calculated as serum lactate dehydrogenase/(pleural adenosine deaminase x pleural lymphocyte percentage) has emerged to address this diagnostic challenge. This scoping review maps the available evidence on its diagnostic performance.
MATERIALS AND METHODS: We conducted a systematic search of PubMed, Scopus, and Web of Science databases from inception to April 2025. Eligible studies assess

RATIONALE: Although it is well recognized that CD4(+) T cells and T helper (Th) 1 cytokines are critical in the cell-mediated response to Mycobacterium tuberculosis, it is also clear that this immunity alone is not enough. Understanding the roles of other T cell subsets and cytokines is essential for vaccine design and clinical immunotherapy against tuberculosis (TB).
OBJECTIVES: To investigate the clinical significance and possible regulatory mechanism of Th17 responses in human TB.
METHODS: The frequencies of IFN-gamma-, IL-4-, IL-17-, FoxP3- and IL-6 receptor (IL-6R)-expressing CD4(+) T c

BACKGROUND: Pleural effusion in children is an important cause of respiratory morbidity with diverse infective and non-infective etiologies. Clinical presentation, biochemical profile, microbiological findings, and imaging characteristics play a major role in determining etiology, severity, treatment strategy, and overall outcome in pediatric pleural effusion.
OBJECTIVES: The study aimed to evaluate the clinico-etiological profile and outcomes of pleural effusion in children admitted to a tertiary care hospital.
METHODS: This cross-sectional study was conducted in the pediatric ICU and pedia

Evaluating human immune response to defined Mycobacterium tuberculosis antigens in patients with different clinical forms of tuberculosis may help in elucidating pathogenesis and in vaccine development. In the present report we evaluated the lymphocyte proliferation, cytokine production and natural killer cell cytotoxicity as parameters to screen four mycobacterial recombinant antigens. Pleural fluid mononuclear cells (PFMC) and peripheral blood mononuclear cells (PBMC) from 13 HIV-negative patients with tuberculous pleurisy, living in a tropical region of Brazil were used in these assays. Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder characterised by widespread inflammation affecting multiple organ systems. Its varied clinical manifestations often complicate diagnosis, as it can closely mimic other conditions, particularly infectious diseases such as tuberculosis. This case report describes a woman in her early 20s initially suspected of having disseminated tuberculosis and treated accordingly due to overlapping symptoms, including fever, night sweats, lymphadenopathy and bilateral exudative pleural effusion. Subsequent evaluation, however, confirmed a dia

We reviewed the radiological findings in 55 hospital inpatients aged 60 and over with bacteriologically proven tuberculosis over a 5-year period 1981-5. Since most of the patients were infected during childhood it was expected that the majority would show the 'typical' findings of post-primary disease with apicoposterior lung lesions. This was found in 43 of the patients (78%). Unusual features were found in 12 patients: solitary pleural effusion (1), miliary tuberculosis (2), tuberculoma (1), isolated mediastinal or hilar lymphadenopathy (1), solitary ventrobasal and middle-lobe or lingular l

A 50-year-old female presented with fever, symmetrical arthralgias, rash, painful oral ulcerations and alopecia since 8 weeks. Examination showed mild hepatospleenomegaly. Investigations revealed leucocytosis, neutrophilia, elevated sedimentation rate and raised ferritin levels (3850 ng/ml). Computerized tomography (CT) abdomen showed hepatospleenomegaly, mild ascitis and mild bilateral pleural-effusion. After ruling out occult infections, tuberculosis, malignancies and autoimmune diseases by appropriate investigations, and due to raised ferritin levels, adult onset stills disease (AOSD) was d

Human brucellosis is an imported zoonosis extremely uncommon in Japan. Brucellosis was found in a 38-year-old surgeon who had never been abroad; he developed intrapulmonary infiltrates and pleural thickening indistinguishable from pulmonary tuberculosis. Despite extensive antibiotic therapy for tuberculosis he developed systemic serositis. A culture of resected lung tissues grew CO2-required coccobacilli. Polymerase chain reaction test using a specific primer pair for Brucella abortus revealed that the isolated pathogen is highly homologous to B. abortus. We emphasize that undetermined cases o
