[Current viewpoints on osteoarticular tuberculosis].

Nowadays the osteoarticular tuberculosis is a disease of adult and senile age. Among my own patients were most frequently affected the spinal column (55.7%), the hip-joint (10.1%) and the knee-joint (9.4%). If other diagnostic methods fail, to the interest of an early ascertainment of the diagnosis the performance of a test excision should not be delayed. On modern conditions a conservative treatment is sufficient in most cases and achieves good results. Here, the chemotherapy follows the usual directives, in which cases on account of the lacking proof of causative organisms frequently must be

Infections of the spinal subdural space in children: a series of 11 contemporary cases and review of all published reports. A multinational collaborative effort.

INTRODUCTION: Positioned anatomically between the spinal epidural space and the intramedullary compartment, the spinal subdural space remains the least common area of localized infection in the central nervous system. Infectious processes of the subdural spinal space include subdural spinal empyema, subdural spinal abscess, infected spinal subdural cyst, and infectious spinal subdural cyst. To date, there has been no systematic review of these entities in children, with the cumulative knowledge of the pathophysiologic, microbiologic, and demographic characteristics of these infections relegate

[Primary anterior focus debridement and bone autograft with internal fixation via transperitoneal approach for tuberculosis of lumbosacral junction].

OBJECTIVE: To evaluate the clinical outcomes of primary anterior focus debridement, bone autograft, and internal fixation via transperitoneal approach in treating tuberculosis of the lumbosacral junction. METHODS: From February 2002 to April 2007, 16 patients with tuberculosis of the lumbosacral junction underwent anterior radical debridement, autologous iliac bone graft (two pieces of iliac bone, 5 cm x 3 cm in size), and internal fixation via transperitoneal approach, including 4 males and 12 females aged 27-63 years old (average 38 years old). The course of the disease ranged from 6 to 18

Tuberculosis of spine: neurological deficit.

The most dreaded neurological complications in TB spine occur in active stage of disease by mechanical compression, instability and inflammation changes, while in healed disease, these occur due to intrinsic changes in spinal cord secondary to internal salient in long standing kyphotic deformity. A judicious combination of conservative therapy and operative decompression when needed should form a comprehensive integrated course of treatment for TB spine with neurological complications. The patients showing relatively preserved cord with evidence of edema/myelitis with predominantly fluid colle

Intradural extramedullary tuberculoma in a case of disseminated tuberculosis: A case report.

BACKGROUND: Intradural extramedullary tuberculoma of the spinal cord (IETSC) is an exceedingly rare manifestation of tuberculosis (TB) affecting the central nervous system. CASE DESCRIPTION: A 33-year-old immunocompetent female with disseminated TB, including pulmonary involvement and leptomeningeal tuberculomas, developed progressive paraplegia and urinary incontinence over 2 months. Magnetic resonance imaging revealed diffuse intradural extramedullary soft tissue from C7 to L2 vertebral levels, indicative of abscess formation and severe spinal cord compression. The surgical intervention inv

One-Stage Posterior Vertebral Column Resection in the Treatment of Adolescent Thoracic and Lumbar Tuberculosis Complicated With Severe Kyphotic Deformity.

BACKGROUND: Our study aims to investigate the clinical outcome of 1-stage posterior vertebral column resection (PVCR) for adolescent thoracic and lumbar tuberculosis with severe kyphotic deformity (Cobb angle≥60°). METHODS: Between January 2008 and January 2016, we recorded 16 (9 male, 7 female) adolescent cases of thoracic and lumbar tuberculosis complicated with severe kyphotic deformity treated by 1-stage PVCR (average age: 15.38 ± 1.54 years; range: 13-18 years). The Cobb angle of kyphosis was 64.56° ± 3.41°. According to the American Spinal Injury Association (ASIA) classification, all p

Evolution of human tuberculosis: a systematic review and meta-analysis of paleopathological evidence.

Tuberculosis is a re-emerging disease and is a major problem in both developing and developed countries today. An estimated one third of the world's population is infected and almost two million people die from the disease each year. Bone lesions occur in 3-5% of active tuberculosis cases and can be used to diagnose the disease in ancient skeletal remains. A meta-analysis was conducted on 531 palaeopathological tuberculosis cases from 221 sites (7250 BCE to 1899) on all continents for the purpose of testing two hypotheses; (1) the frequency of bone lesions does not change through time and (2)

Spectrum of nontraumatic myelopathies in Ethiopian patients: hospital-based retrospective study.

STUDY DESIGN: This is a retrospective hospital-based study. OBJECTIVES: The study aimed at a better understanding of the etiology, clinical presentation and treatment outcome of nontraumatic myelopathies in Ethiopian patients. SETTING: Etiologies of nontraumatic myelopathies have not been evaluated extensively in most sub-Saharan African countries. The available studies in this region were conducted before the widespread clinical use of modern neuroimaging modalities. This study was conducted in Addis Abba, Ethiopia. METHODS: We retrospectively analyzed medical files of patients with a diag

Tuberculous aortoarteritis.

Tuberculous aortoarteritis is a distinct entity. Despite the still wide prevalence of active tuberculosis in developing countries, tuberculous aortoarteritis appears to be rare. The vessel is often involved by a direct extension of the disease from adjacent tuberculous tissue. Occasionally it may result from blood-borne seedlings from an active distant focus. True and false aneurysms are the common manifestations. Stenosing and/or constricting types of lesions and perivascular fibrosis have been encountered by us. The probable pathogenesis is discussed with illustrative cases.

Transpedicular drainage of Pott's abscess. A report of two cases.

The treatment of Pott's disease remains controversial. The authors present two cases of spinal tuberculosis treated with posterior stabilization and supplementary abscess evacuation via the pedicular route with subsequent chemotherapy. The aim of this procedure was to increase the healing rate. Radiographic evidence of bone healing without any sequelae was demonstrated at 13 and 14 months.

Vertebra plana with paraplegia in a middle-aged woman caused by B-cell lymphoma: a case report.

Vertebra plana is a rare presentation of spinal lymphoma. When radiological picture of a patient of paraplegia presents vertebra plana, diagnosis becomes a challenge. In a developing country like India tuberculosis should also be a consideration. Even histology sometimes fails to conclude a diagnosis. Immunohistochemistry is of immense help in clinching a diagnosis.

Cutaneous Tuberculosis With Multisystem Involvement in an Immunocompetent Refugee.

Cutaneous tuberculosis (CTB) is an uncommon skin infection that can mimic other dermatologic conditions and delay recognition. Herein, we present a case of TB chancre, a form of CTB caused by exogenous inoculation. A 19-year-old immunocompetent male refugee from Sudan presented with a five-month history of non-healing wounds on his right shoulder and three weeks of bilateral lower extremity weakness. Diagnostic examination revealed the presence of a spinal epidural abscess and lung consolidation, with culture of the spinal abscess positive for Mycobacterium tuberculosis (MTB). Skin biopsy of t

Infections of the spine: what the orthopedist needs to know.

Infections of the spine usually present with back pain in the absence of constitutional symptoms. Diagnosis requires a high index of suspicion and appropriate radiological and laboratory studies. Most infections can be managed conservatively with organism-specific antibiotics and bracing with good success. Surgery may be indicated to isolate the offending organism and to manage neurological and structural complications.
