Simple patch closure is a treatment option for perforated duodenal ulcer. A 46-year-old male was admitted with evidence of perforation of a duodenal ulcer. The decision was made to carry out laparoscopy to confirm the diagnosis and to close the perforation. This was accomplished without difficulty by suture closure of an anterior perforation and application of an omental patch. The patient had an uneventful recovery. The pathologic features and prognosis of patient in whom gastric cancer simulates at endoscopy as a benign gastric ulcer has been poorly characterized. A retrospective study of 80 patients with biopsy-proven solitary rectal ulcer (SRU) was conducted to review its clinical spectrum. The median follow-up was 25 months. The female-to-male ratio was 1.4:1.0, and the mean age was 48.7 years (range, 14-76 years). Principal symptoms were bowel disturbances (74 percent) and rectal bleeding (56 percent). The use of nonsteroidal anti-inflammatory drugs (NSAIDs) is associated with an increased risk of gastric and duodenal ulcers, especially in patients with previous ulcer disease, heavy smokers, patients who are taking steroids and those with other illnesses. During the years 1973-1985, 145 patients with acute peptic ulcer haemorrhage were treated surgically at the Department of Surgery, Turku University Central Hospital. The bleeding site was gastric ulcer in 76 patients; 58 of these were treated by gastric resection and 17 by ulcer excision combined with vagotomy and pyloroplasty. Twelve patients developed perforation after haemorrhage in a consecutive series of 840 cases admitted for bleeding chronic peptic ulcer. Sucralfate made in Bulgaria (Farmaphim) and its Yugoslavian analogue venter have been tried in gastroduodenal ulcer. Among 7015 upper gastrointestinal endoscopies done during five consecutive years (1984-1988) 642 patients were diagnosed as having benign gastric ulcer. Malignancy in chronic wounds is not uncommon and has been reported as occurring in chronic scars of diverse causes. We report an unusual case of bilateral thermal injuries of the lower extremities that 40 years later presented as very large, painful, fungating wounds of both lower legs. After histologic confirmation of squamous cell carcinoma, eradication of the disease required amputations of both the lower extremities and reconstructive procedures. The article discusses the results of treatment of 256 patients with gastroduodenal hemorrhages. The routine actively temporizing strategy was employed in the treatment of the first group of patients. The total mortality in the group was 13%, postoperative one, 33%. In the second group active surgical strategy was based on the results of recurrent hemorrhage prognosis. The total mortality was 6%, postoperative one, 6.5%. It is concluded that active surgical strategy which is based on prediction of recurrent hemorrhage yields better results of management of patients with ulcerative hemorrhages.
